Pulmonary oedema and heart failure

Pulmonary oedema can cause rapid respiratory failure

Severe hypoxaemia, exhaustion, altered consciousness, shock or frothy sputum requires immediate physiological treatment while the cause is established.

Action: Call for senior help, sit the patient up when appropriate, support oxygenation and ventilation, monitor circulation, treat the suspected precipitant and obtain urgent cardiac assessment without waiting for radiographic certainty.

Synopsis

Interpret radiographic pulmonary congestion and oedema in technical and clinical context, recognise important mimics and use imaging to guide urgent support and cardiac assessment.

  • Cardiogenic pulmonary oedema is a pattern: upper-zone venous redistribution, peribronchial cuffing, septal lines, central or dependent air-space opacity and pleural effusions may occur together.
  • Check projection, rotation, inspiration and exposure before calling cardiomegaly or basal oedema on a portable film; AP magnification and low volume can mimic both.
  • Treat severe respiratory failure or shock immediately even if imaging is incomplete, and keep pneumonia, aspiration, ARDS, haemorrhage and pulmonary embolism active when the pattern is atypical.

Key red flags

Severe respiratory distress, rising carbon dioxide, exhaustion or reduced consciousness may require non-invasive or invasive ventilatory support under an acute pathway.

Hypotension, cool peripheries, oliguria or altered mentation with congestion suggests cardiogenic shock and needs urgent critical-care and cardiology assessment.

A unilateral or markedly asymmetric air-space opacity should widen the differential to pneumonia, aspiration, haemorrhage or focal vascular disease rather than being labelled oedema automatically.

Acute chest pain, new murmur, arrhythmia, myocardial-ischaemia evidence or sudden valve dysfunction can identify a precipitant that changes immediate management.

A normal or equivocal radiograph does not exclude acute heart failure, and radiographic severity may lag or differ from the clinical physiology.

Dangerous haemodynamic state

Shock, a new murmur, ongoing ischaemia or severe arrhythmia with pulmonary oedema signals a time-critical precipitant requiring urgent specialist care.

  1. Treat the patient’s oxygenation, ventilation and circulation while reviewing projection, inspiration, prior films, ECG and bedside examination.
  2. Look for a coherent congestion pattern, pleural fluid and cardiac enlargement but actively search for focal infection, aspiration and procedure complications.
