Non-blanching rash and purpura assessment

Triage a non-blanching eruption without delay, describe purpura accurately across skin tones, distinguish infection, bleeding, inflammation and occlusion, and direct immediate tests and treatment by physiological risk.

Purpura with systemic illness

A rapidly evolving non-blanching eruption with fever, altered behaviour, neck stiffness, limb pain, cold extremities, prolonged capillary refill, hypotension, breathing difficulty or reduced urine can signal meningococcal disease, sepsis or disseminated coagulation before shock is established.

Action: Use an immediate ABCDE assessment, call emergency transport or the resuscitation team, obtain cultures and bloods only if they do not delay care, and start age-appropriate parenteral antimicrobial and sepsis treatment through the current local pathway; isolate and notify suspected meningococcal disease as required.

Purpura results when erythrocytes or blood products lie outside vessels or when a vessel contains fixed thrombotic material. The first question is physiological rather than taxonomic: is the patient ill? Assess airway, breathing, circulation, disability, temperature, capillary refill and urine before counting lesions. Meningococcal disease is a critical possibility, but severe bacterial sepsis from another source, disseminated intravascular coagulation, thrombotic microangiopathy and acute ischaemia can produce the same visual alarm. A rash is neither required for meningococcal disease nor sufficient to diagnose it.

If stable, describe size, elevation, shape, temperature, tenderness, necrosis and distribution. Flat symmetrical petechiae below areas of pressure after coughing may be mechanical; widespread petechiae with oral bleeding point towards thrombocytopenia. Raised dependent papules favour small-vessel inflammation. A broken branching border reflects an occluded cutaneous vascular network and should redirect the assessment towards thrombotic vasculopathy, embolism, calciphylaxis or medium-vessel disease. Examine pulses and capillary refill beside the skin rather than treating morphology as a photograph-only exercise.

Recognition must be equitable across pigmentation. Red-purple contrast may be reduced in deeply pigmented skin, while violet, grey-brown or black change, surface elevation, swelling and temperature remain detectable. Ask whether the patient or carer has noticed a new colour or texture and inspect less pigmented mucosa without assuming that lesions must be visible there. Accurate documentation includes whether pressure truly changed colour and how the pattern evolved over minutes or hours.

Key points

  • Non-blanching means colour remains when firm pressure displaces blood from nearby capillaries; it describes extravasated blood or fixed vascular material, not a diagnosis.
  • Inspect the whole skin in good light, including conjunctivae, mouth, palms, soles and pressure areas; in brown or black skin, petechiae may be clearer on paler mucosa or by side lighting and palpation.
  • Never use a reassuring glass test to overrule an unwell patient: early meningococcal disease may have a blanching, sparse or absent rash, and deterioration can be fast.
  • Petechiae are pinpoint lesions, purpura are larger patches and ecchymoses are bruise-sized; more important bedside distinctions are flat versus palpable and round versus angular retiform morphology.
  • Fever, toxicity or rapidly multiplying lesions demands immediate sepsis and meningococcal assessment before a dermatological differential or biopsy.
  • Flat petechiae with mucosal bleeding favour platelet failure; large ecchymoses and deep bleeding suggest coagulation defects; palpable crops suggest inflamed small vessels.
  • Branching retiform purpura, cold skin, severe pain or necrosis indicates vascular occlusion or tissue ischaemia and needs urgent vascular, haematology, renal or dermatology input.
  • First-line tests in a stable unexplained presentation are FBC with film, coagulation and fibrinogen, renal and liver profiles, CRP and urinalysis, then phenotype-directed infection, haemolysis, autoimmune or clonal testing.
  • Record onset, speed, pain, fever, medicines, infections, bleeding sites and distribution and photograph with consent; do not erase a dangerous trajectory by calling the lesions bruises.

Infection and consumption

Invasive bacterial infection can injure endothelium and trigger disseminated coagulation, producing rapidly progressive purpura alongside shock and organ failure.

Haemostatic failure

Low or dysfunctional platelets, deficient clotting factors, liver disease, anticoagulants and marrow disorders permit blood to escape after minimal vascular stress.

Vessel-wall inflammation

Immune-complex, autoimmune, medicine-related or infection-associated vasculitis damages dermal vessels and creates raised purpura with possible systemic organ involvement.

Vascular occlusion or trauma

Thrombi, emboli, calcification, cryoproteins, pressure or mechanical strain obstruct or rupture vessels and generate retiform infarction, petechiae or bruising.

    Blood leaves the lumen

    Once erythrocytes enter dermis or mucosa, external pressure cannot empty the colour, which explains why true purpura remains non-blanching.

    Defective primary haemostasis produces small-vessel oozing at skin and mucosa, creating petechiae, epistaxis and immediate bleeding after minor trauma.

    Inadequate fibrin formation destabilises the platelet plug and permits larger bruises, soft-tissue haematoma, joint bleeding or delayed procedural haemorrhage.

Petechiae

Pinpoint flat non-blanching macules may reflect platelet failure, capillary pressure, infection or early vascular injury and require context rather than size-based reassurance.

Palpable purpura

Raised red-purple, violaceous or brown papules, often in dependent crops, indicate inflammatory leakage around small dermal vessels and make vasculitis prominent.

Retiform purpuraRed flag

Angular branching non-blanching patches map an obstructed vessel network; pain, coolness, ulceration or black necrosis signals threatened tissue.

Mucosal bleeding phenotype

Gum bleeding, epistaxis, oral petechiae, menorrhagia and widespread flat skin lesions support a platelet-number or platelet-function problem.

Coagulation bleeding phenotype

Large bruises, haematomas, haemarthrosis or delayed bleeding after procedures suggests clotting-factor failure, liver disease, anticoagulation or disseminated consumption.

Septic trajectoryRed flag

Rapid spread with fever, limb pain, reduced responsiveness, poor perfusion or respiratory change is an emergency even if lesions are few or partially blanching.

Mechanical distribution

Petechiae confined above the nipple line after vomiting or coughing, or beneath tight pressure, can be benign only after illness and bleeding risk are excluded.

Red flags requiring action

  • Toxic appearance, rapid lesion spread, fever or hypothermia, altered consciousness, severe limb or abdominal pain, meningism, respiratory compromise, hypotension, delayed refill, oliguria, mucosal bleeding, neurological deficit, pregnancy or immunosuppression requires same-day hospital assessment.

    Prolonged clotting with falling fibrinogen and rising D-dimer in an ill patient supports DIC, while normal screening does not exclude platelet dysfunction or von Willebrand disease.

    Find infection and physiological consequence without postponing antimicrobials in a high-risk presentation.
    Interpretation and limitations
    Take blood cultures before antibiotics when immediately feasible, but a negative culture after treatment or early in disease cannot safely exclude invasive infection.

    Detect nephritis, hepatic synthetic failure and systemic vasculitic or thrombotic injury.
    Interpretation and limitations
    Haematuria or protein with hypertension and rising creatinine needs urgent renal review; liver dysfunction can produce thrombocytopenia and impaired clotting through several mechanisms.

    Investigate anaemia, organ dysfunction or retiform ischaemia for destructive or occlusive disease.
    Interpretation and limitations
    Use reticulocytes, bilirubin, LDH, haptoglobin and film first; add APS, cryoprotein, paraprotein or thrombophilia tests only when phenotype and timing make results interpretable.

    Discuss depth, active edge, lesion age and separate direct-immunofluorescence handling with dermatopathology; never let an elective biopsy delay sepsis or ischaemia treatment.

Fever, rapid progression, limb pain, reduced responsiveness and poor perfusion require emergency infection treatment regardless of early rash appearance.

Shock and multiorgan failure

Invasive infection and DIC can progress within hours to cardiovascular collapse, respiratory failure, renal injury, adrenal haemorrhage and death.

Major haemorrhage

Severe thrombocytopenia or coagulation failure can cause gastrointestinal, pulmonary, intracranial or uncontrolled procedural bleeding despite modest skin findings.

03

Tissue infarction

Thrombotic retiform disease can destroy skin, digits or limbs and leave ulceration, infection, neuropathic pain, scar and amputation.

04

Systemic vasculitic injury

Renal, bowel, pulmonary or nerve involvement can evolve after the rash and cause permanent organ dysfunction if follow-up stops too early.

Safeguarding and diagnostic harm

Misattributed bruises can conceal abuse, while indiscriminate allergy or vasculitis labels can delay necessary medicines and appropriate specialist investigation.

  • In high-risk illness, repeat observations, perfusion, mental state, urine and lesion extent at the interval required by the sepsis or critical-care pathway.
  • Trend platelets, haemoglobin, coagulation, fibrinogen, renal and liver function until the cause and direction of change are secure.
  • Use neutral-light photographs with a scale and consent to show speed of spread, but do not delay transfer to obtain an ideal image.
  • After an apparently benign mechanical episode, provide return advice for fever, new lesions away from pressure sites, mucosal bleeding, bruising or reduced wellbeing.
  • If vasculitis is suspected, repeat blood pressure and urinalysis during early follow-up because renal features can emerge after the first skin assessment.
  • Record any confirmed infectious notification, medicine reaction or haematological diagnosis precisely and remove temporary broad labels when definitive evidence arrives.

Blanching is not immunity

A patient with invasive meningococcal disease may initially have no rash or a blanching eruption, so physiology and trajectory always outrank the tumbler test.

Palpation adds mechanism

Elevation reflects inflammatory oedema and turns an apparently flat colour change into a clue for small-vessel wall injury.

Shape maps blood flow

A branching angular outline reproduces dermal vascular territories and therefore points towards lumen occlusion rather than simple diffuse platelet leakage.

Platelet clumps mislead

EDTA-related clumping can create a spurious automated low count, but confirmation must occur urgently without dismissing genuine bleeding risk.

Normal coagulation is narrow

PT and APTT can be normal in thrombocytopenia, platelet dysfunction, von Willebrand disease and factor XIII deficiency, so bleeding phenotype remains important.

Pigment alters contrast

Side lighting, touch, temperature and mucosal examination help detect purpura when red-purple colour contrast is reduced, while respecting comfort and consent.
