Dyslipidaemia and statin treatment

Interpret lipid patterns in cardiovascular context, identify secondary and familial disease, and use statins and add-on therapy to evidence-based UK targets with safe monitoring.

Time-critical presentation

A lipid result alone rarely requires emergency treatment. Same-day assessment is needed for triglycerides with acute pancreatitis symptoms, rhabdomyolysis with weakness or dark urine, or an acute coronary, cerebrovascular or limb-ischaemia syndrome; do not delay vascular treatment to obtain a fasting lipid profile.

01Purpose and principlesWhat the treatment does and how it fits into care.

Dyslipidaemia is a pattern, not one number. Raised LDL drives atherosclerosis; high triglycerides can mark metabolic disease and, at severe concentrations, pancreatitis risk; low HDL often reflects insulin resistance but is not a direct drug target. Non-HDL cholesterol captures all atherogenic apoB-containing particles and remains usable in non-fasting samples.

Treatment intensity follows absolute cardiovascular risk and disease status. QRISK3 supports primary prevention in appropriate adults, but is not needed when vascular disease already exists and can underestimate lifetime or disease-specific risk in familial hypercholesterolaemia, significant chronic kidney disease and some diabetes contexts. Discuss absolute benefit, adverse effects, interactions and the option to revisit rather than framing the decision as compliance.

Statin-associated symptoms require structured dechallenge, differential assessment and rechallenge when safe. Exercise, hypothyroidism, vitamin D deficiency, inflammatory myopathy and drug interactions can mimic or amplify muscle complaints. A lower dose, different statin or nondaily specialist strategy often preserves benefit; true severe myopathy or rhabdomyolysis requires immediate cessation and acute care.

  • Measure baseline liver transaminases, renal function and diabetes status; check creatine kinase before treatment only when persistent unexplained muscle symptoms are present.
  • Mild stable transaminase elevation below three times the upper limit does not routinely exclude statin treatment, including in steatotic liver disease.
  • Repeat lipids and liver tests two to three months after starting or changing treatment, then act on adherence, response and adverse effects rather than prescribing without feedback.
  • When targets are not reached on maximally tolerated statin, add or refer for ezetimibe, inclisiran, PCSK9 therapy or bempedoic-acid pathways according to indication and current commissioning.
  • Pregnancy and breastfeeding require a separate lipid-specialist plan; routine statins are stopped before conception and avoided during pregnancy under current UK product guidance.
02Indications, selection and cautionsWho may benefit, who needs urgent treatment and important alternatives.
LDL-predominant pattern

Raised LDL and non-HDL with relatively normal triglycerides suggests primary hypercholesterolaemia, diet, hypothyroidism, nephrotic disease or familial hypercholesterolaemia depending on magnitude and history.

Atherogenic metabolic pattern

Raised triglycerides, low HDL and small dense LDL tendency often accompanies central adiposity, type 2 diabetes, steatotic liver and alcohol excess; address each risk factor rather than targeting HDL pharmacologically.

Familial clue

Very high untreated cholesterol, tendon xanthomata or premature coronary disease in the patient or close relatives should trigger an FH pathway, not ordinary QRISK-based reassurance.

Statin myalgia

Symmetrical proximal aching or weakness temporally related to treatment may be statin-associated, but examination, CK when symptomatic, thyroid status, exercise and interacting medicines refine causality.

RhabdomyolysisRed flag

Severe muscle pain or weakness, dark urine, marked CK rise, hyperkalaemia or acute kidney injury demands immediate statin cessation, hospital assessment and interaction review.

Severe triglyceride stateRed flag

Triglycerides above 10 mmol/L, particularly above 20 or with abdominal pain, require urgent fasting confirmation, secondary-cause treatment and specialist planning for pancreatitis prevention.

Reduces LDL-related atherosclerotic events in people whose absolute or lifetime risk justifies pharmacological prevention.

Atorvastatin for primary prevention

Take 20 mg orally once daily when NICE primary-prevention criteria and shared decision-making support treatment; increase intensity if the non-HDL response remains below target after adherence review.

Avoid in pregnancy and breastfeeding; review liver disease, grapefruit intake and strong CYP3A4 interactions. Investigate significant muscle symptoms and use a lower or different statin when tolerability limits benefit.

High-intensity secondary prevention after coronary, cerebrovascular or peripheral arterial disease.

Atorvastatin for secondary prevention

Take 80 mg orally once daily for established cardiovascular disease unless interactions, adverse-effect risk or patient preference support a lower dose, then maximise the tolerated intensity.

Do not delay acute vascular care for baseline lipids. Check current BNF interactions, hepatic status and reproductive plans; severe weakness, dark urine or marked CK elevation requires immediate cessation and assessment.

Reduces intestinal cholesterol absorption and provides additional LDL and non-HDL lowering.

Ezetimibe

Take 10 mg orally once daily, usually added to maximally tolerated statin or used when statin therapy is contraindicated or genuinely not tolerated under NICE guidance.

Review liver tests when combined with statin, pregnancy and breastfeeding, ciclosporin and biliary disease. Confirm adherence and the indication before escalating to injectable therapy.

Provides substantial additional LDL lowering for selected high-risk patients not at target on tolerated oral therapy.

Inclisiran or PCSK9-targeted therapy

Use only the product-specific subcutaneous loading and maintenance regimen through the current NICE-commissioned specialist or primary-care pathway after eligibility and baseline LDL are confirmed.

Eligibility thresholds, commissioning and administration ownership differ by product and jurisdiction. Review pregnancy, injection reactions, adherence to background therapy and follow-up lipids; do not treat a laboratory number without the indicated risk context.

06Complications, monitoring and follow-upAdverse effects, response and longer-term review.
  • Repeat full lipid profile and liver transaminases two to three months after starting or changing lipid-lowering treatment and document percentage non-HDL change.
  • For primary prevention, seek more than 40% non-HDL reduction; for secondary prevention, seek LDL at most 2.0 or non-HDL at most 2.6 mmol/L.
  • Measure liver transaminases again at twelve months and thereafter only when clinically indicated, rather than stopping effective therapy for routine minor fluctuation.
  • Ask about adherence, affordability, formulation, interactions and muscle symptoms before declaring pharmacological failure or escalating treatment.
  • Check CK only when muscle symptoms occur, and monitor renal function, potassium and urine in suspected rhabdomyolysis through acute care.
  • Review diabetes risk and HbA1c according to the person's baseline status without withholding statin when cardiovascular benefit outweighs the modest glycaemic effect.
  • Revisit pregnancy plans and breastfeeding at relevant reviews, especially in younger people with inherited severe cholesterol who need specialist continuity.

Non-HDL survives lunch

Because it is total cholesterol minus HDL, non-HDL remains useful in a non-fasting sample and captures remnant as well as LDL atherogenic particles.

Risk calculators have boundaries

Established vascular disease and familial hypercholesterolaemia already define high risk; asking QRISK to rediscover them can falsely defer necessary treatment.

Symptoms deserve a rechallenge plan

A pause, resolution check and alternative statin can distinguish causality and retain prevention benefit more effectively than a permanent allergy label after one episode.

Fatty liver permits statins

Steatotic liver commonly coexists with vascular risk, and transaminases below three times the upper limit do not by themselves contraindicate statin treatment.

Targets follow indication

Primary prevention uses percentage non-HDL reduction, while secondary prevention now has absolute LDL or non-HDL targets; recording the indication prevents ambiguous review.
