Short-bowel physiology is defined by what remains, not only centimetres removed. Jejunum can adapt to some ileal functions, but terminal ileum uniquely recycles bile salts and absorbs B12. Colon in continuity ferments carbohydrate to absorbable short-chain fatty acids and conserves fluid. An end jejunostomy has no such rescue and can become a net secretor when output exceeds oral intake. Gastric hypersecretion is prominent early after major resection.
Intestinal failure is described by duration and context: type 1 is short-lived and often perioperative; type 2 is prolonged acute illness with sepsis, fistula or complex surgery; type 3 is chronic stable dependence on intravenous fluid or nutrition. Chronic care requires a multidisciplinary intestinal-failure service, specialist nurses, dietitians, pharmacists, surgeons and homecare support. Goals include independence, hydration, nutrition, line preservation, liver and kidney protection and possible restoration of continuity.
Home parenteral support is life-sustaining but creates catheter thrombosis, bloodstream infection, metabolic bone disease, renal injury and intestinal-failure-associated liver disease. Fever during infusion is an emergency, but automatic line removal can sacrifice access; management follows paired cultures and a catheter-salvage or removal protocol. Teduglutide, a GLP-2 analogue, may reduce parenteral requirements in selected stable adults under NICE TA804 and expert monitoring; it is not a substitute for foundational optimisation.

Extensive small-bowel resection
Mesenteric ischaemia, Crohn disease, volvulus, trauma and surgical complications can leave insufficient bowel length for fluid and nutrient autonomy.
Unfavourable remaining anatomy
Loss of ileum, ileocaecal valve or colon continuity increases bile-acid, fluid and sodium loss beyond what length alone predicts.
Functional intestinal failure
Severe dysmotility, fistulae, obstruction and radiation or mucosal disease can make anatomically present bowel unable to absorb adequately.

Too little functioning mucosa remains to absorb macronutrients, micronutrients, water and electrolytes from ordinary intake without additional support.

Output commonly above 1.5 to 2 litres daily with thirst, dizziness, low urine, cramps, weight loss, rising urea or creatinine and hypomagnesaemia suggests net intestinal fluid loss.
Rigors, fever, hypotension or confusion during or after infusion requires sepsis assessment, paired line and peripheral cultures and immediate intestinal-failure or infection contact.
Anaemia, neuropathy, bruising, night blindness, proximal weakness, fractures or poor wound healing can reflect B12, fat-soluble vitamin, mineral or trace-element deficiency.
Progressive cholestatic tests, hepatomegaly or fibrosis risk during long-term parenteral nutrition requires review of sepsis, overfeeding, lipid exposure, oral intake and anatomy.
Abruptly increased output with pain, fever, vomiting or inability to tolerate intake may represent infection, partial obstruction, recurrent disease or medication withdrawal rather than loss of adaptation.

Mucosal or pancreatic malabsorption
Coeliac disease and pancreatic insufficiency cause nutrient loss with preserved bowel length and have distinct serological, histological or pancreatic evidence.
Severe dysmotility
Dilated bowel, stasis and failure of propulsion without major resection suggests chronic intestinal pseudo-obstruction rather than anatomical short bowel.

High-dose loperamide
Specialist doses given 30 to 60 minutes before meals and at night may exceed routine licensed limits in intestinal failure; follow the current BIFA ECG and prescribing guidance.High exposure can prolong QT and cause serious arrhythmia; obtain baseline and follow-up ECG as advised. Exclude obstruction and avoid casual over-the-counter escalation.
Proton pump inhibitor
A current BNF oral or intravenous regimen selected for early gastric hypersecretion, absorption and ongoing output, then reviewed for continued need.Long-term hypomagnesaemia, infection, bone and medicine-interaction concerns. Benefit diminishes after adaptation in some patients, so reassess.
Glucose-saline oral rehydration solution
Specialist recipe or licensed formulation providing about 90 mmol/L or more sodium, sipped in a prescribed daily volume while hypotonic drinks are limited.Ordinary low-sodium sports drinks are inadequate. Cardiac or renal comorbidity and inaccurate home mixing require individual instruction.
Teduglutide
Once-daily specialist weight-based subcutaneous regimen under NICE TA804 and the current SmPC, with renal adjustment and response-led parenteral reduction.Fluid overload, stoma obstruction, biliary or pancreatic disease and potential neoplastic growth require screening and surveillance; stopping can rapidly increase fluid needs.

Dehydration and renal injury
Repeated sodium-rich losses cause thirst, magnesium depletion, kidney stones and progressive renal impairment despite drinking large volumes of water.
Malnutrition and parenteral dependence
Energy, protein and micronutrient deficits cause sarcopenia and bone disease and may necessitate long-term home parenteral support.
Line sepsis and intestinal-failure liver disease
Central access creates bloodstream-infection and thrombosis risk, while prolonged parenteral nutrition and sepsis can cause cholestatic hepatic injury.

- Record stoma output, oral fluids, urine volume, weight and postural symptoms daily during instability and at an agreed interval when stable.
- Check renal function, magnesium, potassium and sodium promptly after output or treatment changes and through chronic follow-up.

- Inspect the catheter and review infusion technique, fever episodes, thrombosis and line replacements to preserve venous access.

Serum sodium can look normal
Hormonal water conservation maintains the concentration while total body sodium and circulating volume are depleted; urine sodium and volume expose the deficit.
Plain water may worsen thirst
Hypotonic fluid entering a sodium-leaky jejunum can increase stomal sodium loss. High-sodium glucose solution changes the absorption gradient.
Colon is an energy organ
Colonic bacteria convert unabsorbed carbohydrate to short-chain fatty acids, reclaiming calories and fluid when colon remains in continuity.
Colon also increases oxalate risk
Unabsorbed fat binds calcium, leaving oxalate free for colonic uptake and calcium-oxalate renal stones.
A line is finite vascular capital
Evidence-led salvage can preserve access, but instability, tunnel infection and certain organisms make removal safer; the decision belongs to the specialist protocol.
