Contact-lens-associated infection

Recognise infection in a contact-lens wearer, arrange urgent corneal assessment, preserve useful microbiology material, and reduce recurrence through a specific lens-care and return-to-wear plan.

Contact-lens-associated microbial keratitis can initially resemble conjunctivitis or a sterile infiltrate and can progress to serious visual damage.

Action: Remove the lenses and arrange urgent same-day ophthalmic assessment for pain, photophobia, visual change, a corneal opacity or suspected infection. Retain the lenses and case for possible culture. Do not restart wear or use a steroid-containing preparation while infection remains unassessed.

Contact lenses change the interaction between the corneal surface, tear film and microorganisms. Problems may arise from epithelial stress, contamination of lenses or cases, overnight wear and exposure to water. The history needs enough detail to identify a real practice rather than a label such as good hygiene. Ask how lenses are obtained, how long they are worn, whether the patient ever sleeps or naps in them, how hands are dried, and exactly how reusable lenses and storage cases are cleaned. Include decorative lenses even when no refractive correction is needed.

The urgent task is to distinguish a potentially infected cornea from less dangerous lens-related irritation. Microbial keratitis may present with redness, pain, watering, discharge, photophobia and visual loss. A sterile contact-lens-associated infiltrative event can produce a small peripheral opacity and relatively mild discomfort, sometimes improving after lens cessation. However, these appearances are not sufficiently specific to let an untrained assessment exclude infection. Measure acuity and inspect the cornea, then use the appropriate urgent eye service when symptoms, an opacity or uncertainty are present.

Acanthamoeba deserves specific consideration after water exposure and in an atypical or persistent course. Severe pain can occur later, but early symptoms may be comparatively mild; waiting for pain out of proportion to examination can delay diagnosis. The specialist may request the actual lenses and case for microbiology, so advise retaining these safely while stopping wear. Treatment for a bacterial ulcer and treatment for amoebic infection differ substantially. Repeated empirical changes to ordinary antibiotic drops cannot substitute for reassessment when the course is unexpected.

Contamination and wearing behaviour

Microorganisms can reach the cornea through contaminated lenses or cases. Overnight wear, inadequate cleaning and poor replacement habits add avoidable stress or microbial exposure.

Water-associated organisms

Acanthamoeba can be associated with exposure of lenses or their equipment to water. A detailed history of showering, swimming and rinsing practices is clinically useful.

Inflammatory lens reactions

Some lens-associated infiltrates reflect an immune response to bacterial material rather than an established stromal infection. Their clinical resemblance to infection makes appropriate assessment essential.

Lens-associated bacterial keratitis

An acutely red painful eye with a focal corneal opacity, epithelial defect or reduced acuity suggests infection, with Pseudomonas among the important contact-lens-associated pathogens.

Possible amoebic disease

Water exposure, a prolonged or atypical course and pain that becomes severe should raise concern for Acanthamoeba, without requiring every classical feature to be present.

Sterile infiltrative event

A small peripheral lesion with mild discomfort can reflect an inflammatory response to bacterial antigens, but specialist assessment must distinguish this from microbial keratitis.

Practical barriers to safe treatment

Limited vision in the other eye, inability to instil frequent drops, inadequate support or difficulty attending review may make an otherwise plausible outpatient plan unsafe.

Red flags requiring action

  • Pain, light sensitivity, reduced vision or a white corneal spot in a lens wearer requires same-day specialist assessment.
  • A history of swimming, showering or rinsing lenses with tap water raises concern for water-associated organisms including Acanthamoeba.
  • Mild early discomfort does not exclude Acanthamoeba or another significant corneal infection.

Sterile infiltrative keratitis

A peripheral infiltrate with limited symptoms may resolve after stopping lenses, but this diagnosis requires differentiation from infection. The clinical course must still be reviewed.

Mechanical epithelial injury

A damaged or poorly fitting lens can abrade the surface and cause pain. Secondary infection remains a concern when an opacity develops or symptoms progress.

Toxic or allergic reaction

Lens solutions or deposits can irritate the ocular surface and produce redness. A focal ulcer, significant pain or visual change requires a more urgent infectious assessment.

Ciprofloxacin 0.3% for an ophthalmologist-selected bacterial ulcer

Ciloxan's licensed ulcer schedule uses two drops per dose: every 15 minutes for the first six hours, then half-hourly to complete day one; hourly on day two; and four-hourly on days three through fourteen. Include overnight administration at each stage. Extension after day fourteen is physician-directed, with a maximum recommended 21 days in the SmPC.

Check quinolone allergy and review for hypersensitivity or deterioration. This schedule is not a treatment for Acanthamoeba, and treatment choice must follow the corneal assessment. Do not wear contact lenses during the infection.

Corneal scarring

Infection can leave opacity or irregular curvature that impairs vision even after microbiological control. A central lesion may have disproportionate consequences for daily visual function.

Progressive ulceration

Uncontrolled infection can deepen and thin the cornea, threatening perforation or intraocular spread. Continued lens wear or delayed treatment can worsen the opportunity for tissue damage.

Recurrent exposure

If the same contaminated equipment or unsafe routine is resumed, avoidable risk persists after recovery. Prevention therefore requires a specific review of actual handling practices.

  • Check objective visual and corneal improvement at each specialist review, as reduced discomfort alone may not demonstrate eradication of infection.
  • Ask directly about missed doses, especially overnight doses, and resolve supply, dexterity or support problems early.
  • Maintain urgent access for increasing pain, a new or enlarging corneal opacity, reduced vision or inability to continue the prescribed treatment.
  • If a sterile infiltrative event has been diagnosed, failure to improve as expected after lens cessation requires reassessment for infection or another cause.
  • Document clearance for lens resumption and the agreed prevention changes so that the next clinician can distinguish ongoing treatment from completed recovery.

Improvement after removal has limits

Sterile infiltrative disease often settles within about 48 hours after stopping lenses, but this expected course only helps after an appropriate assessment has distinguished it from infection.

Retain before replacing equipment

The lens and case may provide useful microbiological information in suspected Acanthamoeba or other infection. Obtain the eye service's advice before cleaning or discarding them.

No tap water in lens care

Tap water is unsuitable for rinsing or storing lenses or cleaning their storage case. Showering and swimming also expose lenses to water and should be avoided during wear.

Antibiotics do not cover every organism

A regimen suitable for a bacterial ulcer may fail against an amoebic or fungal pathogen. Exposure history, specialist testing and the observed response must guide reconsideration.
