Severe pancreatitis is a systemic critical illness, not simply a more painful form of pancreatic inflammation. Cytokine release, endothelial leak and vasodilatation reduce effective circulating volume, impair lungs and kidneys and can cause distributive shock. Local necrosis may be present, but the extent of necrosis and the degree of early organ failure are not identical. The revised severity definition uses persistent organ failure beyond forty-eight hours because this is strongly associated with mortality and complex recovery.
Care requires simultaneous organ support and restraint. Oxygenation may deteriorate through atelectasis, pleural effusions, aspiration, inflammatory lung injury, fluid overload or pulmonary embolism. Renal injury may reflect hypoperfusion, inflammation, abdominal pressure, sepsis or nephrotoxins. Hypotension needs repeated haemodynamic assessment, cautious fluid challenge when responsive and vasopressors in critical care when vasodilatation persists. A protocol that ignores the patient's heart, lungs and cumulative balance can convert attempted resuscitation into harm.
Complications evolve over time. Early sterile necrosis and acute collections often need observation, analgesia and nutrition rather than procedures. Secondary infection more often appears later and is suspected from new sepsis, gas within a collection, positive cultures or clinical deterioration after initial improvement. Bleeding from pseudoaneurysm, bowel ischaemia, abdominal compartment syndrome and perforation are time-critical exceptions. The pancreatic MDT coordinates critical care, endoscopy, interventional radiology and surgery, favouring step-up and delayed intervention when physiology permits.
