Chronic pancreatitis

Diagnose irreversible pancreatic disease, treat pain without stigma, replace exocrine function, detect type 3c diabetes and malnutrition, and select duct or cancer pathways safely.

Time-critical presentation

Progressive jaundice, rapid unexplained weight loss, a new mass, gastrointestinal bleeding, fever, persistent vomiting or a substantial change in pain requires urgent re-imaging and specialist review for pancreatic cancer, obstruction, pseudocyst, infection or bleeding rather than automatic attribution to chronic pancreatitis.

01OverviewDefinition, clinical context and the essential points that orientate the chapter.

Repeated injury activates fibrosis, distorts pancreatic ducts and reduces acinar and islet tissue. Pain may arise from inflammation, duct hypertension, stones, strictures, pseudocysts, neural remodelling or complications outside the pancreas. Symptoms fluctuate, and some people become less painful as the gland fails; pain relief therefore does not prove disease resolution. Diagnosis integrates history, cross-sectional imaging and exocrine/endocrine assessment. Normal early CT does not exclude disease, but neither do non-specific pain and alcohol exposure establish it.

Exocrine pancreatic insufficiency causes maldigestion before overt vitamin levels necessarily fall. Stool becomes oily, difficult to flush, frequent or pale; weight and muscle decline despite intake. Pancreatic enzyme replacement therapy supplies lipase, protease and amylase at the same time as food. Poor response usually prompts a review of adherence, timing, dose, product availability, acid inactivation and alternative conditions such as coeliac disease, bile-acid diarrhoea or small-intestinal bacterial overgrowth rather than immediate abandonment.

Long-term care is multidisciplinary. Pain treatment combines alcohol and smoking support, non-opioid and neuropathic strategies, psychology, dietetics and anatomical correction when appropriate. NICE places surgery first-line for painful chronic pancreatitis with main pancreatic duct obstruction, while shock-wave lithotripsy may suit a dominant stone when surgery is unsuitable. Diabetes, osteoporosis, pseudocysts, biliary or duodenal obstruction, thrombosis and cancer risk need planned surveillance. Hereditary pancreatitis requires specialist genetic counselling and a defined cancer-monitoring programme rather than improvised population screening.

  • Chronic pancreatitis is progressive fibro-inflammatory pancreatic damage causing recurrent or persistent upper-abdominal pain, ductal or calcific structural change and eventual exocrine or endocrine failure.
  • Do not assume alcohol causation because a person drinks: smoking, genetic variants, autoimmune disease, recurrent acute episodes, obstruction, anatomical factors and metabolic disease require consideration.

  • NICE advises exocrine and nutrition assessment at least annually, HbA1c at least every six months and bone-density assessment every two years in adults with chronic pancreatitis.
  • New weight loss, jaundice, a changed pain pattern, duct cutoff, worsening diabetes or gastric-outlet symptoms require prompt pancreatic-cancer assessment rather than automatic attribution to chronic disease.

Alcohol and smoking

Sustained harmful alcohol exposure is an important cause, while smoking independently accelerates progression and compounds cancer and cardiovascular risk.

Recurrent or inherited injury

Repeated acute pancreatitis, genetic susceptibility, metabolic disorders and anatomical variants can drive chronic fibro-inflammatory damage, including in younger patients.

Autoimmune or obstructive disease

Autoimmune pancreatitis, duct strictures, stones and pancreatic tumours can produce chronic inflammation and require cause-specific treatment rather than a lifestyle assumption.

    Recurrent inflammation and cellular stress activate fibrogenic pathways, replacing functioning pancreatic tissue with scar and distorted ducts.

    Strictures, stones and altered duct pressure combine with neural inflammation and central sensitisation to produce variable recurrent or persistent pain.

    Exocrine and endocrine loss

    Progressive acinar and islet destruction reduces digestive enzymes, bicarbonate and insulin, causing maldigestion, deficiency and pancreatogenic diabetes.

Pain-predominant disease

Recurrent or continuous epigastric pain radiating to the back, often aggravated by food, may precede obvious malabsorption or diabetes by years.

Exocrine insufficiency

Steatorrhoea, weight loss, bloating, diarrhoea, sarcopenia and vitamin or mineral deficiency indicate inadequate digestive enzyme delivery and require nutrition support.

Type 3c diabetes

Hyperglycaemia arising after pancreatic damage can be brittle because insulin and glucagon secretion are impaired and intake or absorption fluctuates.

Obstructed duct phenotype

A dilated main duct upstream from dominant stone or stricture with pain suggests pressure-related disease suitable for specialist decompression or surgery assessment.

Cancer or acute complicationRed flag

Progressive jaundice, rapid unexplained weight loss, a new mass, gastrointestinal bleeding, fever, persistent vomiting or substantially altered pain requires urgent re-imaging and specialist review.

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

    Interpretation and limitations

    Interpretation and limitations
    Minor EUS features are not perfectly specific and should not label non-specific pain in isolation; focal lesions need MDT-context sampling.

    Why
    Support diagnosis of moderate or severe pancreatic exocrine insufficiency in a compatible nutrition and stool phenotype.
    Interpretation and limitations
    Low concentration supports insufficiency, but watery diarrhoea dilutes the sample; a normal result may not exclude mild disease and PERT does not invalidate the assay.

    Why
    Assess weight trajectory, BMI, grip or muscle status, FBC, ferritin, albumin, magnesium and vitamins including D, adding others by phenotype.
    Interpretation and limitations
    Albumin is strongly influenced by inflammation and is not a standalone nutrition marker; combined deficiencies can mask expected blood-count patterns.

    Why
    Screen at least six-monthly for diabetes and investigate symptoms of hypo- or hyperglycaemia between planned checks.
    Interpretation and limitations
    HbA1c can be misleading with anaemia or rapid glucose change; type 3c classification informs insulin, education and hypoglycaemia planning.

    Why
    Measure bone density every two years in adults under NICE guidance because malabsorption and lifestyle factors increase fragility risk.

Enteric-coated pancreatin replacement

Prescribe an individual lipase-based amount with every meal and snack, spread through eating and titrated using current BNF and specialist guidance.

Swallow microspheres without crushing, avoid inhalation or oral mucosal contact, check porcine acceptability, and review very high doses because fibrosing colonopathy is reported.

Reduces gastric acid degradation of enzyme microspheres in selected people with continuing maldigestion.

Proton-pump inhibitor adjunct

Use a standard current BNF adult regimen as a specialist-informed trial when adequate correctly timed pancreatin remains clinically ineffective.

First correct timing, dose and adherence; review infection, magnesium, bone and long-term indication risks and do not mask another malabsorption cause.

Replaces inadequate endocrine secretion when non-insulin measures cannot safely control pancreatic diabetes or catabolic symptoms occur.

Insulin for type 3c diabetes

Choose a personalised basal-bolus or other specialist diabetes regimen with glucose monitoring matched to meals, malabsorption and hypoglycaemia risk.

Glucagon response may be impaired and intake variable; teach hypoglycaemia treatment, driving rules, sick-day care and injection-site rotation, and coordinate PERT.

Targets a neuropathic component of chronic pancreatic pain as part of broader multidisciplinary treatment.

Neuropathic pain medicine

Use one NICE-recommended neuropathic-pain option with slow titration, a defined functional target and scheduled assessment of benefit and harm.

Sedation, dizziness, falls, mood effects, renal dosing and dependence potential vary by agent; stop gradually when ineffective and avoid unsafe polypharmacy.

Pancreatic exocrine insufficiency

Maldigestion causes steatorrhoea, weight loss, sarcopenia and fat-soluble vitamin deficiency, sometimes before patients recognise oily stool.

Type 3c diabetes

Loss of islet hormones produces diabetes with variable insulin needs and vulnerability to hypoglycaemia, particularly alongside malnutrition or alcohol use.

Ductal and vascular complications

Pseudocysts, biliary or duodenal obstruction, splenic-vein thrombosis, pseudoaneurysm bleeding and elevated pancreatic cancer risk require specialist follow-up.

09Monitoring and follow-upTreatment response, safety checks and longer-term review.
  • At least annually, review stool, weight, dietary intake, muscle function, FBC and relevant vitamins or minerals with the specialist-centre protocol.
  • Record HbA1c at minimum six-month intervals and assess symptoms or capillary glucose sooner when diabetes or hypoglycaemia is suspected.
  • Arrange two-yearly adult bone-density assessment, then integrate the result with fracture history, falls, vitamin D, calcium and smoking.
  • During PERT titration, follow capsule timing, amount eaten, stool quality, weight and supply continuity rather than relying only on repeat faecal elastase.
  • For long-term analgesics, assess pain interference, function, mood, constipation, sedation, early refill and overdose risk at every structured review.
  • Escalate a new mass, duct cutoff, jaundice, rapid weight loss or sudden diabetic deterioration to the pancreatic cancer pathway.

Pain can fade late

Reduced pain may accompany gland burnout while malnutrition and diabetes worsen, so it is not necessarily recovery.

Enzymes meet food

PERT taken long before or after eating cannot mix effectively with the meal, even if the nominal daily dose seems high.

Elastase can dilute

A watery sample may create a spuriously low concentration and should be repeated on a formed specimen when doubt matters.

Type 3c is unstable

Loss of both insulin and glucagon, inconsistent absorption and alcohol or reduced intake make hypoglycaemia unusually hazardous.

Cancer changes the baseline

A new pattern deserves new imaging; long-standing pancreatic pain is not a licence to normalise progressive alarm features.
