Otoscopy and tympanic-membrane findings

Perform a safe, systematic ear examination, describe tympanic membrane abnormalities precisely, and connect the findings and hearing history to the appropriate next investigation or referral.

A normal-looking drum can accompany an emergency

Otoscopy examines the sound-conducting structures that can be seen; it cannot exclude an acute cochlear or neurological cause of hearing loss.

Action: For hearing loss developing over three days or less during the previous 30 days, without an external or middle-ear explanation, arrange ENT or emergency assessment within 24 hours. New facial weakness, severe vertigo with neurological signs, or serious head injury requires the appropriate emergency pathway.

01Purpose and principlesWhat the assessment is for and the core concepts behind it.

Otoscopy is most useful when it answers a defined clinical question. In someone with pain, it can identify an inflamed canal, a distended tympanic membrane or a complication needing further assessment. In someone with hearing difficulty, it can reveal an obstructed canal or a middle-ear explanation and determine whether formal hearing tests can proceed safely. The image must be interpreted alongside the chronology: a normal drum in a person who woke unable to hear is very different from the same appearance in someone with slowly progressive difficulty following conversation.

The external auditory canal has a more mobile cartilaginous outer portion and a sensitive bony inner portion. The tympanic membrane separates the canal from the middle ear. Its larger pars tensa and superior pars flaccida should both be considered during inspection, together with the handle of the malleus, membrane position and surrounding margin. Looking only for a light reflex can miss clinically important abnormalities elsewhere. A light reflex changes with the viewing angle and cannot, by itself, prove or disprove infection.

Prepare the encounter before approaching the ear. Ask about pain, discharge, previous perforation, grommets, mastoid surgery and recent treatment. Establish which ear is symptomatic and what the person hopes the assessment will resolve. Explain that the instrument stays in the canal, that they should remain still, and that they should signal pain promptly. Speak while the person can see your face, particularly before removing a hearing aid. A child or someone who finds examination distressing may need demonstration, a supportive companion and a carefully agreed position.

The examination should distinguish observation from interpretation. For example, an amber drum with an air-fluid level suggests middle-ear fluid, whereas a markedly bulging opaque drum in an acutely unwell patient supports an inflammatory middle-ear process. Neither a red colour alone nor the mere presence of wax fully explains every symptom. Record what is visible and why a particular diagnosis is favoured, while preserving uncertainty when debris, anatomy or discomfort prevents an adequate view.

Canal disease and obstruction

Assess whether wax is merely present or actually obstructs examination or sound transmission. Oedema, tenderness, wet debris, a foreign body and focal skin lesions have different implications. Severe narrowing can conceal the membrane; do not assume it is intact because no perforation was seen.

Membrane contour and middle-ear fluid

Look for bulging, retraction, loss of translucency, a fluid level or air bubbles. Compare these with pain, fever, recent respiratory symptoms and hearing change. Redness can follow irritation or crying, so contour and the complete clinical pattern are more informative than colour alone.

Perforation and postoperative anatomy

Describe the site and approximate extent of a defect and whether discharge is present. Grommets, scars and a surgically altered cavity can change familiar landmarks. A known operation should prompt review of previous records rather than labelling an unfamiliar appearance as a new traumatic hole.

Retraction with retained skin

Inspect the attic and posterior membrane carefully for a deep pocket, crust or pale keratinous material. Persistent unpleasant-smelling discharge and unilateral hearing deterioration increase concern for cholesteatoma. The deepest part of a pocket may be hidden, so apparent surface size does not define disease extent.

Changes around the ear

Postauricular redness, swelling, focal tenderness, displacement of the pinna, vesicles or a previous surgical scar can redirect the examination. An ear complaint with systemic illness or cranial nerve findings needs assessment beyond the canal and drum.

A reassuring view with an unexplained symptom

A clear canal and intact membrane do not assess hair cells, the auditory nerve or central pathways. Persistent asymmetry, recurrent vertigo and sudden loss require further assessment even when the visible conductive pathway appears satisfactory.

Red flags requiring action

  • Sudden unexplained hearing loss needs time-critical referral even when the canal is clear and the tympanic membrane looks normal.
  • Discharge or hearing loss with new ipsilateral facial weakness or vestibular failure requires emergency ENT assessment; consider stroke when the neurological pattern suggests it.
  • Blood behind the drum after significant head trauma may indicate a skull-base injury and requires emergency assessment rather than routine ear cleaning.
  • A retraction pocket containing keratin, an unexplained polyp or persistent offensive discharge needs ENT assessment for destructive middle-ear disease.

Investigation order

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

Adapt to anatomy and stop if pain, bleeding or movement makes the procedure unsafe.

    Interpretation and limitations
    Otoscopy helps determine whether the ear is suitable for testing. Audiometry quantifies hearing rather than relying on membrane appearance, while tympanometry can support assessment of fluid or pressure abnormalities. Interpret any flat trace with its ear-canal volume and the examination.

    Resolve an obscured view or investigate a suspected structural complication.
    Interpretation and limitations
    ENT examination can clarify a retraction pocket, polyp or altered cavity and permit controlled cleaning when appropriate. Imaging is selected for the suspected problem; a routine scan is not necessary for every visible abnormality, and an incomplete community examination should be described honestly in the referral.

05Risks, monitoring and follow-upComplications, safety checks and further assessment.
  • Record the side, relevant symptoms, canal condition, membrane integrity and contour, plus any region that remained unseen.
  • After treatment of an obstructing or inflammatory problem, reassess both the physical findings and the hearing complaint; an improved canal does not establish recovered cochlear function.
  • Ensure a specific review or referral plan exists for persistent effusion, recurrent discharge, a perforation or a retraction whose full depth could not be examined.
  • Give clear instructions to seek earlier help for sudden hearing deterioration, facial weakness, severe dizziness, increasing pain, postauricular swelling or systemic illness.
  • If a video image is stored, label the correct ear and date and follow the service's consent and information-governance arrangements; an image should supplement an adequate clinical record.

The whole membrane may need several views

A single central view can leave the attic or anterior margin unseen. Small changes in the otoscope angle or the patient's head position can improve access, but repeated painful probing is not a substitute for suitable equipment or specialist help.

An opaque patch is not always active disease

Old scarring or tympanosclerosis can alter the drum after infection or grommet placement. Distinguish a stable plaque within the membrane from retained debris in a pocket or a new mass behind it, using the history and the surrounding contour rather than colour alone.

Pain and hearing severity need their own assessment

Marked otalgia may arise outside the ear when otoscopy is normal, and major hearing loss can occur with no visible lesion. Consider the wider head and neck or neurological examination according to symptoms rather than forcing the complaint to fit a minor incidental finding.

Communication continues throughout the procedure

Removing a hearing aid and turning away can make instructions inaccessible. Agree a stop signal in advance, explain changes before moving the instrument, and restore communication support promptly after the examination. Assess understanding of the resulting plan, not just agreement to the procedure.

Avoid assumptions about an operated ear

Mastoid cavities, grafts and reconstructed membranes can require different cleaning and surveillance. Ask who normally follows the ear, recover the operative history where possible, and seek advice before introducing fluid or performing a procedure unfamiliar to the examiner.
