Cancer of unknown primary

Recognise metastatic malignancy without an identified origin, stabilise urgent complications, perform a focused high-yield diagnostic work-up, and avoid burdensome testing that cannot change treatment or goals.

Time-critical presentation

Treat spinal cord compression, airway or superior vena cava obstruction, bowel obstruction, severe bleeding, sepsis, pathological fracture, symptomatic brain metastasis and severe hypercalcaemia immediately. The absence of a known primary must not delay physiological rescue, corticosteroid or radiotherapy discussion where indicated, or urgent surgical and acute-oncology involvement.

Cancer of unknown primary is a heterogeneous syndrome rather than one biological disease. A metastatic deposit is identified, but the site of origin remains occult after a defined assessment. Some primaries are small, regressed or difficult to visualise; others lose organ-specific differentiation. The immediate questions are whether the tissue is truly malignant, which broad lineage it represents, whether a treatable site or favourable subset is present and whether further testing will change management.

NICE recommends a focused initial evaluation: comprehensive history and examination, full blood count and biochemistry, urinalysis, symptom-directed tests, and CT chest, abdomen and pelvis, with biopsy and immunohistochemistry. Additional endoscopy, breast imaging, PET-CT, germ-cell markers, CA125 or PSA is used selectively. A marker elevation alone rarely establishes origin because many rise in several cancers and benign illness.

Management depends on pathology, molecular findings with validated relevance, distribution, performance status, symptoms and preferences. Local therapy can help solitary or favourable-pattern disease; empiric systemic therapy may be considered for fit patients without a site-specific route. Early palliative care treats pain, breathlessness, nausea, psychological distress and planning needs alongside any tumour-directed option.

Occult small primary

The primary tumour may remain below imaging resolution while biologically capable metastatic clones establish clinically visible disease elsewhere.

Regressed or anatomically hidden origin

Some primaries involute, arise in difficult-to-image mucosal sites or are obscured by metastatic burden and treatment-related change.

Loss of differentiation

Advanced tumour evolution can reduce organ-specific morphology and marker expression, making the original tissue lineage difficult to identify confidently.

Metastatic adenocarcinoma

Adenocarcinoma in liver, lung, bone, peritoneum or nodes without an evident primary is common but still requires clinicopathological correlation.

Favourable nodal pattern

Axillary adenocarcinoma in a woman, cervical squamous carcinoma or inguinal squamous disease may follow a site-specific pathway despite occult primary.

Peritoneal pattern

Serous papillary peritoneal disease in a woman can resemble ovarian cancer and requires gynaecological pathology and MDT assessment.

Midline germ-cell pattern

Young age, midline mass and compatible marker pattern can indicate an extragonadal germ-cell tumour with substantially different curative treatment.

Acute metastatic complicationRed flag

Neurological deficit, obstruction, severe pain, pathological fracture or metabolic deterioration requires emergency treatment before the primary is found.

Lymphoma

Nodal, marrow or extranodal lymphoma can mimic metastatic carcinoma but requires haematological immunophenotyping and a completely different treatment pathway.

Metastatic melanoma

Amelanotic melanoma may resemble undifferentiated carcinoma; skin examination and melanocytic markers establish lineage when morphology is unclear.

Germ-cell tumour

A young person with midline disease and compatible markers may have a highly treatable germ-cell malignancy rather than unfavourable CUP.

Multiple primary cancers

More than one tumour can coexist, so discordant morphology or distribution should not be forced into a single occult-primary explanation.

Metastatic organ failure

Progressive liver, lung, marrow, brain or skeletal involvement can cause jaundice, breathlessness, cytopenias, neurological loss, fracture and severe pain.

Diagnostic burden

Repeated non-directed scans, endoscopies and biopsies create procedural harm, incidental findings and delay without necessarily improving treatment selection.

Psychological uncertainty

Not knowing the cancer's origin can intensify distress and complicate prognostic discussion, requiring clear communication and consistent clinical ownership.

Treatment toxicity without benefit

Empiric therapy can harm when lineage, organ reserve, performance status or realistic goals have not been defined carefully.

  • Maintain one CUP results timeline covering pathology, immunohistochemistry, molecular addenda, imaging and MDT decisions to prevent repeated low-yield testing.
  • Reassess pain, neurological function, nutrition, performance status and organ complications at every contact because clinical need may change faster than diagnostic certainty.
  • During anticancer treatment, monitor regimen-specific blood counts, renal, hepatic and toxicity parameters and provide 24-hour acute-oncology contact.
  • Review whether further tests remain decision-changing after each result, stopping investigations that cannot improve treatment or symptom planning.
  • Document preferred place of care, escalation decisions and family communication while preserving the option to revise goals as disease evolves.

Unknown is not uninvestigated

CUP is assigned after an appropriate focused assessment, not simply because the first scan fails to show a primary.

Lineage comes first

Distinguishing carcinoma, lymphoma, melanoma, sarcoma and germ-cell tumour changes the entire pathway before organ origin is debated.

Markers create noise

Non-specific tumour-marker elevations are common in metastatic and benign disease and can generate misleading site labels.

Favourable subsets matter

A small minority has biology or distribution that supports site-specific and sometimes curative-intent treatment.

Stopping tests is active care

When no result can alter treatment or goals, avoiding further invasive investigation reduces harm and creates time for symptom control.
