Breastfeeding medicine decisions compare infant exposure through milk with the consequences of leaving maternal pain or infection inadequately treated. Transfer depends on maternal concentration, protein binding, lipid solubility, half-life, oral absorption by the infant and infant clearance. The youngest, premature and medically unstable infants have less physiological reserve, so SPS compatibility statements for full-term healthy infants cannot be extrapolated automatically.
For common breast infection, penicillins and selected macrolides are compatible with continued feeding, while the agent and duration follow the diagnosed infection and local resistance guidance. Paracetamol and ibuprofen are preferred simple analgesics. Compatibility does not mean absence of all risk: maternal contraindications remain, infants are monitored for specific effects and codeine is avoided because variable conversion to morphine can cause unpredictable infant toxicity. Flucloxacillin
The NHS Dumfries & Galloway adult bacterial-mastitis example is flucloxacillin 500 mg by mouth four times a day for 10–14 days. Swallow the selected 500 mg capsule with 250 mL water, at least one hour before or two hours after meals, and do not lie down immediately after taking it.Exclude flucloxacillin/product or beta-lactam hypersensitivity, including serious delayed reactions, and previous flucloxacillin-related jaundice or hepatic dysfunction. CrCl under 10 mL/min requires individual consideration of a reduced dose or extended interval before prescribing this schedule; do not add a dialysis replacement dose. Review existing hepatic disease, methotrexate, probenecid, warfarin/INR and possible loss of voriconazole effect. Flucloxacillin with paracetamol warrants particular caution in severe renal impairment, sepsis or malnutrition because of high anion gap metabolic acidosis; use the risk-conditioned monitoring and stopping advice in the paracetamol card. Higher flucloxacillin doses or additional hypokalaemia risks warrant potassium monitoring. For a healthy term breastfed infant, watch for diarrhoea, thrush, rash and feeding change.
Paracetamol
500–1,000 mg orally when required, with at least four hours between doses and no more than four doses or 4 g in 24 hours for a standard adult.Avoid product hypersensitivity and concurrent duplicate paracetamol products; calculate the full 24-hour exposure, then choose one coherent regimen. Seek an individual maximum for low weight, malnutrition, chronic alcohol excess, liver or kidney impairment, and review prolonged daily use and warfarin. In a patient also receiving flucloxacillin, severe renal impairment, sepsis or glutathione depletion increases high anion gap metabolic acidosis risk, particularly with maximal or prolonged paracetamol use: review the combination and arrange close clinical/acid-base monitoring when this risk is present. Suspected pyroglutamic acidosis requires prompt paracetamol cessation and urgent assessment; urinary 5-oxoproline can help the investigating team. This is not a routine testing requirement or a prohibition of short-course coadministration in a healthy parent. Seek urgent advice after excess dosing even if initially well.
Ibuprofen
For the selected 200 mg coated-tablet product, take 200–400 mg orally with or after food up to three times daily as needed. Leave at least four hours between doses and do not exceed 1,200 mg in 24 hours. Use the smallest effective dose briefly and seek review for persistent or worsening symptoms.Do not use with product/aspirin/NSAID hypersensitivity, including triggered asthma or urticaria; previous NSAID-related GI bleeding or perforation; active or recurrent peptic ulcer/haemorrhage; increased bleeding tendency; or severe cardiac, hepatic or renal failure. It is contraindicated in the third trimester of pregnancy. Other renal, liver or cardiovascular disease and dehydration require an individual decision and appropriate monitoring. Avoid another NSAID and check anticoagulants, antiplatelets, corticosteroids, SSRIs, ACE inhibitors/ARBs, diuretics, lithium and methotrexate. Stop for GI bleeding or serious skin/allergic reactions. SPS supports ibuprofen during breastfeeding of a healthy term infant, with observation for vomiting or diarrhoea; pain relief must not delay reassessment of worsening infection.

- Review maternal infection trajectory at about 48 hours and arrange ultrasound, culture or admission if fever, erythema or focal mass is not improving.
- Ask about maternal nausea, diarrhoea, rash, jaundice, wheeze, gastrointestinal bleeding, reduced urine or other agent-specific harm.
- During penicillin therapy, monitor a healthy term infant for diarrhoea, thrush, rash and feeding difficulty and seek specialist advice for vulnerable infants.
- Recalculate total daily analgesic doses at every review, including combination cold, headache and prescription products, before recommending escalation.

Milk level is not infant dose
A medicine present in milk may be poorly absorbed orally or rapidly cleared. Risk assessment uses relative infant dose, toxicity and infant maturity rather than detection alone.
Codeine response is unpredictable
Maternal CYP2D6 variation changes morphine production, creating a risk of infant sedation and respiratory depression that cannot be controlled reliably by ordinary dose selection.
Feed timing rarely solves risk
For short-half-life compatible agents, elaborate pumping and dumping may reduce supply without meaningful benefit. Choose the correct medicine and infant monitoring plan instead.
Source control outranks escalation
A persistent abscess will not resolve through broader or longer antibiotics alone. Ultrasound and drainage are more important than accumulating antimicrobial exposure.
