Primary lymphoedema reflects developmental lymphatic dysfunction and may present at any age. Secondary disease is more common and follows lymph-node surgery/radiotherapy, malignancy, recurrent infection, trauma or severe obesity; chronic venous oedema can also overload and damage lymphatics.
Diagnosis is usually clinical after excluding systemic and vascular causes. The distribution (including foot/hand or genital involvement), skin texture, pitting, Stemmer sign, operative/cancer history and time course are more useful than one circumference measurement.
Treatment controls rather than cures swelling. Compression must be individualised after arterial assessment where relevant, and manual drainage should be delivered/taught by trained lymphoedema clinicians—particularly when cancer, heart failure or acute infection complicates the picture.
Key points
- Lymphoedema is protein-rich interstitial swelling from impaired lymph transport; it may be primary or secondary to cancer treatment, nodal disease, infection, obesity, trauma or chronic venous disease.

02AetiologyUnderlying causes, associations and risk factors, with why each one matters.
Primary lymphatic dysfunction
Developmental undergrowth or abnormal function of lymphatic vessels and valves reduces transport capacity. It may present at any age, sometimes after a physiological stress exposes limited lymphatic reserve.
Cancer and its treatment
Lymph-node surgery, radiotherapy, tumour recurrence or nodal obstruction can interrupt regional drainage. The distribution usually reflects the treated or obstructed lymphatic territory and may emerge months or years later.
Infection, inflammation or trauma
Recurrent cellulitis, tissue injury and scarring can damage lymphatic channels. Each inflammatory episode may further reduce drainage, creating a cycle of increasing swelling and infection susceptibility.
Venous overload and severe obesity
Chronic venous hypertension or marked obesity increases interstitial fluid formation beyond lymphatic capacity. Prolonged overload can eventually injure lymphatics, producing combined venous and lymphatic oedema.
03PathophysiologyThe causal sequence from the underlying abnormality to symptoms and harm.
- 1Impaired lymph transport
Reduced lymphatic capacity prevents normal return of interstitial fluid, proteins and immune cells to the circulation. Protein-rich fluid therefore accumulates within the affected tissues.
- 2Persistent tissue inflammation
Protein-rich fluid, impaired immune trafficking and lymphatic dysfunction promote chronic inflammation, adipose deposition and tissue remodelling. Early soft pitting swelling can consequently become less reversible.
- 3Fibrosis and skin change
Continuing inflammation stimulates collagen deposition and dermal thickening. The limb may become firm and non-pitting, with deepened folds, papillomatosis and impaired joint or soft-tissue movement.
- 4Infection feedback loop
Oedematous skin may become thickened or fissured, weakening the barrier while local immune clearance is impaired. Cellulitis can then cause further lymphatic injury and accelerate chronic swelling.

Asymmetric pitting swelling with heaviness/tightness, often involving the dorsum of the foot or hand; may lessen overnight.
Firm non-pitting tissue, thickened skin, squared toes, deepened folds, papillomatosis and reduced limb mobility.
Inability to pinch a skin fold at the base of the second toe/finger supports dermal thickening; sensitivity is incomplete.
New warmth, tenderness, erythema/blotchy rash, rapid swelling, fever or flu-like symptoms; systemic toxicity requires emergency care.
Usually bilateral symmetrical painful fat enlargement that spares feet, bruises easily and has a negative Stemmer sign; secondary lymphoedema can coexist.

Deep-vein thrombosis
DVT more often causes sudden unilateral swelling, pain and venous distension. Acute onset or thromboembolic risk requires the formal DVT pathway rather than attribution to established lymphoedema.
Chronic venous oedema
Venous disease often worsens with dependency and accompanies varicosities, pigmentation or venous ulceration. Lymphoedema more characteristically involves the dorsal foot, digits and progressive skin thickening.
Lipoedema
Lipoedema usually produces bilateral, symmetrical, tender fat enlargement with easy bruising and sparing of the feet. A negative Stemmer sign supports it but does not exclude coexisting lymphoedema.
Systemic fluid retention
Heart, renal, hepatic or protein-losing disease more often causes bilateral or generalised oedema with systemic clinical or laboratory clues. These conditions can coexist with local lymphatic failure.
Additional chapter-specific clues
Sudden unilateral swelling, venous distension, pain, new nodes, weight loss or progression after cancer treatment requires urgent vascular/oncology reassessment.

Flucloxacillin
500 mg–1 g orally every 6 hours for 14 days for uncomplicated cellulitis in established lymphoedema.1 g four times daily is off-label; check true penicillin allergy and liver history. Deterioration at any time or no response by 48 hours needs escalation; generic NICE cellulitis courses are shorter, but BLS specifically advises 14 days in lymphoedema.
Clarithromycin
500 mg orally twice daily for 14 days.Review QT prolongation and CYP3A interactions, especially statins; erythromycin is preferred if a macrolide is needed in pregnancy.
Emollient
Apply liberally at least once daily and after washing; quantity depends on limb area and product.Paraffin-containing products create a serious fire risk on fabrics; keep away from flames/smoking and wash clothing/bedding regularly.
08ComplicationsImportant consequences, why they occur and why they matter clinically.
Recurrent cellulitis and sepsis
Skin breaks and impaired local immune clearance facilitate bacterial infection. Cellulitis can spread systemically and also damages lymphatics further, increasing the likelihood of another episode.
Progressive fibrosis and deformity
Chronic inflammation and collagen deposition make tissue increasingly firm, heavy and misshapen. Garments become harder to fit, while mobility, dexterity and daily activities may deteriorate.
Lymph leakage and chronic wounds
High tissue pressure can drive lymph through fragile skin and delay wound healing. Persistent leakage macerates surrounding tissue and creates additional portals for infection.
Pain and functional limitation
Limb weight, tight tissues and recurrent inflammation can restrict joint movement and exercise. Reduced activity weakens the muscle pump, which may further impair fluid clearance.

- Inspect interdigital spaces, nails, folds and wounds for tinea, fissures, dermatitis and lymph leakage.

- Record cellulitis episodes, organism/portal when known, antibiotic response and 48-hour review; recurrent episodes trigger prophylaxis discussion.
- Reassess for cancer recurrence, venous obstruction or systemic oedema when swelling changes unexpectedly.

Pitting evolves
Early lymphoedema may pit; late fibrosis may not, so neither feature alone makes or excludes the diagnosis.
Feet and hands are clues
Dorsal foot/hand and digit involvement favours lymphoedema, while classic lipoedema spares the feet.
Compression and exercise work together
The external counter-pressure lets muscle contraction propel fluid; an inactive compressed limb is less effective.
Diuretics can mislead
Temporary water loss may leave protein-rich tissue disease unchanged and expose the patient to electrolyte/renal harm.
Cellulitis courses differ
BLS lymphoedema-specific guidance advises 14 days, longer than generic uncomplicated cellulitis guidance because early recurrence is a particular concern.
