A platelet count is one observation in a dynamic system. Automated analysers can undercount when platelets clump, are unusually large or adhere around leucocytes. Confirm a surprising result before labelling disease, but do not let repeat sampling delay treatment of actual bleeding or a thrombotic microangiopathy. Compare every available historical count: an abrupt fall from 300 to 100 × 10^9/L may be more mechanistically important than a stable count of 90.
Mechanisms overlap. Reduced marrow production often affects other lineages and may show macrocytosis, blasts or dysplasia. Immune destruction can be isolated with large young platelets. Consumption from sepsis, DIC, TTP, HUS or major bleeding produces systemic findings and sometimes fragments. Splenic sequestration accompanies portal hypertension and splenomegaly. Dilution follows massive transfusion or surgery. Medicines can act through immune antibodies, marrow toxicity, thrombotic microangiopathy or heparin-PF4 immune activation, so an exact exposure timeline matters.
Investigation is tiered. Every patient gets clinical assessment, repeat count where needed and film. Coagulation, fibrinogen, haemolysis, renal and liver tests answer urgent mechanistic questions. Pregnancy testing and blood pressure identify obstetric microangiopathy. HIV and hepatitis testing, B12 and folate, spleen or liver imaging and marrow examination are selected from phenotype. Bone-marrow biopsy is not routine in a typical isolated ITP pattern but is appropriate with other cytopenias, abnormal film, systemic features or unexplained persistence.
Key points
- Confirm that thrombocytopenia is real: EDTA-dependent platelet clumping, difficult sampling and platelet satellitism can produce a spuriously low automated count.
- Repeat the count in a citrate or other validated alternative tube and inspect a blood film when clumping or an unexpected isolated result is reported.
- First determine urgency from bleeding, physiology, neurological or renal features, pregnancy, sepsis, thrombosis and the direction and speed of count change.
- The blood film can reveal schistocytes, blasts, dysplasia, giant platelets, malaria, clumping and a leucoerythroblastic pattern and should be reviewed early.
- Isolated thrombocytopenia with otherwise normal film and no systemic illness suggests ITP or a drug effect after HIV, hepatitis and other secondary causes are considered.
- Anaemia with schistocytes, raised LDH and low haptoglobin indicates microangiopathic haemolysis and activates TTP, HUS, DIC, malignant hypertension or pregnancy pathways.
- Prolonged PT and APTT, low fibrinogen and high D-dimer support consumptive DIC in the right clinical setting but normal early coagulation does not exclude evolution.
- A fall beginning five to ten days after heparin with new thrombosis is assessed using the 4Ts score before PF4 laboratory testing and alternative anticoagulation.
- Liver disease and splenomegaly cause sequestration, while alcohol, medicines, B12 or folate deficiency, marrow failure and malignancy reduce platelet production.
- Do not transfuse platelets simply because the count is low; bleeding, procedure, mechanism and diagnosis determine benefit, and platelets can be harmful in TTP or HIT.
02Indications, selection and cautionsWhen it is useful, when urgency changes and important limitations.
The analyser flags clumps, the film shows platelet aggregates and an alternative-anticoagulant sample produces a more credible count without bleeding features.
Petechiae, purpura, epistaxis, gum bleeding and menorrhagia fit platelet-type haemostasis failure more than isolated deep-muscle or joint bleeding.
Schistocytes, anaemia, high LDH, low haptoglobin and organ dysfunction indicate red-cell fragmentation with platelet consumption.
Thrombocytopenia plus microangiopathic haemolysis and neurological, renal or abdominal features requires immediate plasma-exchange service contact.
Blasts, dysplasia, pancytopenia, bone pain or a leucoerythroblastic film raises malignant infiltration, myelodysplasia or marrow failure.

- Trend coagulation, fibrinogen, haemolysis and organ function during sepsis, DIC, pregnancy microangiopathy or other consumptive illness.
- After a suspected medicine cause, document withdrawal, count recovery and future avoidance in prescribing and patient-held records.
- For chronic stable thrombocytopenia revisit alcohol, liver, spleen, infection, marrow and autoimmune features when the count pattern changes.
- After platelet transfusion record indication, dose, increment and bleeding response and investigate repeated poor recovery rather than giving automatic extra pools.

Trajectory carries mechanism
A rapid relative fall after heparin or sepsis can be dangerous even when the absolute count remains above conventional bleeding thresholds.
Film is a branching test
Clumps, fragments, blasts and giant platelets send the same numerical result into entirely different diagnostic and treatment pathways.
Normal coagulation narrows
TTP usually preserves routine clotting times, whereas DIC often consumes coagulation factors, although early results require serial context.
Platelets are not neutral
A component can fuel pathological thrombosis or be immediately destroyed, so diagnosis and bleeding matter more than cosmetic count correction.
