Imaging in pregnancy and breastfeeding

Select and justify imaging during pregnancy or breastfeeding without delaying essential diagnosis, while distinguishing radiation, contrast, MRI, and nuclear-medicine considerations.

Failure to diagnose serious maternal disease can harm both the pregnant patient and fetus.

Action: Use the fastest adequate examination after focused justification and optimisation; do not delay solely because pregnancy is known or possible.

Imaging risk is not considered in isolation. Untreated maternal hypoxia, haemorrhage, infection, cancer or thromboembolic disease may create far greater fetal and maternal harm than a justified diagnostic exposure.

The practical sequence is to define the clinical question, check whether a timely non-ionising test is adequate, justify any ionising exposure, optimise anatomy and technique, and communicate realistic benefits, uncertainties and alternatives.

Breastfeeding considerations differ by agent. Routine CT and MRI contrast do not require interruption according to UK professional guidance, whereas nuclear medicine can expose the infant through milk and needs agent-specific instructions.

Breastfeeding with contrast

Iodinated and gadolinium contrast require clear reassurance that routine feeding interruption is unnecessary after clinically indicated CT or MRI.

Breastfeeding with radionuclide

Radiopharmaceutical excretion and half-life vary; interruption, expressed-milk disposal and contact restriction may be agent-specific.

Communication need

Anxious patients need absolute, comprehensible risk framing and an explanation of the health risk created by withholding useful imaging.

Red flags requiring action

  • Shock, major trauma, suspected pulmonary embolism, acute neurological deficit or surgical abdomen requires urgent senior imaging discussion.
  • A patient may have been discharged before pregnancy status or a critical result is clarified, requiring active recall and communication.
  • A breastfeeding nuclear-medicine patient needs radiopharmaceutical-specific advice rather than generic contrast reassurance.

Delay also carries risk

Counselling should compare exposure risk with the concrete risk of untreated maternal disease, avoiding language that presents non-imaging as automatically safer.

Cancer statement scope

The RCR and RCOG position addresses suspected cancer, but its anti-delay principle supports serious-disease reasoning while disease-specific pathways still govern modality choice.

MRI requires its own assessment

Absence of ionising radiation does not make every MRI automatically suitable; acoustic noise, operating mode, implant status, access and gadolinium remain relevant.

Contrast classes differ

Iodinated contrast and gadolinium-based agents have different pharmacology and risk profiles; neither should be conflated with radiopharmaceutical exposure.

Fetal dose is examination-specific

Gestation, scanned anatomy, technique and distance from the beam matter, so broad labels such as one CT cannot supply an individual estimate.

Patient choice needs context

A balanced explanation includes expected diagnostic benefit, alternatives and consequences of delay, not radiation facts stripped from the clinical decision.
