A penicillin-allergy label is a clinical history that may or may not represent persistent immune hypersensitivity. Many labels originate from childhood viral rashes, predictable adverse effects or remote events that were never characterised. Conversely, severe immediate and delayed reactions can recur with major harm. The assessment therefore seeks a phenotype and risk category, not a binary answer from memory alone.
Timing is crucial. Immediate reactions generally occur soon after exposure and feature urticaria, angio-oedema, bronchospasm or cardiovascular compromise. Delayed maculopapular eruptions differ from severe cutaneous adverse reactions involving mucosa, blistering, systemic symptoms, eosinophilia or organ injury. The exact drug and any later tolerated penicillin or cephalosporin materially change probability.
Testing should answer a prescribing need under a governed protocol. Low-risk histories may be suitable for direct oral challenge in specialist-led services; other immediate phenotypes may need skin testing followed by challenge. Severe delayed syndromes are generally not challenged. A negative supervised evaluation must be translated into visible record changes and patient explanation or the harmful label will return.
Key points
- Ask which drug was taken, why, route, timing after the dose, symptoms, treatment, hospitalisation, time since reaction and subsequent beta-lactam tolerance.
- Nausea, diarrhoea, headache, family history or an unknown childhood label is not equivalent to IgE-mediated allergy, although uncertainty still needs safe documentation.
- Immediate urticaria, angio-oedema, bronchospasm or hypotension suggests an immediate hypersensitivity phenotype; severe blistering, mucosal or organ injury indicates a high-risk delayed reaction.
- Do not perform an unsupervised test dose. Skin testing and direct oral challenge require a risk-stratified specialist or locally governed pathway with rescue capability.
- Document the specific culprit and phenotype rather than 'penicillin allergy', and update both prescribing and primary-care records after specialist testing.
- De-labelling can restore narrower first-line treatment and reduce broad-spectrum exposure, but patient safety and shared understanding take priority over label removal targets.
02Indications, selection and cautionsWhen it is useful, when urgency changes and important limitations.
Isolated gastrointestinal upset, headache, family history, an unknown remote label or a non-urticarial rash without systemic features may represent intolerance or low probability.
Rapid urticaria, angio-oedema, wheeze, throat symptoms, hypotension or collapse after a beta-lactam suggests IgE-mediated or other immediate allergy.
Mucosal involvement, blistering, skin pain, facial oedema, eosinophilia, hepatitis, nephritis or systemic illness suggests a severe cutaneous or organ reaction.
Documented subsequent therapeutic exposure to the same penicillin without reaction substantially weakens the original label, provided drug identity and course are reliable.

- Audit whether a removed label reappears and whether alternative broad-spectrum antibiotics continue despite documented tolerance.
- For a current serious reaction, trend airway, circulation, skin and mucosal progression, eosinophils, liver and kidney function and report through MHRA systems.

The drug name matters
A reaction to co-amoxiclav could relate to amoxicillin, clavulanate or an unrelated illness; the label should not automatically expand to every antimicrobial.
Cross-reactivity is side-chain dependent
Cephalosporin risk is influenced more by related side chains and phenotype than by the beta-lactam ring alone, requiring medicine-specific assessment.
Negative testing needs communication
A technically successful challenge has little benefit if electronic and primary-care records still block penicillin or the patient believes the allergy persists.
Severe delayed reactions are different
Reassuring statistics about remote childhood rashes must never be applied to SJS/TEN, DRESS or organ-specific immune injury.
