01Purpose and principlesWhat the assessment is for and the core concepts behind it.
A full blood count measures several related but biologically distinct systems. Haemoglobin and red-cell indices describe oxygen-carrying cells; the white-cell differential reflects innate, adaptive and clonal populations; platelets reflect production, consumption, destruction and sequestration. Interpretation begins by deciding whether one, two or three lineages are abnormal and whether the change is acute, chronic, progressive or intermittent. Recent fluids, bleeding, transfusion, pregnancy, infection, surgery and medicines can alter several values simultaneously, so the request time and clinical state belong beside the numerical report.
Red-cell reasoning should separate size from marrow response. Microcytosis directs attention towards iron restriction and haemoglobinopathy, whereas macrocytosis can reflect B12 or folate deficiency, alcohol, liver disease, hypothyroidism, reticulocytosis, medicines or marrow dysplasia. Normocytic anaemia is not diagnostically neutral: early deficiency, renal disease, inflammation, haemolysis and acute blood loss remain possible. Reticulocytes show whether marrow output is appropriate for the degree of anaemia, but recent transfusion and deficient iron, B12 or folate can blur the expected response.
White-cell and platelet abnormalities require the same discipline. Neutrophilia may be reactive to infection, inflammation, corticosteroids or stress, but left shift, basophilia, splenomegaly or persistence can suggest a myeloid neoplasm. Lymphocytosis may be reactive or clonal; morphology, duration, age and symptoms determine the next test. Thrombocytopenia may be artefactual, immune, consumptive, drug-related, infective, hepatic or marrow based. Thrombocytosis is often reactive to inflammation or iron deficiency, yet persistent unexplained elevation warrants a clonal assessment. The safe output is a problem representation and next action, not a list of every possible cause.

- Read haemoglobin, white-cell differential and platelets together before focusing on a flagged value; abnormalities across several lineages suggest marrow, infiltrative, consumptive or systemic disease rather than an isolated deficiency.
- Compare with the patient's baseline and the laboratory's age-, sex- and pregnancy-appropriate reference interval. Direction and speed of change often carry more clinical information than a single result just outside range.
- For anaemia, begin with mean cell volume, mean cell haemoglobin, red-cell distribution width and reticulocytes, then integrate film, iron studies, B12, folate, renal, inflammatory and haemolysis testing as the phenotype directs.
- Use absolute neutrophil, lymphocyte, eosinophil, basophil and monocyte counts rather than percentages. A normal total white count can conceal a clinically important differential-count abnormality.
- Confirm unexpected thrombocytopenia on a fresh citrate or otherwise laboratory-advised sample and film when platelet clumping is possible, while examining for bleeding, sepsis, drugs and thrombotic microangiopathy.
- A machine flag is a request for review, not a diagnosis. Blasts, nucleated red cells, schistocytes, atypical lymphocytes and platelet clumps need skilled film correlation and clinical context.
- Do not use the full blood count as a cancer rule-out test. Persistent lymphadenopathy, splenomegaly, weight loss, unexplained bleeding or recurrent infection can require investigation despite initially normal counts.
- Close the loop on critical laboratory calls: verify identity and sampling, assess the patient, compare prior results, repeat only when useful, and record who will act on film, culture, haematinic or referral results.
02Indications, selection and cautionsWhen it is useful, when urgency changes and important limitations.
Low haemoglobin with preserved white cells and platelets supports an erythroid, nutritional, renal, inflammatory, bleeding or haemolytic process. Symptoms and haemodynamic state determine urgency before morphology defines cause.
Anaemia plus neutropenia and thrombocytopenia raises marrow failure, infiltration, acute leukaemia, severe deficiency, sepsis, hypersplenism or treatment toxicity. Fever, bleeding or blasts makes the pattern urgent.
Persistent leucocytosis, basophilia, thrombocytosis, circulating immature myeloid cells or splenomegaly suggests a myeloproliferative or leukaemic process after reactive causes are assessed.
Platelet clumps, an implausible abrupt haemoglobin change, cold-agglutinin analyser flags or a result incompatible with the patient should prompt laboratory discussion and a correctly handled repeat sample.
Blasts, promyelocytes with coagulopathy, schistocytes with thrombocytopenia, profound neutropenia with fever, or active bleeding with severe thrombocytopenia requires same-hour clinical and haematology escalation.

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

- Use a fresh sample when pre-analytical error is plausible, but first assess an unwell patient. Stable chronicity narrows the differential; a rapid fall or rise suggests bleeding, haemolysis, consumption, acute inflammation, drug effect or evolving marrow disease.

- Blasts or abnormal promyelocytes trigger urgent acute-leukaemia pathways; schistocytes suggest mechanical fragmentation or thrombotic microangiopathy; spherocytes, target cells, teardrops, rouleaux and toxic change require cause-specific correlation rather than diagnosis from one cell.

Reticulocyte count and haemolysis profile - Why
- Determine whether marrow production is appropriate and whether red-cell destruction is occurring.
- Interpretation and limitations
- An appropriately raised absolute reticulocyte count supports blood loss or haemolysis, while a low response suggests impaired production. Interpret bilirubin, LDH, haptoglobin and direct antiglobulin test together and account for recent transfusion or deficiency.

- Low ferritin supports depleted iron stores, but inflammation can raise ferritin and conceal iron restriction. B12 results need NICE's indeterminate and functional-testing pathway; folate should not be replaced in isolation before B12 has been considered.

- Renal impairment, liver disease, hypothyroidism and inflammation can each alter indices. Their presence does not automatically explain a severe or progressive abnormality, and mixed aetiologies are common.

- Select the test with haematology and the laboratory: flow may classify a lymphoid clone, molecular tests can support a myeloid neoplasm, and marrow morphology supplies architecture, lineage and fibrosis information.

- Trend each lineage against dates, clinical events, transfusions and treatment rather than copying isolated flags into the problem list.
- For anaemia, monitor symptoms, haemoglobin, reticulocyte response and the cause-specific marker; failure to respond should trigger adherence, absorption, bleeding, inflammation and diagnostic review.
- For neutropenia, provide explicit fever and infection safety-netting and increase monitoring after chemotherapy, marrow-toxic medicines or a falling serial count.
- For thrombocytopenia, document bleeding, platelet trajectory, anticoagulant or antiplatelet exposure and any planned procedure instead of using the count alone.
- Assign a named clinician to pending film comments, flow cytometry, molecular tests and referrals, and communicate a critical revised result directly.

Reference ranges are statistical
A result just outside range can occur in health, whereas a technically normal result may be abnormal for that patient's baseline. Magnitude, trajectory and phenotype determine meaning.
Percentages conceal absolute counts
A high lymphocyte percentage during neutropenia may coexist with a normal absolute lymphocyte count. Clinical decisions use the relevant absolute population.
Normal MCV can hide two processes
Iron deficiency with B12 deficiency, or microcytosis with reticulocytosis, may average to a normocytic index. RDW and film can expose the mixed population.
Platelets are vulnerable to artefact
EDTA-dependent clumping can produce a false low count and an unnecessary bleeding pathway. Film review and a laboratory-advised alternative tube clarify the result.
A normal count is not reassurance alone
Early marrow or lymphoid malignancy can present with symptoms and normal circulating counts. Examination and symptom-led imaging or tissue remain important.
