Pityriasis rubra pilaris is uncommon enough that delayed or changing diagnosis is expected. Awareness rests on recognising the combination of follicular roughness, coalescing cephalocaudal plaques, spared islands and waxy palms or soles.
Colour terminology must not dominate. Orange-red is classic, but deeply pigmented skin may show brown, violaceous or less obvious colour while keratotic follicular papules and sharp islands remain palpable and visible.
Management depends on extent and physiology. Local treatment improves comfort; extensive PRP belongs with dermatology because systemic therapies are often off-label, evidence is limited and erythroderma can become medically unstable.

Usually sporadic inflammation
PRP is a rare inflammatory keratinisation disorder whose cause is usually unknown. It is not infectious and affects people of all racial groups and sexes.
Occasional familial disease
Most cases are not inherited, but familial juvenile forms occur and can involve CARD14-related pathways. Childhood onset and affected relatives justify specialist genetic consideration rather than routine testing.
Six clinical types
Classification includes classic and atypical adult, three juvenile patterns and HIV-associated PRP. Type identifies expected distribution and natural history but does not replace assessment of current severity.

Discrete patches of normal-appearing skin remain within large confluent plaques. This striking geometry favours PRP over many other erythrodermas but is not independently diagnostic.
Palms and soles become diffusely thick, smooth, orange-yellow or waxy and develop painful fissures. Record walking, grip and secondary infection rather than describing appearance alone.
Plaques may be deep brown, violaceous or subtly warm-toned rather than visibly orange-red. Compare with uninvolved islands and prioritise scale, follicular texture, heat and symptoms.
Near-total involvement with shivering, temperature change, oedema, tachycardia, oliguria or breathlessness indicates barrier failure and needs immediate acute care.

Psoriasis
Psoriasis can resemble PRP, including erythroderma. Follicular papules, orange waxy palmoplantar keratoderma and islands of sparing favour PRP, but overlap often requires histology and longitudinal review.
Severe eczema or drug eruption
Eczema is usually more exudative or lichenified; drug erythroderma follows an exposure timeline and may have facial oedema, eosinophilia or organ injury. Mucosal blistering redirects urgently.
Cutaneous lymphoma
Persistent atypical erythroderma, disproportionate itch, nodes, palmoplantar change or abnormal blood findings can indicate cutaneous T-cell lymphoma. Multiple biopsies and specialist staging may be needed.
Keratinisation disorders
Ichthyosis, keratoderma syndromes, follicular keratoses and Darier disease can overlap, especially in juvenile or longstanding atypical forms. Age at onset and family pattern refine classification.
Crusted infection
Crusted scabies or dermatophyte infection can generalise in immunosuppression. Contacts, burrows, scrapings and microscopy help, while sepsis features require treatment before final taxonomy.

Erythrodermic physiology
Widespread disease causes dehydration, electrolyte loss, hypoalbuminaemic oedema, temperature disturbance and high-output cardiac stress. Older or cardiorenal patients are especially vulnerable.
Painful palmoplantar fissures
Waxy thickening splits under load and can severely impair gait, grip, work and self-care. Deep fissures also create portals for secondary bacterial infection.
Secondary infection
Barrier disruption and systemic immunomodulation increase bacterial, fungal and viral infection risk. Culture when clinically indicated but distinguish colonisation from invasive disease.
Treatment toxicity
Retinoids, methotrexate and biologic therapy are often off-label for PRP and bring reproductive, hepatic, lipid, marrow and infection risks. Monitoring follows the selected agent and protocol.
Psychological burden
Rarity, diagnostic delay, visible scale, odour, pain and unpredictable course can cause isolation and low mood. Explicit validation and access to support are part of clinical care.

Texture can precede colour
Palpable follicular roughness may reveal PRP in deeply pigmented skin before orange-red change is obvious. Compare involved skin directly with a spared island.
Islands are high-value clues
Normal skin embedded within confluent plaques is memorable and useful, but psoriasis and other erythrodermas can occasionally mimic it, so histology and course still matter.
No confirmatory blood test
Laboratory abnormalities in extensive disease measure physiological loss or treatment safety. They should not be presented as a diagnostic PRP panel.
Natural history varies
Classic adult disease often remits within years, while atypical adult and familial juvenile disease can persist. Prognosis should follow subtype and observed course.
Rare disease invites anchoring
Once PRP is suggested, clinicians can overlook drug reaction, lymphoma or infection. Each major change deserves a fresh differential rather than automatic escalation.
