Cardiovascular medicines

Choose and review cardiovascular medicines by clinical purpose, haemodynamics and organ function, with safe initiation and a reasoned response to adverse effects.

01Purpose and principlesWhat the treatment does and how it fits into care.

Cardiovascular prescribing becomes clearer when every medicine has an explicit job. A loop diuretic may make a congested patient breathe more comfortably without replacing therapies that reduce future heart-failure events. A calcium-channel blocker may lower blood pressure or relieve angina but does not automatically supply the benefits of a disease-modifying HFrEF regimen. Conversely, a medicine with prognostic benefit can still cause symptomatic hypotension or biochemical harm in a particular patient. The practical task is to find an effective and tolerable combination, using the clinical diagnosis and the patient’s priorities to decide what to introduce, what to titrate and what to reassess.

The familiar label cardiac medicines hides several different physiological effects. Renin–angiotensin system treatment can alter glomerular haemodynamics and potassium handling; vasodilators can lower pressure and redistribute fluid; beta-blockers slow heart rate and modify sympathetic drive. A new symptom after titration should therefore generate a mechanistic explanation that can be checked. For example, ankle swelling during amlodipine treatment is not by itself proof of worsening cardiac congestion. Equally, presyncope in a patient with an apparently acceptable seated blood pressure may reflect postural hypotension. Good review connects measured physiology with the person’s experience rather than treating a list of numerical targets independently.

  • Before ACE inhibitor or ARB initiation, check blood pressure, renal function, sodium and potassium. Repeat laboratory monitoring within 1–2 weeks and after dose changes, earlier when risk is higher.
  • A creatinine rise must be interpreted by indication, magnitude, symptoms, potassium, volume status and trend. Current CKD and heart-failure guidance do not use an identical automatic stopping threshold.
  • New angioedema, symptomatic severe hypotension, dangerous hyperkalaemia or marked bradycardia needs prompt assessment. Do not manage potentially unstable physiology by simply scheduling a routine review.
02Indications, selection and cautionsWho may benefit, who needs urgent treatment and important alternatives.
Define the cardiovascular phenotype

Distinguish uncomplicated hypertension, stable angina, recent infarction and heart failure with a documented ejection-fraction category. Treatment choices and expected outcomes vary between them. Obtain the echocardiogram or specialist assessment when the diagnosis is uncertain. A problem-list entry saying heart failure does not establish whether an HFrEF combination is indicated.

Assess volume and perfusion together

Examine for congestion while asking about thirst, postural dizziness, reduced intake and recent diuretic changes. Oedema and intravascular depletion can coexist in complex illness. Weight trends, orthopnoea, jugular venous pressure, perfusion and renal trajectory help interpret the situation. Prescribing another diuretic solely because ankles look swollen can worsen hypotension when the mechanism is medication-related.

Identify linked adverse effects

ACE-inhibitor cough, amlodipine-related ankle oedema and beta-blocker-associated bradycardia suggest different responses. Establish timing, dose changes and competing causes before attributing symptoms. Facial or tongue swelling is an emergency concern for angioedema, not a routine tolerability issue. A clinically significant adverse effect should be recorded clearly enough to guide future selection.

Anticipate vulnerability before titration

Older age, frailty, kidney impairment, diabetes, concurrent diuretics and poor oral intake increase the need for careful review. Ask about pregnancy or planned pregnancy before renin–angiotensin treatment. Assess medicines that also lower pressure or raise potassium, including non-prescription NSAIDs and potassium-containing salt substitutes. A safe titration plan includes the person who will review the results.

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

    Why
    Relate treatment intensity to pressure and postural symptoms.
    Interpretation and limitations
    Use an appropriate cuff and repeat unexpected readings. Standing measurements are especially useful with falls, dizziness or frailty. A lower pressure without symptoms may be acceptable in one heart-failure patient but concerning when accompanied by confusion, oliguria or poor perfusion. Interpret the measurement in its therapeutic and physiological context.

    Creatinine, eGFR, sodium and potassium
    Why
    Establish baseline safety and assess the response to treatment changes.
    Interpretation and limitations
    Compare results with the pre-treatment baseline and calculate percentage change when useful. Distinguish chronic stable impairment from acute deterioration. Review dehydration, infection, NSAIDs and other potassium-raising drugs before deciding whether a renin–angiotensin medicine caused an unacceptable response. Markedly abnormal potassium needs timely clinical action, not isolated repeat prescribing.

    Pulse and electrocardiogram when indicated

    Assess rate, rhythm and conduction before changing rate-limiting medicines.
    Interpretation and limitations
    A slow pulse with presyncope needs more than dose arithmetic. Check for conduction disease and duplicate rate-limiting treatment, such as a beta-blocker combined with a non-dihydropyridine calcium-channel blocker. New rhythm changes may explain deterioration that initially appears to be medication intolerance.

    Distinguish congestion from other causes of swelling or breathlessness.
    Interpretation and limitations
    Compare serial weights under similar conditions, examination and symptom change. Echocardiography and natriuretic peptides serve diagnostic questions rather than replacing bedside assessment of acute instability. A drug review should use the established cardiac phenotype instead of inferring ejection fraction from the presence of a diuretic prescription.
04Treatment approachPreparation, options, escalation and aftercare.
01InitiationStart an ACE inhibitor for hypertensionFirst stepAn adult has confirmed hypertension and an ACE inhibitor is an appropriate choice.
  1. 1Review the NICE hypertension selection pathway, existing medicines, pregnancy potential and previous angioedema. Confirm that the diagnosis and treatment target are established, and check baseline pressure, kidney function, sodium and potassium before prescribing.
  2. 2For a suitable adult, start ramipril 2.5 mg orally once daily. Consider 1.25 mg with medical supervision when clinically appropriate for greater first-dose hypotension risk, including relevant diuretic use, and apply the product’s renal dose limits.
  3. 3Arrange renal and electrolyte checks within 1–2 weeks, or earlier in higher-risk patients, and a pressure review. Explain dizziness, swelling of the lips or tongue, and when acute illness should prompt contact for an individual medicine plan.
  4. 4Increase only after reviewing tolerability, pressure and laboratory response. The usual hypertension titration interval is 2–4 weeks, with a maximum of 10 mg daily in suitable renal function; a maximum dose is not a target that overrides symptomatic harm.
02CombinationBuild a disease-modifying HFrEF regimenAn adult has confirmed heart failure with reduced ejection fraction.
  1. 1Assess current stability, congestion, blood pressure, pulse, kidney function and potassium, and establish what therapies are already tolerated. Acute decompensation may require immediate management before routine outpatient titration can proceed safely.
  2. 2Use the current NICE combination of ACE inhibitor, beta-blocker, MRA and SGLT2 inhibitor, with sequencing and intervals adapted to the person. Explain the separate purposes of these medicines and any diuretic used for congestion.
  3. 3Plan the specific observations and laboratory checks required after each introduction or dose change, with a named clinician receiving results. Avoid making several changes without a way to interpret emerging hypotension, bradycardia or biochemical disturbance.
  4. 4Review persistent symptoms despite maximum tolerated treatment and consider the NICE circumstances for switching to an ARNI. ACE inhibitor and ARNI treatment must not overlap; any switch requires its product-specific washout and contraindication checks through the responsible heart-failure team.
03ReassessmentRespond to renal deterioration after titrationA repeat kidney-function result has worsened after an ACE inhibitor or ARB change.
  1. 1Contact and assess the patient according to severity. Ask about reduced intake, diarrhoea, infection, dizziness, urine output and new NSAID use; review pressure, potassium and volume status rather than treating the creatinine value in isolation.
  2. 2Calculate the change from the actual baseline and identify whether CKD guidance, heart-failure guidance or an acute-illness pathway best applies. A threshold used to trigger investigation in one population is not a universal instruction to stop beneficial therapy in every patient.
  3. 3Correct reversible contributors and arrange the indicated repeat test, dose adjustment or temporary withholding with clinical oversight. Severe hyperkalaemia, major acute kidney injury or haemodynamic compromise requires urgent assessment rather than a delayed outpatient titration decision.
  4. 4Document the reason for the change, the next blood-test date and a restart or re-titration decision if treatment was interrupted. Confirm that the dispensing record reflects the intended regimen so that the previous dose is not inadvertently resumed.
05Regimens, contraindications and interactionsTreatment details and the circumstances that modify them.
An ACE inhibitor selected according to the hypertension pathway and comorbidity.

Ramipril for selected adults with hypertension

Usually 2.5 mg orally once daily; increase at 2–4-week intervals according to response, to 10 mg daily maximum with suitable renal function. Selected first-dose-risk patients may start at 1.25 mg.

This is the hypertension SmPC schedule, not every cardiac indication. CrCl 30–60 mL/min limits the maximum to 5 mg daily; CrCl 10–30 starts at 1.25 mg with maximum 5 mg. Avoid pregnancy and previous ACE-inhibitor angioedema. With hepatic impairment, initiate under close supervision and do not exceed 2.5 mg daily.

A dihydropyridine calcium-channel blocker for blood-pressure reduction or angina relief.

Amlodipine for hypertension or angina

5 mg orally once daily initially, increasing if required and tolerated to a maximum 10 mg once daily; treatment continues with regular benefit and adverse-effect review.

Assess oedema, headache and hypotension. Titrate cautiously in older adults; hepatic impairment requires cautious dose selection. It is not a substitute for disease-modifying HFrEF treatment and is unsuitable in shock or severe hypotension.

06Complications, monitoring and follow-upAdverse effects, response and longer-term review.
  • For ACE inhibitor or ARB treatment, obtain the relevant post-initiation and post-titration kidney and electrolyte tests, usually within 1–2 weeks and earlier with higher risk. The result must reach someone able to act; ordering it is only the first step.
  • Assess standing symptoms, falls, exercise tolerance and daily function alongside clinic pressure. A regimen that meets a numerical target but causes recurrent presyncope needs review of the whole combination and the agreed treatment priorities.
  • For amlodipine, ask whether swelling appeared after initiation or escalation and whether there is other evidence of congestion. Treating an adverse effect with an additional medicine without reconsidering the cause can increase burden and obscure the original problem.
  • Revisit the medicine plan after hospitalisation, acute illness or substantial weight change. Update any temporary withholding instructions and verify that beneficial long-term treatment has a documented restart assessment rather than disappearing from care.

Targets belong to a clinical context

The same blood pressure can be reassuring in an asymptomatic stable patient and dangerous in someone with cool peripheries and oliguria. A treatment target supports judgment; it does not replace assessment of perfusion or acute illness.

Cardiorenal responses differ by indication

A modest expected haemodynamic change after renin–angiotensin treatment is different from progressive injury during dehydration. Current NICE and SPS recommendations distinguish CKD and heart-failure circumstances, so record which guidance and clinical features informed the decision.

Tolerated doses remain meaningful

A patient may benefit from several complementary therapies at tolerated doses even when one cannot reach its nominal target. Discuss constraints with the heart-failure team instead of abandoning the entire combination after a single dose-limiting symptom.

Medicine purpose supports deprescribing

When a drug has no clear indication, reconstruct why it was started before stopping it. A historic antihypertensive may now provide another benefit, while a temporary inpatient medicine may no longer be needed. Purpose is the starting point for both continuation and withdrawal.

08Common pitfallsFrequent interpretation and management errors.

    Adding a diuretic automatically for ankle oedema during amlodipine treatment can create a prescribing cascade when a dose or agent review is needed.

  2. 02

    Combining ACE inhibitor and ARB treatment routinely increases harm and should not be inferred from the fact that both classes appear in cardiovascular guidance.

    Applying a remembered creatinine percentage as an automatic stopping rule without identifying CKD, heart failure or acute illness can mismanage a clinically important change.

    Titrating several pressure- or rate-lowering medicines without planned observations makes adverse effects harder to identify and may leave the patient without timely support.
