Epigastric symptoms require precise language. Epigastric pain syndrome describes pain or burning centred above the umbilicus; postprandial distress features early satiation and uncomfortable fullness. Heartburn rises retrosternally, while biliary pain generally builds to a sustained severe epigastric or right upper-quadrant episode and is not simply meal-responsive indigestion. Pancreatitis produces persistent pain, often radiating to the back, with vomiting and systemic illness. Peptic ulcer disease may cause localised dyspepsia or present only when it bleeds or perforates. No timing rule reliably distinguishes benign from serious disease, so physiology and warning features come first.
Assessment should identify cancer risk and reversible causes. Record age, symptom onset and progression, dysphagia, odynophagia, weight trajectory, vomiting, bleeding, anaemia, family history and previous ulcer or gastric surgery. Ask about NSAID and aspirin exposure, including over-the-counter products, tobacco and alcohol. Examination looks for pallor, cachexia, nodes, oral signs, jaundice, abdominal mass, organomegaly and tenderness; a normal examination is common in both functional dyspepsia and early structural disease. Apply the current NICE NG12 recommendations rather than memorising an isolated age threshold because symptom combinations and pathway wording can change.
In a stable person without an urgent endoscopy indication, choose between non-invasive H pylori test-and-treat and empirical PPI treatment according to NICE and local practice. Urea breath testing or monoclonal stool antigen testing requires removal of PPI suppression for two weeks; recent antibiotics can also cause false-negative results. If symptoms persist after eradication or adequate acid suppression, confirm what was actually taken, revisit warning features and consider endoscopy or alternative imaging. A normal endoscopy with a compatible phenotype supports functional dyspepsia, which is managed through explanation, regular smaller meals, avoidance of individual triggers rather than blanket restriction, and selected pharmacological or psychological treatment.

Acid and mucosal disease
Helicobacter pylori, peptic ulcer, reflux and NSAID-related injury commonly cause upper abdominal burning or pain, sometimes with bleeding or anaemia.
Functional dyspepsia
Disordered gastric accommodation, visceral sensitivity and gut-brain signalling can produce early satiation, postprandial fullness or epigastric pain without explanatory structural disease.
Non-gastric pathology
Biliary, pancreatic, cardiac, vascular and malignant disease may present as epigastric discomfort and should be separated using timing, alarms and targeted tests.

Melaena, haematemesis, syncope or anaemia suggests bleeding; sudden severe generalised pain with rigidity suggests perforation. NSAIDs, aspirin, H pylori and previous ulcer disease raise probability.
Pressure, exertional association, breathlessness, sweating, radiation or cardiovascular risk should prompt ECG and acute coronary assessment. Reproducible meal association and antacid response cannot safely exclude myocardial ischaemia.
Chronic epigastric pain or meal-related fullness with no explanatory structural disease and no new warning feature supports a gut-brain interaction disorder. Symptoms remain real and may coexist with reflux or IBS.

- Ultrasound is first-line for gallstone questions, while CT or MR techniques answer pancreatic and ductal questions. Do not substitute endoscopy for a strong biliary hypothesis or vice versa.

Biliary or pancreatic disease
Right-upper-quadrant attacks, jaundice, back radiation, persistent severe pain or abnormal liver and pancreatic tests support hepatopancreatobiliary imaging.
Additional chapter-specific clues
Sustained right upper-quadrant or epigastric attacks, jaundice or fever point to biliary disease, while persistent back-radiating pain with vomiting and systemic change supports pancreatitis. These require targeted blood tests and imaging, not routine dyspepsia treatment alone.

Early satiety, pain or vomiting can reduce intake and cause dehydration, sarcopenia and deficiencies even when the disorder is non-malignant.
Delayed serious diagnosis
Repeated acid suppression without reviewing alarm features can postpone cancer, cardiac, biliary or pancreatic diagnosis and create avoidable harm.

Dyspepsia is a symptom family
Epigastric burning, early satiation and postprandial fullness reflect different mechanisms. Naming the dominant feature makes treatment and reconsideration more precise than calling every upper abdominal sensation reflux.
PPI response lacks specificity
Acid suppression can improve ulcer, reflux and functional symptoms through several pathways. Clinical improvement therefore supports continued management but cannot prove benignity or exclude cancer when warning features exist.
Test preparation changes truth
PPI and recent antibiotic exposure suppress H pylori and create false-negative breath or stool tests. Recording washout is part of interpretation, not merely an administrative instruction.
Eradication and cure differ
Prescribing an eradication pack does not establish microbiological success. Adherence, resistance and indication determine whether a correctly timed non-invasive confirmation test is required.
Meal symptoms can starve
