Terminology and pathways are evolving. UK services may still use legacy NAFLD labels in documents and coding; clinicians should recognise the equivalence while documenting alcohol exposure and other causes carefully enough to identify combined aetiology.

Pioglitazone

Advanced fibrosis and decompensation
Progressive scarring causes portal hypertension, ascites, variceal bleeding and encephalopathy even when aminotransferases remain normal or mildly raised.
Hepatocellular carcinoma
Cancer risk rises substantially with cirrhosis and occasionally advanced pre-cirrhotic disease, linking accurate fibrosis staging to surveillance decisions.
Cardiovascular and metabolic disease
Myocardial infarction, stroke, diabetes progression and chronic kidney disease often dominate overall morbidity and require coordinated risk reduction.

Fibrosis drives prognosis
Steatosis quantity and ALT attract attention, but fibrosis stage is the strongest routine liver-related risk marker and should organise referral and surveillance.
Normal ALT can coexist with cirrhosis
Enzyme release measures current injury rather than scar burden. Platelets, synthesis, imaging and validated non-invasive fibrosis tests remain necessary in high-risk patients.
Cut-offs are pathway dependent
NICE and newer BSG-endorsed ELF material do not currently show the same threshold. Use one commissioned algorithm consistently and state it in the record.
Lean MASLD is real
Normal body mass index does not remove metabolic or genetic risk. Waist distribution, diabetes, lipids, ethnicity and sleep apnoea can reveal a high-risk phenotype.
Alcohol and metabolism combine
The newer name does not imply alcohol is irrelevant. Quantify exposure sensitively because combined injury may accelerate fibrosis and alter advice and coding.
Statins protect the main risk
Fear of mild stable transaminitis can lead to undertreatment of cardiovascular disease. Follow NICE monitoring rather than withholding indicated statin therapy reflexively.
