Abnormal reflex with a new squint
A child with a new-onset squint and loss of the fundal reflex needs immediate ophthalmology referral because serious intraocular disease is possible.
Action: Contact ophthalmology immediately. An absent or white reflex without squint also requires prompt specialist referral; do not wait for the next routine developmental check.
Synopsis
Detect an abnormal infant fundal reflex, arrange time-critical specialist assessment and explain the prolonged visual rehabilitation required after childhood cataract treatment.
Congenital lens opacity can block the patterned visual input required for normal brain development.
Assess both fundal reflexes in suitable conditions and compare brightness, colour and any central shadow.
Normal reflex colour varies with pigmentation; unexplained asymmetry or a white reflex remains concerning.
Key red flags
A persistent white pupil or white reflex in photographs must not be dismissed as a normal camera effect.
Investigation priorities
01
Specialist dilated ocular examinationFirst step
Confirm the location and significance of the visual obstruction.
Management branches
ScreenAct on an abnormal infant reflex
Newborn or infant examination shows an absent, white or asymmetric fundal reflex.
Record which eye is affected and the exact finding, explain the concern to the parents and arrange a tracked specialist referral.
For a positive newborn NIPE eye screen, arrange review within two weeks of the screening examination; discuss significant concerns with ophthalmology before discharge.
National guidance is shown before implementation-dependent detail. Typical adult dose examples remain subject to patient factors, contraindications and the live BNF or specialist protocol. Source check completed 7 Sept 2026; clinical approval remains outstanding.
