Abnormal reflex with a new squint

A child with a new-onset squint and loss of the fundal reflex needs immediate ophthalmology referral because serious intraocular disease is possible.

Action: Contact ophthalmology immediately. An absent or white reflex without squint also requires prompt specialist referral; do not wait for the next routine developmental check.

Synopsis

Detect an abnormal infant fundal reflex, arrange time-critical specialist assessment and explain the prolonged visual rehabilitation required after childhood cataract treatment.

  • Congenital lens opacity can block the patterned visual input required for normal brain development.
  • Assess both fundal reflexes in suitable conditions and compare brightness, colour and any central shadow.
  • Normal reflex colour varies with pigmentation; unexplained asymmetry or a white reflex remains concerning.

Key red flags

A persistent white pupil or white reflex in photographs must not be dismissed as a normal camera effect.
