Venous and gravitational eczema

Recognise lower-leg eczema caused by chronic venous hypertension, exclude arterial and acute vascular disease, treat skin inflammation, and coordinate safe compression and venous referral.

Time-critical presentation

Arrange same-day assessment for a hot painful swollen leg with systemic illness, suspected deep-vein thrombosis, acute limb ischaemia, rapidly spreading cellulitis or a bleeding varicose vein.

01OverviewDefinition, clinical context and the essential points that orientate the chapter.

Look from toes to groin. Record oedema, varicosities, haemosiderin, atrophie blanche, lipodermatosclerosis, ulcers, pulses and temperature. In deeply pigmented skin, active inflammation may appear darker, purple or grey and be detected through warmth, itch, scale and ooze; chronic haemosiderin is brown and persists after inflammation settles.

Management integrates dermatology and vascular care. Emollient and topical corticosteroid settle skin, but venous pressure requires movement, leg elevation and suitable compression. NICE referral supports duplex assessment and definitive treatment options for venous skin changes. A painful or infected ulcer needs a separate wound and vascular pathway.

Venous and gravitational eczema care should combine visible inflammation with itch, pain, sleep, occupation and treatment burden. Agree where each product goes, how much is used and when response will be reviewed; explain urgent features separately so normal fluctuation is not confused with infection or treatment failure.

Venous valve failure

Primary varicosities, previous thrombosis and calf-pump impairment cause reflux and sustained ambulatory venous hypertension in the lower leg.

Oedema and immobility

Reduced walking, obesity, prolonged standing or sitting and dependent sleep impair calf-pump return and increase fluid accumulation.

Skin exposure and sensitisation

Chronic exudate, dressings, adhesives, topical antibiotics and preservatives can add irritant or allergic dermatitis to the venous inflammatory background.

    Raised venous pressure distends capillaries and promotes fluid, erythrocyte and protein leakage into tissue, generating oedema and inflammation.

    Extravasated erythrocytes break down and leave iron-containing pigment, producing persistent brown lower-leg colour that does not indicate active eczema alone.

    Chronic cytokine and subcutaneous fat inflammation hardens the gaiter area as lipodermatosclerosis and reduces local tissue resilience.

Gaiter-zone dermatitis

Itchy scaly or weeping inflammation clusters above the medial ankles with dependent oedema and varicose veins.

Chronic venous markers

Brown haemosiderin, white atrophic scars, ankle flare and indurated inverted-bottle legs support long-standing hypertension.

Bilateral inflammatory pattern

Both lower legs may be warm, itchy and inflamed without systemic illness, often mistaken for bilateral cellulitis.

Ulcer and contact pattern

An irregular gaiter ulcer plus sharply geometric dermatitis under adhesive or topical treatment suggests concurrent contact allergy.

Acute vascular departureRed flag

Sudden one-sided swelling, calf pain, cold foot, rest pain, absent pulse or chest symptoms needs urgent assessment.

Cellulitis

Acute progressive unilateral warmth, tenderness, swelling and systemic illness differs from usually bilateral chronic itchy venous inflammation, although infection can complicate eczema.

Allergic contact dermatitis

Sharp worsening beneath dressings or after topical products, especially spreading beyond the ulcer area, supports sensitisation and patch-test referral.

Arterial disease

Rest pain, cool pale foot, tissue loss, reduced pulses and abnormal pressure assessment indicate impaired arterial supply and alter compression safety.

Deep-vein thrombosis

Sudden unilateral swelling and pain with thrombosis risk requires urgent diagnostic assessment rather than routine eczema treatment.

A potent topical corticosteroid for inflammatory venous eczema alongside emollient, oedema control and vascular management.

Betamethasone valerate 0.1% cream

Apply thinly to active intact lower-leg eczema once or twice daily for up to four weeks, then reduce or stop when inflammation is controlled.

Avoid ulcer beds, untreated infection, large denuded areas and unplanned occlusion; review atrophy and contact sensitivity and do not mistake haemosiderin pigment for active disease.

Venous ulceration

Progressive inflammation, oedema and minor trauma can produce chronic gaiter-region ulcers with pain, exudate, infection risk and major care burden.

Lipodermatosclerosis

Fibrotic inverted-bottle contour and painful inflammatory panniculitis reduce mobility and may be mistaken for recurrent bacterial cellulitis.

Secondary infection

Barrier loss can lead to cellulitis, lymphangitis or systemic infection, particularly when oedema and ulcers remain uncontrolled.

Contact sensitisation

Repeated exposure to dressings and topical agents makes lower-leg patients vulnerable to allergy that perpetuates apparently treatment-resistant eczema.

  • Measure oedema, skin activity, ulcer dimensions, pain, mobility and compression tolerance at each venous-eczema review.
  • Check garment fit and condition, arterial symptoms and whether the patient can put on and remove compression safely.
  • Reassess persistent dermatitis for contact allergy, tinea and inadequate venous control rather than cycling topical antibiotics.
  • At review of Venous and gravitational eczema, compare itch, sleep, fissuring or ooze, affected sites, function and treatment use with the agreed baseline.
  • For Venous and gravitational eczema, record adverse effects, new contact exposures and the safety-net for pain, fever, rapidly spreading disease, eye symptoms or systemic illness.

Brown does not equal active

Haemosiderin pigment remains after eczema settles, so itch, scale, warmth and ooze are better markers of current inflammation.

Bilateral cellulitis is uncommon

Two chronically inflamed lower legs without systemic illness more often reflect venous eczema or lipodermatosclerosis.

Compression needs a circulation check

An appropriate arterial assessment and trained fitting protect patients whose lower-leg eczema coexists with arterial disease.

Skin change warrants referral

NICE includes pigmentation or eczema caused by chronic venous insufficiency among indications for vascular-service assessment.

Dressings can become allergens

Chronic ulcer care repeatedly exposes skin to adhesives, preservatives and topical antibiotics, making patch testing valuable in refractory cases.
