Exogenous steroid suppression and safe withdrawal

Identify hypothalamic-pituitary-adrenal suppression from every glucocorticoid route, separate withdrawal from disease relapse, and taper safely while maintaining stress cover and crisis readiness.

01Purpose and principlesWhat the treatment does and how it fits into care.

The taper has two phases with different purposes. Above physiological equivalence, dose reduction is governed primarily by control of the disease for which the steroid was prescribed. At and below physiological exposure, the question becomes whether endogenous ACTH-cortisol function can recover without symptomatic deficiency. The initiating clinical team owns the first decision and should involve endocrinology when recovery is unclear.

Suppression is not confined to tablets. Potent inhaled preparations, extensive topical use, intra-articular or intramuscular injections and combined routes can produce systemic exposure. Ritonavir, cobicistat and other metabolic inhibitors can turn an apparently modest inhaled or injected dose into iatrogenic Cushing syndrome followed by prolonged adrenal insufficiency.

Safe withdrawal combines education and contingency planning. Temporary tiredness is common, but postural collapse, vomiting, hypoglycaemia or haemodynamic compromise is not an expected inconvenience. Patients and carers should know when to pause the taper, use sick-day dosing, inject emergency hydrocortisone and seek urgent help.

Key points

  • Glucocorticoid withdrawal syndrome can cause fatigue, anorexia, low mood, myalgia and arthralgia despite an adequate cortisol dose; distinguish it from adrenal insufficiency and recurrence of the treated disease.
  • Do not measure morning cortisol while oral glucocorticoid exposure is still at or above physiological replacement, because suppression is expected and the result will not answer whether tapering is safe.
  • When NICE-directed morning cortisol testing is indicated, withhold hydrocortisone, prednisolone or dexamethasone for their specified intervals and use local assay thresholds.
  • Any patient at meaningful suppression risk needs sick-day and procedure advice, an NHS Steroid Emergency Card where indicated, and immediate parenteral hydrocortisone if crisis is suspected.

Suppression risk

More than four weeks of adult glucocorticoid treatment, repeated courses, multiple routes or potent interacting medicines increases likelihood of tertiary adrenal insufficiency.

Iatrogenic Cushing phenotype

Central weight gain, bruising, proximal weakness, facial change, hypertension, diabetes or osteoporosis during treatment signals substantial systemic exposure and likely axis suppression.

Adrenal insufficiency

Weight loss, nausea, poor appetite, postural dizziness, hyponatraemia, weakness or hypoglycaemia after reduction suggests inadequate cortisol availability.

Withdrawal syndrome

Diffuse aches, fatigue, sleep disturbance and low mood may emerge below prior supraphysiological exposure even when haemodynamics and cortisol coverage are adequate.

Disease flare

Return of organ-specific inflammatory symptoms, objective markers or imaging abnormalities indicates recurrence of the original condition rather than endocrine failure alone.

Interaction-driven exposure

Unexpected Cushingoid change in a patient using inhaled or injected steroid should prompt review for CYP3A4 inhibition and hidden formulations.

Red flags requiring action

  • Hypotension, collapse, confusion, hypoglycaemia or severe vomiting after steroid reduction warrants immediate treatment for adrenal crisis without awaiting cortisol results.
  • Inability to retain oral steroid during gastroenteritis requires emergency injection and hospital assessment because repeating swallowed doses cannot guarantee absorption.
  • Severe breathlessness, neurological deficit, inflammatory organ dysfunction or other objective relapse features may require restoration of disease-treatment dosing alongside endocrine protection.
  • A patient taking ritonavir or cobicistat with a potent inhaled, injected or topical glucocorticoid needs urgent interaction review; abrupt removal can unmask profound suppression.
  • Major surgery or invasive procedure without a documented stress-dose plan exposes a suppressed patient to avoidable perioperative shock.

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

    Interpretation and limitations
    Using modern immunoassays, NICE considers below 150 nmol/L suggestive, 150 to 300 uncertain and above 300 very unlikely adrenal insufficiency; local assays may require different limits.

    Why
    Prevent exogenous drug from invalidating the planned morning cortisol measurement.
    Interpretation and limitations
    NICE advises pausing prednisolone 24 hours, hydrocortisone 12 hours or dexamethasone 72 hours before the sample, then restarting physiological cover as directed.

    Why
    Identify acute consequences and competing causes of symptoms during tapering.
    Interpretation and limitations
    Hyponatraemia or hypoglycaemia supports concern but can be absent; hyperkalaemia is not expected in isolated tertiary insufficiency because aldosterone is preserved.

    Separate endocrine symptoms from relapse of the condition treated by glucocorticoids.
    Interpretation and limitations
    Use organ-specific examination, inflammatory markers, spirometry, imaging or specialist assessment rather than assuming every symptom is steroid withdrawal.

    Clarify uncertain axis function when morning cortisol and clinical course do not resolve the question.

04Treatment approachPreparation, options, escalation and aftercare.

03ILLIntercurrent illness or procedureA patient with known or possible suppression develops physiological stress or is scheduled for intervention.
  1. 1Determine whether the patient remains on a therapeutic dose already sufficient for the expected stress and consult the procedure-specific guidance.
  2. 2For significant adult illness, provide at least the NICE oral sick-day exposure when absorption is reliable and reassess daily.
  3. 3If vomiting recurs or severe illness prevents absorption, administer 100 mg hydrocortisone intramuscularly or intravenously and arrange emergency care.
  4. 4Return from stress dosing only after the trigger resolves, then resume the pre-illness taper rather than stopping suddenly.
04CRISISAcute decompensation during withdrawalHaemodynamic compromise, altered consciousness, severe weakness or hypoglycaemia occurs after dose reduction.
  1. 1Give parenteral hydrocortisone at once and call for emergency support; diagnostic blood sampling is secondary to survival.
  2. 2Infuse 0.9% sodium chloride with frequent blood pressure, glucose and electrolyte reassessment, adapting volume to comorbidity.
  3. 3Continue the hospital hydrocortisone regimen and treat infection, trauma or another precipitating event with endocrine advice.
  4. 4Before discharge, analyse why prevention failed and replace the card, kits, prescriptions, teaching and taper ownership.
05Regimens, contraindications and interactionsTreatment details and the circumstances that modify them.
Prevent cortisol deficiency after therapeutic steroid exposure has fallen below the dose required for the original disease.

Physiological glucocorticoid cover

Use the NICE equivalent replacement range, commonly hydrocortisone 15 to 25 mg daily in divided doses, while assessing axis recovery.

Preserve a larger morning component, avoid chronic supraphysiological replacement and provide sick-day cover; a low morning cortisol is expected if testing is performed without the required pause.

Mimic the normal cortisol rise during fever, trauma or other meaningful stress when oral absorption remains reliable.

Oral sick-day hydrocortisone or prednisolone

During significant adult physiological stress, NICE advises at least 40 mg hydrocortisone daily or 10 mg prednisolone daily until recovery.

Patients already taking at least 10 mg prednisolone daily generally do not add more but may split the dose; prolonged unnecessary stress dosing increases adverse effects.

Restore essential glucocorticoid activity rapidly during shock, severe illness or failed gastrointestinal absorption.

Emergency parenteral hydrocortisone

Administer 100 mg intramuscularly or intravenously immediately when crisis is suspected or persistent vomiting prevents oral treatment.

Do not delay for cortisol testing and do not fear short emergency over-treatment; continue hospital dosing and fluid resuscitation until stable and absorbing tablets.

Maintain control of asthma, inflammatory, neurological, oncological or transplant disease while systemic exposure is reduced.

Original therapeutic glucocorticoid

Taper from disease-controlling doses according to the initiating specialty's activity markers before using the NICE endocrine withdrawal phase.

Potency and half-life differ; dexamethasone may be changed to prednisolone for final adult withdrawal, while interactions and combined routes can amplify exposure.

06Complications, monitoring and follow-upAdverse effects, response and longer-term review.
  • At each reduction, record current steroid equivalence, symptoms, blood pressure, weight and objective activity of the original illness with a clear next step.
  • Check whether the patient has an emergency card, understands sick-day rules and possesses usable injection kits when their risk category warrants them.
  • Review glucose, blood pressure, infection, bone and eye consequences from prior overexposure while avoiding a taper so fast that crisis replaces toxicity.
  • After low or indeterminate morning cortisol, maintain appropriate replacement and repeat or escalate testing only through the local endocrine pathway.
  • Reconcile every inhaled, topical, nasal and injected product at transitions of care, especially when antiviral or azole therapy changes.
  • Document recovery and safely stop emergency precautions only when the responsible clinical team concludes the axis and clinical course support it.

Therapeutic and endocrine tapers differ

The original disease controls reductions at high dose; adrenal recovery becomes the limiting issue only near physiological exposure.

Cushing features predict suppression

If exogenous steroid has been sufficient to cause a Cushingoid phenotype, assume meaningful axis suppression regardless of delivery route.

Aldosterone usually survives

Tertiary adrenal insufficiency lowers ACTH-driven cortisol but retains renin-angiotensin control of aldosterone, so fludrocortisone is not routine treatment.

Withdrawal is biologically real

Cytokine and neuropsychological symptoms can occur even with adequate cortisol, but this diagnosis should follow exclusion of crisis and disease relapse.

Testing needs pharmacology

Without an appropriate drug pause, assay cross-reactivity and residual suppression can turn a morning cortisol into a confidently misleading number.

08Common pitfallsFrequent interpretation and management errors.

    Do not stop a long glucocorticoid course abruptly because the original symptoms have improved; adrenal recovery often lags disease control.

    Do not test morning cortisol while an adult remains at or above physiological oral replacement and then diagnose permanent adrenal failure from the low value.

    Do not attribute every ache to adrenal insufficiency without checking recurrence of the treated disease and withdrawal syndrome.

    Do not overlook inhaled, topical, rectal, intranasal or injected exposure, particularly when strong CYP3A4 inhibitors are prescribed.

    Do not advise a vomiting patient simply to double tablets repeatedly; failed absorption is an indication for injected hydrocortisone and urgent care.

    Do not leave procedure cover to the day of surgery when a documented steroid-risk history could support a safe anaesthetic plan in advance.
