SVT commonly affects a varicose great saphenous tributary, but the clinically important questions are whether deep thrombosis is already present and how close the clot lies to a deep-vein junction.
Anatomy, length and patient risk—not redness alone—determine anticoagulation. Fondaparinux has a licensed UK regimen for isolated, spontaneous lower-limb SVT at least 5 cm long after DVT and SVT within 3 cm of the saphenofemoral junction have been excluded.
Upper-limb cannula-related phlebitis is assessed differently: remove an unnecessary cannula, look for infection or extension and investigate central/upper-extremity DVT when swelling is extensive or a central device is involved.
Key points
- Superficial vein thrombosis (SVT) produces a tender, erythematous, palpable cord along a superficial vein; it is not always benign.
- Duplex ultrasound should define thrombus length, distance from the saphenofemoral/saphenopopliteal junction and any concomitant DVT when disease is extensive, proximal or clinically uncertain.
- SVT at least 5 cm long and at least 3 cm from a deep-vein junction is treated with fondaparinux 2.5 mg subcutaneously once daily for 45 days when suitable.
- SVT within 3 cm of a deep-vein junction is managed as high-risk extension with therapeutic anticoagulation, usually for 3 months after specialist assessment.

- Cancer, previous VTE, thrombophilia, pregnancy, recent surgery, non-varicose-vein SVT and rapid progression lower the threshold for imaging and specialist treatment.
- Do not prescribe antibiotics unless there is separate cellulitis, suppuration or another bacterial infection; erythema over thrombus is inflammatory.
- After the acute episode, assess symptomatic varicose veins/venous reflux because definitive venous treatment may reduce recurrence.

Varicose veins and venous reflux
Dilated, poorly emptying superficial veins create local stasis and wall inflammation, making a varicose great-saphenous tributary a common site for superficial thrombosis.
Transient venous thromboembolism risks
Recent surgery, immobility, pregnancy, the postpartum period and oestrogen exposure increase stasis or coagulability and may produce more extensive superficial disease.
Persistent prothrombotic states
Active cancer, previous venous thromboembolism and thrombophilia increase extension and recurrence risk, especially when thrombosis arises in a previously normal non-varicose vein.
Cannula-related or septic injury
A peripheral cannula can injure an upper-limb superficial vein. Purulence, fever or systemic toxicity suggests superimposed infection rather than uncomplicated sterile inflammation.

Local venous stasis, endothelial injury or systemic hypercoagulability shifts haemostasis towards fibrin-rich thrombus within a superficial vein of the affected limb.

Thrombus and vessel-wall inflammation produce pain, warmth and linear erythema along the affected vein; cellulitis more often produces diffuse tissue inflammation.
- 3A palpable cord forms
The thrombosed, inflamed vein becomes firm and tender, creating a linear cord that tracks the anatomical course of a superficial vessel.

Thrombus can extend proximally through a saphenous junction or perforating vein into the deep system, where it behaves as deep-vein thrombosis and may embolise.

Localised pain, warmth, erythema and a firm tender cord tracking a superficial vein, often within visible varicosities.

Deep-vein thrombosis
Whole-limb swelling, deep calf tenderness and pitting oedema out of proportion to a superficial cord raise concern for concomitant or alternative deep thrombosis.
Cellulitis
Diffuse spreading erythema, skin tenderness, fever and no palpable venous cord favour cellulitis; sterile superficial thrombosis can still produce striking local redness, and the conditions can coexist.
Lymphangitis
Fine red streaking towards regional lymph nodes, a skin portal and tender lymphadenopathy suggest lymphatic infection rather than thrombosis confined to a superficial vein.
Ruptured Baker's cyst or muscle injury
Posterior knee pathology, bruising or pain linked to movement may mimic calf thrombosis; duplex imaging clarifies venous anatomy when clinical distinction remains uncertain.
Superficial abscess or septic thrombophlebitis
Fluctuance, purulent drainage, an infected cannula, fever or rigors indicate a focal or intravascular infection requiring cultures and source control.

Fondaparinux
2.5 mg subcutaneously once daily for 45 days (licensed course is a minimum of 30 and maximum of 45 days in the eligible high-risk population).Do not use this low-dose regimen for SVT within 3 cm of the junction. Contraindicated if CrCl <20 mL/min; reduce to 1.5 mg daily at CrCl 20–50 mL/min, noting that this reduced dose has not been studied for efficacy/safety. For the SVT indication the SmPC does not recommend fondaparinux below 50 kg, and active cancer/recent VTE were not studied.
Ibuprofen
400 mg orally three times daily with/after food for up to 5–7 days when needed.Avoid or minimise with anticoagulation, peptic ulcer, CKD, heart failure, uncontrolled hypertension or pregnancy; topical NSAID may have less systemic exposure.
Paracetamol
500 mg–1 g orally every 4–6 hours as needed, maximum 4 g in 24 hours (lower maximum in low body weight, frailty or liver risk).Check combination products and hepatic/alcohol risk.

Extension into the deep venous system
A clot approaching a saphenofemoral or saphenopopliteal junction can propagate into deep veins, changing both embolic risk and the required treatment intensity.
Pulmonary embolism
If superficial thrombus propagates into the deep circulation, embolisation to pulmonary arteries can cause acute dyspnoea, pleuritic pain, haemoptysis, syncope or haemodynamic collapse.
Recurrent superficial thrombosis
Untreated venous reflux or a persistent systemic driver can predispose to repeated episodes, chronic local discomfort and further opportunities for proximal propagation.
Septic thrombophlebitis
Bacterial infection of thrombus, particularly around an intravascular device, may seed the bloodstream and cause persistent bacteraemia, sepsis or metastatic infection.

- During fondaparinux, monitor bleeding, haemoglobin/platelets when clinically indicated and renal function if unstable or impaired.

- Confirm completion of 45 days or, for junctional/deep extension, the documented therapeutic-duration plan.
- After recovery, review recurrent SVT and symptomatic varicose veins for duplex-guided venous treatment.

Centimetres determine the pathway
The ultrasound report should state both clot length and exact distance from the deep junction.
Fondaparinux dose is prophylactic-intensity
The 2.5 mg dose is effective for eligible isolated SVT but is not adequate treatment for DVT or junctional extension managed as VTE.
Red does not mean infected
Sterile venous inflammation commonly causes erythema; antibiotics need separate clinical evidence of infection.
Non-varicose SVT deserves more thought
SVT in a normal vein is more strongly associated with systemic provoking factors than a short clot in established varicosities.
Upper-limb SVT is not automatically low risk
Central devices, diffuse arm swelling or symptoms beyond a peripheral cannula site require upper-extremity DVT/central obstruction assessment.
