EoE reflects type 2 immune inflammation causing epithelial injury, remodelling and fibrosis. Untreated disease can progress from inflammatory furrows and exudates to rings, narrow calibre and strictures. The clinical clue may be dramatic impaction, but many people normalise years of slow eating and food avoidance. Reflux, infection, Crohn disease, connective-tissue disease, coeliac disease and drug reactions can also increase oesophageal eosinophils, so histology is interpreted with symptoms and distribution.
Diagnosis requires adequate sampling even when endoscopy appears normal. BSG/BSPGHAN guidance supports at least six biopsies across at least two levels, targeted to abnormalities as well as apparently normal mucosa. The histology report should provide peak eosinophil density and other features such as basal-zone hyperplasia or fibrosis where available. A failed PPI course is no longer required before the diagnosis is made, although PPIs remain a useful first-line treatment for some patients.
Treatment is shared and long term because relapse is common when therapy stops. Budesonide orodispersible tablets have NICE support for induction in adults and a defined licensed technique; swallowed topical steroid alternatives, PPIs and elimination diets depend on age, local commissioning and patient preference. Allergy tests do not reliably identify the food drivers for elimination. Diet restriction should be supervised by an experienced dietitian to protect nutrition and avoid unsafe broad avoidance.

Peptic stricture or ring
A focal narrowing may explain solid dysphagia, but multiple-level biopsies are required because EoE mucosa can appear normal.
Achalasia or cancer
Liquids-and-solids dysphagia with abnormal manometry suggests achalasia, while rapid progression, weight loss or an irregular lesion raises malignancy concern.
Additional chapter-specific clues
Rapidly progressive dysphagia, weight loss, anaemia, bleeding or an irregular mass requires urgent cancer and structural assessment even when eosinophilia is present.

For adult induction, the licensed dose is 1 mg allowed to dissolve on the tongue twice daily, morning and evening, for six weeks; inadequate responders may continue to twelve weeks under specialist direction.Do not eat, drink or perform oral hygiene for at least thirty minutes after dosing under the SmPC. Oropharyngeal or oesophageal Candida can occur; review adrenal, interaction, hepatic and systemic steroid risks, and distinguish induction commissioning from maintenance access.
Proton pump inhibitor
A gastroenterologist selects a full-dose licensed PPI regimen, often once or twice daily during induction according to local EoE practice, reflux burden and the current BNF; reassess rather than continuing blindly.Response does not mean the original diagnosis was merely GORD. Review adherence, interactions and long-term need; biopsy is usually required to demonstrate mucosal response because symptoms alone are unreliable.

Food impaction

Stricture and narrow-calibre oesophagus

Perforation and nutritional restriction
Impaction, instrumentation or severe retching can tear the oesophagus, while unsupervised elimination diets may cause deficiency or impaired quality of life.

- After dilation, give written advice for expected transient discomfort and urgent review for severe pain, fever, breathlessness or haematemesis.
- Agree a maintenance plan because untreated recurrence can resume inflammation and fibrosis even after successful induction.

Ask how, not just whether
Compensatory behaviours can hide dysphagia. A patient who avoids steak and takes forty minutes to eat may have substantial disease while answering 'no' to swallowing difficulty.
Normal endoscopy needs biopsies
A visually unremarkable oesophagus does not exclude EoE; inadequate sampling is a common reason for delayed diagnosis.
Symptoms and histology separate
Dilation or food avoidance can improve swallowing while active eosinophilic inflammation continues, so mucosal reassessment matters.
Allergy tests do not make the menu
Skin-prick and specific-IgE tests identify immediate allergy but predict EoE trigger foods poorly; elimination should be dietetic and response-based.
Dilation is not failure
Fibrotic narrowing may need mechanical treatment even when inflammation is controlled. The error is dilation without ongoing disease modification.
