Lymphoedema follows loss or dysfunction of lymphatic transport. Axillary clearance confers greater risk than sentinel biopsy, while nodal radiotherapy, obesity, infection and extensive surgery add injury. Early fluid is soft and may pit or fluctuate. Persistent protein triggers adipose deposition and fibrosis, making late swelling firm and less reversible. Breast and chest-wall oedema may show peau d’orange and heaviness without a dramatic arm-volume difference.
A swollen arm after breast cancer needs diagnostic discipline. Sudden onset, cyanosis, catheter dysfunction or collaterals suggests venous thrombosis; fever and erythema suggests cellulitis; hard nodes, progressive plexus pain or rapid refractory change suggests recurrence. Duplex ultrasound is first-line for accessible venous thrombosis, while CT or MRI evaluates central obstruction and tumour. Lymphoscintigraphy or indocyanine-green mapping answers selected uncertain or surgical-planning questions but is not required for a classic clinical case.
Treatment aims to move fluid through remaining pathways, limit refill and prevent inflammation. Compression provides an external pressure gradient, exercise activates muscle pumps, skin care reduces microbial entry and weight management lowers load. A specialist programme may begin with multilayer bandaging and manual drainage, then transition to a fitted sleeve and self-management. Axillary web syndrome and shoulder restriction require physiotherapy because reduced movement worsens both function and lymph flow.
Survivorship review is broader than lymphoedema. Surgery can cause seroma, chronic post-mastectomy pain and reconstruction complications. Radiotherapy can cause dermatitis, breast oedema, fibrosis, pneumonitis and later cardiac or plexus injury. Chemotherapy and targeted therapy can cause neutropenia, neuropathy, cardiomyopathy and pneumonitis. Endocrine treatment affects bone, joints, thrombosis, uterus and sexual health. Each new symptom needs a toxicity, recurrence and common-disease differential rather than automatic attribution to treatment.

- Investigate new or rapidly progressive swelling, hard nodes or neuropathic pain for recurrence even when lymphoedema has been stable for years.
- Review cardiac, pulmonary, bone, menopausal, neuropathic, cognitive and sexual toxicity systematically rather than treating survivorship as cancer-free only.
- Selected refractory disease may benefit from lymphovenous bypass, vascularised node transfer or liposuction after expert imaging and optimisation, but compression often remains necessary.

Surgery, radiotherapy, cytotoxic, endocrine, HER2-directed, immune and bone-modifying therapies create acute and late tissue, nerve, marrow, cardiac, lung, bone and reproductive toxicity.
Recurrent infection and inflammation
Skin breaks and cellulitis increase capillary leak and lymphatic injury, while protein-rich stagnant fluid impairs local immune trafficking and makes future infection more likely.
Cancer recurrence or venous obstruction
Axillary or supraclavicular tumour, central venous thrombosis and indwelling catheters can block drainage and may cause or worsen an apparently treatment-related swollen arm.

For a stable adult with non-purulent cellulitis and no immediate penicillin allergy, a common lymphoedema regimen is flucloxacillin 500 mg orally four times daily for 14 days, adjusted to severity, culture and local guidance.Hospitalise and use intravenous therapy for systemic toxicity, rapid progression, neutropenia or oral failure; review at 48 hours, account for allergy and hepatic risk and do not confuse recurrence with infection.
Phenoxymethylpenicillin prophylaxis
After recurrent proven cellulitis, a specialist protocol may use phenoxymethylpenicillin 250 mg orally twice daily, often 500 mg twice daily when BMI is 33 kg/m² or higher, with duration reviewed after at least 12 months.Confirm that episodes were cellulitis, review penicillin allergy, adherence and resistance and reassess need rather than prescribing indefinitely without lymphoedema optimisation.
Neuropathic pain treatment

Chronic pain and sensory disturbance
Intercostobrachial nerve injury, scar, cording, radiation fibrosis and plexus or recurrence-related neuropathy can produce persistent mixed pain.
Psychological and social burden
Visible asymmetry, garment use, fear of infection and recurrence and loss of work or clothing choices can undermine body image and participation.
Rare lymphangiosarcoma
A new bruised, purple or nodular lesion in longstanding lymphoedema can represent Stewart–Treves angiosarcoma and requires urgent biopsy.

Symptoms can precede volume change
Heaviness and tightness may reflect early lymphatic overload before a conventional percentage difference, offering a window for reversible intervention.
Compression is not one garment
Sleeve, glove, bra, truncal and night systems address different distributions and must be fitted and revised as tissue changes.
Exercise is treatment, not a prohibition
Gradually progressed resistance and aerobic activity improves function and does not increase lymphoedema when introduced with monitoring.
Diuretics remove the wrong compartment
They reduce intravascular sodium and water but not protein-rich lymphatic material, so routine use can dehydrate without correcting the mechanism.
