Wax, a retained object and a torn tympanic membrane can all produce a blocked-ear complaint, yet the correct intervention differs substantially. The first decision is whether the ear is suitable for routine care at all. Establish the onset of symptoms, any attempted cleaning, previous perforation or surgery, and whether a hearing-aid component or another object may be missing. Examine before recommending drops or a removal method whenever the history suggests trauma, discharge or a non-routine ear.
Cerumen supports the protective environment of the canal and usually migrates out without assistance. An accumulation becomes clinically relevant when it interferes with hearing, causes other symptoms or obstructs a necessary examination or procedure. Removal is not required simply because some wax is visible. Cotton buds and improvised instruments can push material medially and abrade the canal, converting a minor obstruction into pain, infection or membrane injury.
Foreign bodies differ in their capacity to cause damage. A small inert object may remain relatively quiet, whereas organic material can provoke inflammation and become harder to retrieve with delay. A button battery presents a much more urgent tissue-injury risk. Deep location, a smooth object that cannot be securely grasped, poor cooperation or previous unsuccessful attempts all reduce the safety of further manipulation outside a suitably equipped service.
A traumatic perforation can follow instrumentation, a blow, blast or pressure change. The resulting hearing deficit may be conductive, but the same event can injure the ossicular chain or inner ear. The size of the visible hole does not explain every aspect of the presentation. Record vestibular symptoms and facial movement, and assess for associated head trauma when the mechanism warrants it. Conservative healing advice is appropriate only after more serious injury has been considered.

Narrow canals, hearing devices and self-cleaning can contribute to retained wax. Objects inserted deliberately or accidentally may obstruct the canal, and attempts at removal can worsen impaction or damage its delicate skin.
Mechanical or pressure injury
A direct instrument, slap, explosion or abrupt pressure difference may tear the tympanic membrane. Associated injury depends on the force and mechanism rather than simply whether a perforation is visible.
Retained material with different hazards
Inert items, organic matter and batteries do not have equivalent urgency. Organic material can inflame the canal, while a button battery can rapidly damage tissue and requires immediate specialist action.

Ask specifically about a known hole, ventilation tube, prior surgery, an only-hearing ear, troublesome vertigo and complications during previous clearance. Diabetes, immunocompromise and anticoagulants also influence risk assessment, though they do not create an identical prohibition for every method.
Identify the suspected material and its position without probing behind it. A hearing-aid dome or earplug fragment may be unrecognised by the patient. Organic material deserves prompt ENT assessment; NHS GGC specifies review within 24–48 hours, while an uncomplicated inert object can usually follow a routine specialist route.
Pain, blood, tinnitus or hearing change immediately after an injury supports the diagnosis. Examine gently and avoid introducing fluid. Severe imbalance, facial signs or significant head injury should change the urgency and scope of assessment.

Infection or chronic middle-ear disease
Discharge and obstructing debris may reflect external-ear infection, a perforated middle-ear infection or cholesteatoma. Clearing presumed wax without recognising the underlying disease can delay the necessary medical or surgical assessment.
Deeper traumatic damage
Persistent major hearing impairment, severe vertigo or facial dysfunction after injury raises concern beyond an uncomplicated membrane tear. Ossicular, inner-ear or temporal-bone injury needs assessment directed by the mechanism and associated findings.

Canal trauma and infection
Abrasion can follow either the original object or its attempted extraction. Discomfort, swelling, discharge or worsening hearing after ear care requires review, particularly in someone with increased infection or bleeding risk.
Persistent perforation or hearing deficit
Some tears fail to close, and hearing may remain impaired despite membrane healing if another structure was injured. Continued symptoms require otoscopic and audiological review, with ENT assessment when recovery is incomplete.
Battery-related tissue destruction
A retained battery can damage canal and middle-ear structures, with possible lasting hearing consequences. Its identification changes the problem from routine removal to an emergency requiring immediate access to appropriate extraction expertise.

Routine protocols have a defined scope
The BSA aural-care guidance addresses routine wax removal. Its restrictions should not be misread as saying that an ENT specialist can never use suction around a perforation or operated ear. Such ears require a different risk assessment and appropriate expertise, rather than automatic use of a community routine protocol.
Microsuction still has risks
Suction can be noisy and can provoke discomfort, vertigo or a temporary hearing change. An only-hearing ear, troublesome tinnitus or previous adverse reactions deserves particular care. Consent should explain the chosen method's practical risks and establish a signal to stop immediately if symptoms occur.
Flying advice depends on the actual condition
The NHS advises that flying with a perforated drum itself is generally safe, but a person who has undergone myringoplasty should wait until the surgical team says flying is safe. Do not substitute advice for a fresh postoperative ear for advice about an uncomplicated unoperated perforation.
A successful extraction is not the final assessment
Once the object or wax has gone, inspect what it concealed and ask whether hearing has returned to the patient's baseline. A persistent defect, infection or unexplained deficit can remain clinically important even after an apparently straightforward procedure.
