Most gallstones are cholesterol-rich and remain silent. Symptoms arise when a stone transiently obstructs the cystic duct, increasing gallbladder pressure without sustained inflammation. The classic label 'colic' is misleading: the pain is commonly constant rather than cramping. It often builds after food, lasts long enough to interrupt activity and resolves completely when the obstruction releases. Dyspepsia, isolated bloating and reflux are non-specific and should not be attributed to stones solely because an ultrasound reports them.
The central diagnostic task is to decide whether disease is still confined to the gallbladder. Persistent focal tenderness and systemic inflammation suggest acute cholecystitis; jaundice or a cholestatic liver profile suggests common-duct obstruction; fever or organ dysfunction with obstruction suggests cholangitis; severe epigastric pain radiating to the back with raised lipase suggests pancreatitis. Cardiac, pulmonary, peptic, hepatic and renal causes remain important alternatives, particularly when the symptom pattern is atypical.
Definitive care prevents recurrent attacks and complications. NICE advises laparoscopic cholecystectomy for symptomatic gallbladder stones, commonly as day surgery when appropriate. People with no symptoms and a normal biliary tree should not be medicalised. While waiting, provide realistic safety-netting rather than promising that dietary avoidance prevents migration; smaller or lower-fat meals may reduce an individual's triggers but do not dissolve established stones.
Key points
- Biliary colic is usually a steady epigastric or right-upper-quadrant pain lasting from roughly half an hour to several hours, often with nausea and sometimes radiating to the right scapula or back.
- Pain that persists, fever, rigors, jaundice, hypotension, confusion, peritonism or repeated vomiting is not reassuring biliary colic; assess urgently for cholecystitis, cholangitis, pancreatitis or another abdominal emergency.
- NICE recommends liver blood tests and transabdominal ultrasound when gallstone disease is suspected; a normal examination between episodes does not invalidate a characteristic history.
- Ultrasound establishes gallbladder stones and duct calibre but cannot reliably exclude a distal common-duct stone; abnormal liver tests or duct dilatation should trigger MRCP, with EUS when MRCP remains non-diagnostic.
- Incidental, asymptomatic stones in an otherwise normal gallbladder and biliary tree generally need reassurance rather than treatment under NICE CG188.
- Offer laparoscopic cholecystectomy for symptomatic gallbladder stones after shared assessment of operative fitness, pregnancy, anticoagulation, competing diagnoses and the person's priorities.
- Analgesia treats the episode but does not remove the stone reservoir; NSAIDs can be effective when renal, gastrointestinal, cardiovascular and pregnancy risks permit.
- Advise prompt reassessment for fever, jaundice, pain lasting longer than the person's usual attack, dark urine, pale stool or inability to maintain hydration while awaiting surgery.

Cholesterol supersaturation
Most UK gallstones form when bile contains excess cholesterol relative to bile salts and phospholipids, allowing crystals to nucleate.
Pigment stone formation
Increased bilirubin load, infection or biliary stasis promotes pigment stones, including in chronic haemolysis and altered duct anatomy.
Gallbladder stasis and host risk
Age, pregnancy, obesity, rapid weight loss, ileal disease and reduced gallbladder emptying modify stone risk without making symptoms inevitable.

Pain continuing beyond the expected attack, fever and local guarding or a convincing Murphy sign indicate sustained cystic-duct obstruction with inflammation rather than simple biliary colic.
Scleral or skin jaundice, dark urine, pale stool, pruritus or a cholestatic biochemical profile raises choledocholithiasis, stricture or malignancy and needs expedited imaging.
Rigors, fever, jaundice, hypotension, delirium or lactate elevation may represent ascending cholangitis; start emergency sepsis care and obtain urgent biliary-source-control advice.

Burning linked to acid, reflux symptoms or endoscopic ulceration favours upper luminal disease; incidental gallstones may not explain chronic dyspepsia.
Acute cholecystitis
Sustained pain, fever, focal peritonism and inflammatory gallbladder changes indicate persistent inflammation rather than uncomplicated biliary colic.
Pancreatic, cardiac or vascular pain
Back radiation with enzyme elevation, exertional symptoms or severe pain out of proportion requires evaluation beyond the biliary tree.
Additional chapter-specific clues
Chest symptoms, exertional association, dyspnoea, haematemesis, peritonism, pulsatile mass or pregnancy-related concern should redirect assessment towards cardiac, vascular, perforation, pulmonary or obstetric pathways.

Acute cholecystitis
Persistent cystic-duct impaction produces gallbladder oedema, infection, necrosis or perforation rather than a resolving pain episode.
Choledocholithiasis and cholangitis
Migrated stones obstruct the common duct, causing jaundice and potentially life-threatening infected biliary hypertension with sepsis risk.
Gallstone pancreatitis or ileus
Ampullary obstruction can trigger pancreatitis, while chronic fistulation may rarely allow a large stone to obstruct small bowel.

- Recheck temperature, heart rate, blood pressure, pain trajectory and abdominal signs before discharge from an acute episode.
- Review liver tests and duct imaging promptly if jaundice appears or attacks become more prolonged between referral and surgery.
- Track recurrent emergency presentations and expedite the surgical plan rather than repeatedly supplying analgesia without definitive ownership.
- After ERCP, observe under the local endoscopy protocol for pancreatitis, bleeding, infection and perforation, and document stent follow-up.
- Following cholecystectomy, investigate continuing biliary-type pain on its merits instead of assuming every symptom represents a retained stone.

Colic is often constant
The traditional name describes episodic obstruction, not necessarily a wave-like pain; renal or bowel colic is more typically cramping.
Stone size misleads
Small calculi may migrate into the common duct, while a large gallbladder stone can remain asymptomatic; symptoms and anatomy guide decisions.
Transient transaminitis matters
A short aminotransferase spike can accompany stone passage before a more recognisable cholestatic pattern develops or tests normalise.
Ultrasound has a blind spot
Bowel gas and distal duct anatomy limit direct visualisation, so a reassuring gallbladder scan cannot cancel credible biochemical obstruction.
Incidental is not symptomatic
A common radiological finding should not be used to explain non-specific indigestion without a compatible biliary pain pattern.
