Warfarin monitoring and reversal

Start and monitor warfarin safely, interpret INR in clinical context, manage interruptions and reverse excessive anticoagulation according to bleeding severity and urgency.

Major warfarin-associated haemorrhage

Intracranial, retroperitoneal, pericardial, airway or ongoing haemodynamic bleeding requires immediate reversal even before the laboratory INR returns when warfarin exposure is known.

Action: Stop warfarin, resuscitate and control the source, send urgent INR, FBC, fibrinogen, renal, liver and group samples, give four-factor prothrombin complex concentrate with 5–10 mg slow intravenous phytomenadione under the major-haemorrhage protocol and recheck clinical haemostasis and INR.

01Purpose and principlesWhat the treatment does and how it fits into care.

Warfarin has a narrow therapeutic index, multiple interactions and a delayed pharmacodynamic response, but remains essential for mechanical valves and useful when DOACs are unsuitable. Its effect reflects turnover of vitamin-K-dependent factors rather than the plasma drug concentration. Factor VII falls early, so an initial INR rise does not mean full antithrombotic effect; factor II takes longer. In acute VTE, overlap a therapeutic parenteral anticoagulant for at least 5 days and until the INR is adequately therapeutic. Initiation schedules should be locally validated and reduced or supervised closely in older, frail, malnourished, liver-impaired or heart-failure patients.

The target must be stated, not assumed. Many AF and VTE indications use target 2.5, range 2.0–3.0. Mechanical valves use valve-specific targets based on thrombogenicity, mitral versus aortic position and embolic risk; obtain the operation card or cardiology plan. Time in therapeutic range measures control over time, but any acute high INR must be acted on now. Investigate a change before repeatedly altering dose: adherence, duplicate tablets, antibiotics, amiodarone, antifungals, anticonvulsants, alcohol, reduced intake, diarrhoea, fever, liver dysfunction and heart failure are common causes.

Reversal is determined by bleeding, site and urgency, not INR alone. Major or life-threatening bleeding needs immediate four-factor PCC for rapid factor replacement and IV phytomenadione for sustained correction. A product such as Beriplex is dosed by pretreatment INR and body weight up to 100 kg: 25 IU/kg for INR 2.0–3.9, 35 IU/kg for 4.0–6.0 and 50 IU/kg above 6.0, with maxima 2500, 3500 and 5000 IU. Follow the stocked product's protocol. Recheck INR and haemostasis; repeated PCC is not routine because it increases thrombotic exposure and may not correct an uncontrolled source.

For excessive INR without bleeding, avoid the harm of over-reversal. At INR 5–8, omit warfarin, identify the cause and restart at a lower dose when in range; give low-dose vitamin K only when bleeding risk warrants. Above INR 8, omit warfarin and give oral phytomenadione, commonly 1–5 mg, then repeat INR within the local timeframe. Minor bleeding needs local haemostasis plus an INR-led pathway; clinically important bleeding may justify vitamin K even below the major threshold. Vitamin K route and dose matter: IV acts faster but must be infused slowly because severe reactions can occur, while large doses can cause prolonged warfarin resistance.

Interruption requires thrombotic as well as procedural assessment. Many low-bleeding-risk procedures can continue warfarin within a defined INR; others require timed cessation, commonly 5 days, and pre-procedure INR confirmation. Bridging is not routine for AF because it increases bleeding. It is reserved for selected high-risk mechanical valves or very recent or severe thrombosis under a written plan. Restart after procedural haemostasis, considering whether post-operative therapeutic dosing is immediately safe. Never improvise bridging solely because an INR has become subtherapeutic.

Key points

  • PCC dosing is product and INR based: for Beriplex, 25, 35 or 50 IU factor IX/kg for INR 2–3.9, 4–6 or above 6 respectively, with stated maximum doses.
  • Bridge acute VTE with therapeutic heparin for at least 5 days and until the INR has been therapeutic for the protocol-defined period because early protein C depletion is not immediate protection.
  • Provide an anticoagulant record and consistent advice on missed doses, vitamin-K-containing food, alcohol, interacting medicines, pregnancy and when to seek urgent help.

Bleeding severity

Classify critical-site, haemodynamic or ongoing major bleeding separately from minor bleeding and an isolated high INR because reversal intensity differs.

INR trajectory

Review current and prior results, tablet strengths and recent dose changes; a rising INR after a held dose may reflect delayed interaction or illness.

Interaction pattern

Antibiotics, azole antifungals, amiodarone, anticonvulsants, NSAIDs, antiplatelets, alcohol and reduced nutrition change anticoagulant effect or bleeding independent of INR.

Thrombotic indication

Mechanical mitral valves, recent VTE and selected severe thrombophilia carry different interruption risk from stable AF without recent embolism.

Occult toxicity

Fatigue, dyspnoea, melaena, haematuria, headache, abdominal or back pain may reveal significant bleeding before hypotension develops.

Red flags requiring action

  • New neurological deficit, severe headache, collapse or head injury while taking warfarin requires emergency intracranial imaging and reversal assessment irrespective of an apparently modest INR.
  • Haemodynamic instability, ongoing gastrointestinal bleeding, haemoptysis, haematuria with obstruction or a rapid haemoglobin fall requires major-haemorrhage escalation and source control.
  • An INR above 8, interacting antibiotic, acute liver or renal failure, inability to take nutrition or repeated dosing error predicts further INR rise even when bleeding is not yet visible.

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

Warfarin

Use a locally validated oral loading protocol and adjust the total weekly dose to the stated target INR; acute VTE additionally requires therapeutic heparin for at least 5 days and until INR is therapeutic for the locally defined interval.

Check interacting medicines, liver function, nutrition, alcohol and adherence. Provide an anticoagulant record and close INR monitoring after change. It crosses the placenta and can cause embryopathy and fetal bleeding; pregnancy and conception require specialist planning.

Restores hepatic production of functional vitamin-K-dependent factors and sustains correction after short-lived infused factor replacement.

Phytomenadione

For major warfarin bleeding give 5–10 mg by slow intravenous injection with four-factor PCC; for INR above 8 without bleeding give 1–5 mg orally, then recheck and repeat only according to response and protocol.

IV administration must be slow because severe anaphylactoid reactions have occurred. Oral treatment is slower and unsuitable as sole emergency therapy. Avoid unnecessarily large doses because prolonged warfarin resistance can complicate re-anticoagulation.

Rapidly replaces factors II, VII, IX and X for life-threatening or major vitamin-K-antagonist bleeding or urgent reversal when immediate haemostasis is required.

Four-factor prothrombin complex concentrate

For Beriplex major warfarin bleeding give 25 IU factor IX/kg when INR 2.0–3.9, 35 IU/kg when INR 4.0–6.0 or 50 IU/kg when INR exceeds 6.0; maxima are 2500, 3500 and 5000 IU and weight is capped at 100 kg.

Give with vitamin K and follow the exact stocked product protocol. PCC can cause arterial or venous thrombosis and DIC; balance this against haemorrhage, monitor response and avoid routine repeat dosing without senior haematology input.

06Complications, monitoring and follow-upAdverse effects, response and longer-term review.
  • During initiation, measure INR at the frequency specified by the loading protocol and do not infer stable maintenance need from the first response.
  • Once stable, extend testing through the anticoagulation service schedule, but shorten immediately after interacting drugs, acute illness, intake change, missed doses, bleeding or a dose adjustment.
  • Track time in therapeutic range and investigate persistent values below 65% rather than accepting oscillation as inevitable; assess cognition, access, adherence and suitability of the indication.
  • After PCC and vitamin K, repeat INR and clinical haemostasis at protocol-defined times, monitor for rebleeding and thrombosis and ensure the underlying source is controlled.
  • At each review verify the current target, valve or thrombotic indication, duration, concurrent antiplatelet need, pregnancy plans and peri-procedural instructions.

INR is not severity

A patient can have fatal intracranial bleeding at a modest INR or remain asymptomatic at a very high INR; anatomy and physiology determine emergency action.

Vitamin K sustains PCC

PCC supplies functioning factors immediately but they decay. Intravenous phytomenadione enables endogenous replacement and reduces rebound anticoagulation.

Interaction effects lag

Because factor half-lives differ, an INR may continue rising after warfarin is withheld; the interacting medicine and reduced intake can outlast the dose change.

Bridge selectively

Peri-procedural LMWH increases bleeding and is not a badge of adequate care. Reserve it for clearly high thrombotic risk under a timed written plan.

Tablets create traps

Multiple strengths and alternating doses cause error. Confirm colour, strength and schedule directly and simplify the weekly regimen whenever control permits.

08Common pitfallsFrequent interpretation and management errors.
  1. 01

    Do not wait for an INR before reversing known warfarin exposure with life-threatening bleeding; draw blood, then treat while source control proceeds.

  2. 02

    Do not give fresh frozen plasma routinely when four-factor PCC is available; plasma is slower, larger volume and less reliable for rapid vitamin-K-factor replacement.

  3. 03

    Do not give PCC without vitamin K for warfarin bleeding, because correction may disappear as the infused factors are cleared.

  4. 04

    Do not give 10 mg vitamin K for every high INR without bleeding; excessive reversal creates prolonged resistance and avoidable thrombotic-management difficulty.

    Do not assume a subtherapeutic INR always requires therapeutic LMWH; establish indication, duration below range and actual thrombotic risk first.
