The metabolic-syndrome concept is useful for recognising shared risk but can become a shortcut. A person does not gain or lose cardiovascular risk by crossing one arbitrary waist or triglyceride threshold. Record and treat the actual conditions: hypertension, prediabetes or diabetes, dyslipidaemia, central adiposity, smoking, sleep apnoea and kidney or vascular disease.
Steatotic liver associated with metabolic dysfunction is common in this cluster. Ultrasound may identify fat incidentally but is insensitive to mild steatosis and cannot stage fibrosis reliably. Liver transaminases reflect current cellular leakage, not scar burden; some people with cirrhosis have normal results. Once MASLD is established, a validated non-invasive fibrosis pathway decides who can remain in metabolic follow-up and who needs hepatology.
Treatment prevents both vascular and liver outcomes. Cardiovascular disease is a major cause of morbidity in MASLD, so a statin should not be withheld merely because liver fat is present. Weight loss can improve steatosis and metabolic risk, but interventions must remain nutritionally complete and free from stigma. Specialist pharmacological treatment directed at liver histology is evolving and should be checked against current NICE, licence and commissioning guidance rather than inferred from diabetes or obesity indications.

- An ELF score of 10.51 or above indicates advanced fibrosis in the NICE pathway and warrants specialist liver assessment; below threshold requires periodic reassessment.
- Statins are generally safe and important for vascular risk in MASLD; NICE only considers stopping if transaminases double within three months of starting.
- Weight management, physical activity, glycaemic care, blood pressure treatment, lipid lowering, sleep-apnoea care and smoking cessation form the core treatment package.
- People with cirrhosis need hepatology-led variceal, hepatocellular carcinoma, nutrition, vaccination and decompensation surveillance beyond a primary metabolic review.
02Assessment and patient selectionRisk features, eligibility and important cautions.
Central adiposity, hypertension, raised triglycerides, low HDL, acanthosis nigricans, prediabetes or type 2 diabetes commonly coexist; each component needs measurement and its own treatment threshold.
Increased echogenicity on ultrasound or fat reported on CT is often asymptomatic and should prompt cause, alcohol, medicine and fibrosis assessment rather than an isolated repeat ALT.
Thrombocytopenia, splenomegaly, low albumin, prolonged INR, nodular liver, portal-vein changes or an elevated validated fibrosis test raises concern even when transaminases are normal.
Ascites, jaundice, variceal bleeding, encephalopathy, sarcopenia or hepatorenal dysfunction indicates advanced disease and requires urgent or expedited hepatology depending on physiological stability.
Marked enzyme rise, cholestasis, systemic illness, drug timing, viral risk, autoimmune features or iron overload may indicate another or additional liver disease and needs targeted investigation.
Snoring, witnessed apnoeas and daytime sleepiness often accompany central metabolic risk; untreated OSA worsens wellbeing and blood pressure and warrants a validated sleep pathway.

Use the NICE indication-based oral dose, commonly 20 mg once daily for eligible primary prevention or 80 mg once daily for secondary prevention, adjusted for interactions and tolerability.Fatty liver alone is not a contraindication. Check baseline transaminases and the current BNF; NICE suggests considering cessation if liver enzymes double within three months, while pregnancy and severe active liver disease need specialist review.
Pioglitazone in specialist-selected advanced fibrosis
Use only an endocrinology or hepatology-selected once-daily regimen within the current NICE and diabetes context after discussing uncertain liver benefit and patient-specific risks.Can cause weight gain, oedema, heart failure, fractures and other adverse effects; review bladder history, pregnancy and liver function. Do not prescribe solely for an incidental fatty ultrasound in primary care.
06Targets, monitoring and follow-upResponse, safety and longer-term review.
- Repeat ELF at the NICE interval, generally every three years in adults below the advanced-fibrosis threshold, or sooner when clinical evidence changes through specialist advice.
- Monitor HbA1c, blood pressure, lipids, renal function, smoking and sleep-apnoea treatment according to each condition's guideline rather than a single annual syndrome panel.
- After statin initiation or dose change, repeat lipids and liver transaminases at two to three months and interpret enzyme trend in context.
- Track platelets, albumin, INR, bilirubin, weight and symptoms when fibrosis risk is present; decompensation features override the planned routine interval.
- Review alcohol quantity and pattern without stigma, offering evidence-based support and revisiting because exposure and metabolic vulnerability can change over time.
- In cirrhosis, follow the hepatology schedule for ultrasound-based hepatocellular-carcinoma surveillance, variceal assessment, vaccination, nutrition and medicine safety.
- Assess nutrition and muscle function during substantial weight loss, particularly in older adults or advanced liver disease where sarcopenia worsens outcome.

ALT is not a scar gauge
Transaminases can normalise as cirrhosis advances because fewer hepatocytes remain inflamed; fibrosis assessment needs validated markers and clinical context.
Fat and alcohol can coexist
Metabolic risk does not cancel alcohol-related injury, and alcohol use does not make central adiposity irrelevant; quantify both rather than choosing one label.
Cardiovascular care saves livers too
Exercise, diabetes control, blood-pressure care and smoking cessation reduce whole-person risk; avoiding statins because of steatosis may increase the larger immediate hazard.
Fibrosis directs follow-up
Simple fat on imaging does not predict an individual's liver outcome as well as scar burden, which is why the ELF step changes referral and surveillance.
High BMI can hide malnutrition
Cirrhosis, rapid loss or severe dietary restriction can deplete muscle and micronutrients even when weight remains above an obesity threshold.
