No biomarker should be read in isolation. ER positivity identifies a target but does not show whether chemotherapy adds value; HER2 positivity changes biological therapy but does not determine stage; triple-negative status removes those targets while opening current phenotype-specific pathways. Treatment decisions integrate anatomy, grade, response, genomic testing where commissioned, age, organ function, reproductive goals and the patient’s tolerance for benefit and harm.

- 1For one to three positive nodes, HTG719 lists EndoPredict, Oncotype DX and Prosigna as options alongside clinical risk for postmenopausal women. It also permits consideration for men and trans, non-binary or intersex people using clinical judgement about suitability and hormonal profile, while retaining each test’s intended use. Do not extrapolate the node-positive recommendation to four or more positive nodes.
- 2Do not use these three tests to guide adjuvant chemotherapy for premenopausal women with one to three positive nodes. MammaPrint is not recommended in the one-to-three-node population. A low expression result must not be used to override this premenopausal restriction.
- 3For node-negative disease, including micrometastatic disease, the three tests can be used while more evidence is generated only when a validated tool such as Predict or the Nottingham Prognostic Index places distant-recurrence risk in the intermediate group, and clinicians and companies provide timely, complete, linkable record-level data to the National Cancer Registration and Analysis Service. MammaPrint and IHC4+C should not guide chemotherapy decisions in this node-negative population.
- 4Check the test’s intended purpose: EndoPredict requires ER-positive disease; Oncotype DX allows ER- or PR-positive disease; Prosigna allows ER- or PR-positive disease only in women who have been through menopause. Confirm the patient will use the result to decide about chemotherapy with the clinician, the agreed NHS access discount, and the processing laboratory’s UK national external quality-assurance participation.

Record a shared chemotherapy decision rather than converting an isolated score into an automatic prescription. The test does not determine surgical margins, replace nodal staging or remove independent indications for endocrine or local treatment.

- Before any systemic regimen, verify phenotype, indication, treatment intent, performance, blood count, organ function, interactions, pregnancy and fertility discussions.
- During cytotoxic therapy, use the oncology urgent-contact pathway for fever or sepsis and track marrow, neuropathy, mucosal, cardiac and organ-specific toxicity.
- During endocrine therapy, review adherence, menopausal and sexual effects, thrombosis or abnormal bleeding with tamoxifen, and bone or joint effects with aromatase inhibition.
- During HER2-directed treatment, follow the selected product and oncology protocol for cardiac function and respond to meaningful decline or heart-failure symptoms.

A patient needs to know how many recurrence events treatment may prevent at their baseline risk, not only the relative reduction reported in trials.
Genomic is not germline
Expression profiling of tumour tissue predicts recurrence or chemotherapy value in selected disease; it does not diagnose inherited cancer susceptibility.
Residual disease changes options
Post-neoadjuvant pathology is an active treatment-selection result, particularly in HER2-positive and triple-negative cancers.
Adherence is a clinical outcome
Long endocrine courses fail when toxicity is not elicited; symptom management and shared switching decisions preserve more benefit than blame.
