Functional dyspepsia and disorders of gut-brain interaction

Diagnose functional dyspepsia positively after appropriate assessment, distinguish meal-related and pain phenotypes from dangerous structural disease, and deliver integrated gut-brain treatment without invalidating symptoms.

Time-critical presentation

Functional dyspepsia cannot explain haemodynamic instability, haematemesis, melaena, rigid peritonism, acute coronary features, jaundice with sepsis or persistent vomiting with severe dehydration. Stabilise and investigate these presentations urgently. Progressive dysphagia, iron-deficiency anaemia, an upper-abdominal mass, persistent vomiting or qualifying weight loss requires the current urgent endoscopy or suspected-cancer pathway, even when earlier endoscopy was normal.

Dyspepsia describes upper abdominal symptoms, whereas functional dyspepsia is a positive diagnosis within the disorders of gut-brain interaction. Identify the dominant experience: uncomfortable fullness after an ordinary meal, inability to finish a normal portion, localised epigastric pain or burning. Establish timing, meal relationship, weight trajectory, vomiting, reflux, bowel symptoms and medicine exposure. Delayed gastric emptying, impaired fundic accommodation, duodenal signalling, visceral hypersensitivity and central amplification may each contribute, but no routine biomarker assigns a mechanism to an individual. Functional dyspepsia commonly overlaps IBS, reflux, nausea syndromes and anxiety; overlap should broaden an integrated plan rather than create a sequence of competing labels.

Before making the diagnosis, triage structural and extra-gastrointestinal threats. Ask about dysphagia, bleeding, anaemia, progressive vomiting, mass, family history, NSAIDs and age-related cancer risk. Consider acute coronary syndrome when epigastric pressure is exertional or autonomic; gallstone disease when discrete sustained attacks reach the right upper quadrant or back; pancreatitis with persistent severe back-radiating pain and illness. In an uncomplicated presentation, NICE supports H pylori test-and-treat and a four-week full-dose PPI trial. Apply the current NG12 symptom combinations rather than memorising one age cut-off. Endoscopy answers alarm or persistent structural questions but should not become a periodic reassurance ritual.

Treatment needs a coherent explanation and staged choices. Regular modest meals may reduce accommodation demand, while aerobic activity and sleep support overall symptom regulation. Test and eradicate H pylori correctly and stop NSAIDs where feasible. A PPI may help even when the eventual diagnosis is functional, but response is not proof of acid disease and treatment should be stepped down after review. For persistent pain, a low-dose TCA is used as a gut-brain neuromodulator, generally at a dose below antidepressant treatment. A prokinetic may suit selected postprandial symptoms, but UK availability, cardiac risk and local formularies constrain choice. Dietetic review protects nutrition when early satiation causes restriction, and gut-directed psychological therapy can improve coping and symptom processing without implying psychiatric causation.

Intermittent pain or burning centred in the epigastrium without a consistent bowel relationship supports epigastric pain syndrome after ulcer, biliary, pancreatic and cardiac alternatives have been assessed.

Upper gastrointestinal alarmRed flag

Progressive dysphagia, haematemesis, melaena, iron-deficiency anaemia, persistent vomiting, palpable mass or qualifying unexplained weight loss activates urgent structural and cancer assessment rather than routine functional treatment.

Gastroparesis or obstruction signal

Repeated vomiting of food eaten hours earlier, severe postprandial distension, diabetes, connective-tissue disease or previous gastric surgery may justify structural exclusion and formal gastric-emptying assessment rather than an empirical label.

Investigation order

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

    Interpretation and limitations
    Hold PPI treatment for two weeks when clinically safe and observe the assay interval after antibiotics. A positive result warrants local eradication; serology can remain positive and generally cannot confirm cure.

    Identify iron deficiency or blood loss that changes endoscopy urgency and challenges a functional diagnosis.
    Interpretation and limitations
    Anaemia or depleted iron stores need explanation through the appropriate upper and lower gastrointestinal pathway. Normal indices do not neutralise progressive dysphagia or a palpable mass.

    A high-quality normal procedure supports functional dyspepsia, but only after reviewing whether H pylori, coeliac, biliary, pancreatic or motility questions remain unanswered.

    Investigate sustained upper-abdominal pain, jaundice or systemic illness for hepatobiliary disease and acute pancreatitis.
    Interpretation and limitations
    A cholestatic pattern supports biliary imaging, while lipase elevation with compatible pain supports pancreatitis. Routine normal results do not independently prove functional dyspepsia.

    Why
    Evaluate epigastric symptoms with exertional, autonomic or cardiovascular-risk features for acute coronary syndrome.
    Interpretation and limitations
    Follow the acute chest-pain pathway; one normal ECG may not exclude evolving ischaemia. Do not delay serial assessment while administering an antacid trial.

    Why
    Test delayed emptying only when persistent vomiting, diabetes, surgery or another strong motility phenotype remains after obstruction is excluded.
    Interpretation and limitations
    Use a validated specialist protocol and interpret medicines and glycaemia that alter emptying. Delayed emptying and symptom severity correlate imperfectly and require clinicophysiological review.

Anaemia, weight loss, persistent vomiting, bleeding, dysphagia or an abnormal examination requires endoscopy rather than a functional diagnosis.

Biliary or pancreatic disease

Discrete right-upper-quadrant attacks, jaundice, back-radiating persistent pain or abnormal imaging and enzymes indicate hepatopancreatobiliary pathology rather than functional dyspepsia.

Cardiac ischaemia or GORD

Exertional pressure and cardiovascular risk require cardiac evaluation, while dominant retrosternal heartburn and regurgitation favour reflux disease.

Proton-pump inhibitor

Use a current NICE full-dose preparation for a four-week empirical dyspepsia course, then step down to the lowest effective or intermittent strategy after review.

Stop for two weeks before H pylori breath or stool testing when safe. Review interactions, infection risk and long-term indication, and never let symptomatic response override alarm features.

Eradicates active H pylori, reducing ulcer risk and providing sustained symptom benefit for a minority with associated dyspepsia.

Helicobacter pylori eradication regimen

Use the locally recommended seven-day combination selected for penicillin allergy, prior macrolide or metronidazole exposure and regional resistance information.

Confirm active infection, allergies, pregnancy considerations and interactions. Failed treatment requires adherence and resistance-aware review; do not repeat the same antibiotics blindly or use serology to confirm cure.

Reduces epigastric pain through central and peripheral neuromodulation when first-line explanation, H pylori care and acid suppression are insufficient.

Low-dose tricyclic neuromodulator

Start with the local low nightly dose and increase slowly only after tolerability review; functional-dyspepsia dosing is generally below antidepressant treatment ranges.

Discuss sedation, dry mouth, constipation, urinary retention, glaucoma, falls, cardiac conduction and overdose risk. Obtain ECG or specialist advice when cardiovascular history or interacting medicines warrant it.

May improve meal-related fullness or nausea by modifying gastric motility in a subset whose symptoms fit that mechanism.

Specialist prokinetic

Select and dose only through the current local formulary for a clearly defined postprandial or delayed-emptying phenotype, using the shortest justified trial.

UK availability and licensed indications differ. Review QT and neurological risks, interactions and duration; do not prescribe metoclopramide or domperidone as indefinite unmonitored functional-dyspepsia therapy.

Phenotype sharpens treatment

Early satiation points towards meal accommodation and nutrition, whereas focal epigastric burning may respond more to acid or pain neuromodulation. The categories overlap but prevent indiscriminate prescribing.

Endoscopy has boundaries

A normal endoscopy excludes many visible mucosal lesions at that time; it does not test gastric emptying, biliary pain or every microscopic condition, nor guarantee against future disease.

Neuromodulator is descriptive

Low-dose TCA treatment targets visceral pain processing rather than declaring depression. Explaining this difference improves informed choice and avoids the damaging message that symptoms are imaginary.

Preparation determines H pylori truth

PPI and recent antibiotics suppress organism detection. A negative test without documented washout may generate false reassurance and inappropriate long-term acid treatment.

Food fear becomes pathology

Severe early satiation may promote progressive avoidance, weight loss and micronutrient risk. Early dietetic support is treatment, not an optional extra after every medicine fails.
