Latent autoimmune diabetes and ketosis-prone diabetes

Ketosis determines immediate treatment, not the eventual subtype label

Adults thought to have type 2 diabetes can present with DKA through slow autoimmune failure or a ketosis-prone phenotype. Vomiting, abdominal pain, deep breathing, dehydration, drowsiness or raised blood ketones demands acute assessment even if the person is older, has obesity or glucose is modest.

Action: Measure glucose, blood ketones, venous pH or bicarbonate, electrolytes and renal function and start the current DKA pathway when criteria are met. Do not delay insulin for antibodies or C-peptide, and do not attempt insulin withdrawal until recovery, reliable classification and specialist follow-up.

Recognise adult autoimmune diabetes and ketosis-prone phenotypes, treat acute insulin deficiency safely, and revisit long-term classification using antibodies and beta-cell reserve.

  • Latent autoimmune diabetes in adults describes slower autoimmune beta-cell loss that initially resembles type 2 diabetes but progresses towards insulin dependence.
  • GAD antibodies are commonly detected; local panels may include IA-2 or ZnT8, and testing is most informative close to diagnosis when clinical classification is uncertain.
  • Rapid deterioration, catabolic weight loss, ketosis, personal autoimmunity or failure of non-insulin therapy should prompt reassessment for autoimmune diabetes.

  2. Continue or establish background insulin according to the pathway and involve the inpatient diabetes team early for transition and education.
