Giant-cell arteritis is an inflammatory disease of medium and large arteries that predominantly affects older adults. In ophthalmic practice its importance lies in interruption of the arterial supply to the optic nerve and other ocular structures. Arteritic anterior ischaemic optic neuropathy can cause profound permanent loss, and the fellow eye may be threatened even if only one eye is currently symptomatic.
The immediate objective is to prevent further ischaemic injury while arranging confirmation. Treatment is justified by a sufficiently convincing clinical assessment and should not be postponed for a temporal-artery biopsy or vascular ultrasound. The subsequent pathway must revisit the diagnosis, evaluate systemic arterial involvement where indicated and manage the substantial consequences of prolonged glucocorticoid exposure.

Immune-mediated arterial inflammation
GCA involves inflammatory injury in medium and large arteries, particularly in older adults. Its precise initiating cause is incompletely understood, and a compatible clinical pattern is more useful than searching for one identifiable trigger.
Cranial and extracranial involvement
Disease can involve branches supplying the head and eyes as well as larger arteries elsewhere. The distribution explains why cranial symptoms, polymyalgia features and limb or systemic manifestations may occur in different combinations.
03PathophysiologyThe causal sequence from the underlying abnormality to symptoms and harm.
- 1Narrowing of the arterial lumen
Inflammation and vessel-wall remodelling impair blood flow through affected arteries. Tissue supplied downstream becomes vulnerable to ischaemia, particularly when collateral circulation cannot compensate for the reduction in perfusion.
- 2Optic-nerve head ischaemia
Compromise of the arterial supply to the anterior optic nerve can cause swelling and severe loss of visual function. The injury may become permanent rapidly, making treatment before further events more valuable than waiting for established damage.
- 3Transient and permanent ocular events
Intermittent reductions in ocular perfusion may cause brief visual blackouts before sustained infarction occurs. Diplopia can also reflect ischaemic involvement, so complete symptom resolution does not remove concern for the underlying arterial disease.
04Clinical features and red flagsSymptoms, examination findings, patterns of presentation and time-critical warnings.
Ask about new headache, scalp sensitivity, jaw pain or fatigue brought on by chewing, and tongue symptoms. Jaw claudication is more concerning than nonspecific temporomandibular discomfort. The absence of any one feature does not exclude disease when other findings support arteritis.
Transient monocular dimming or blackout, new diplopia and persistent visual loss all require urgent attention. Record timing, duration and whether the symptom was truly monocular. A normal interval acuity measurement after a transient episode does not establish that the eye is safe.
Severe visual loss with an afferent pupil defect and a pale swollen optic disc is concerning for arteritic anterior ischaemic optic neuropathy. The ophthalmic examination also considers retinal arterial ischaemia and other causes; a painless presentation is not reassuring.
Ask about weight loss, fever, fatigue, shoulder or hip-girdle stiffness and limb claudication. Examine pulses and blood pressure as clinically appropriate. Polymyalgia symptoms can coexist, but their absence does not exclude cranial disease or threatened vision.
Some patients present mainly with ocular ischaemia and lack a classic systemic history. Normal inflammatory markers reduce support for the diagnosis but cannot independently exclude it. Interpret age, symptoms, examination and investigations together rather than relying on a single result.

Non-arteritic ischaemic optic neuropathy
NAION also causes sudden optic-nerve dysfunction, often in patients with vascular risks. The distinction matters because suspected arteritic disease requires immediate systemic treatment, and age or a painless presentation cannot decide between them alone.
Embolic retinal ischaemia
Carotid or cardiac embolic disease can cause transient monocular loss or retinal arterial occlusion. Vascular assessment may be required alongside evaluation for arteritis when the clinical picture leaves both possibilities plausible.
Other headache and jaw disorders
Migraine, dental disease and temporomandibular disorders can imitate parts of the history. New symptoms in an older adult, especially with visual change or exertional jaw discomfort, require careful assessment rather than automatic attribution.

Intravenous access or scheduling must not postpone initial oral treatment when immediate intravenous delivery is unavailable.
Prednisolone when intravenous therapy is delayed
For acute or intermittent visual loss when intravenous treatment is not immediately possible, BSR permits 60–100 mg orally once daily for up to three days, then specialist-directed continuation.This is the visual-ischaemia bridge regimen, not a routine prolonged maintenance dose. Review diabetes, psychiatric history, infection and gastrointestinal risks without allowing routine checks to delay sight-saving treatment.
Prednisolone without visual ischaemia
Usually start 40–60 mg orally once daily, then taper according to disease control and the specialist plan rather than using a fixed short course.Long treatment can cause infection, hyperglycaemia, hypertension, osteoporosis and adrenal suppression. Establish monitoring and protection early, and give clear instructions about relapse, intercurrent illness and avoiding abrupt withdrawal.
08ComplicationsImportant consequences, why they occur and why they matter clinically.
Irreversible bilateral visual loss
Ischaemic injury can affect one eye and subsequently threaten the other. Prompt suppression of inflammation aims to preserve remaining vision, although it may not reverse established optic-nerve infarction.
Systemic arterial complications
Large-vessel involvement can produce limb ischaemic symptoms and later aortic complications in some patients. The need for further vascular assessment is determined by the disease pattern and specialist follow-up.
Glucocorticoid treatment morbidity
Prolonged systemic therapy can affect bone, glucose control, blood pressure, infection risk, mood and adrenal function. Monitoring and preventive measures form part of disease management rather than an optional addition after symptoms improve.

- At each review, ask directly about transient or persistent visual change, diplopia, jaw symptoms and new headache rather than relying solely on inflammatory markers.
- Review glucose, blood pressure, weight, mood, infection and bone health during glucocorticoid treatment, adjusting the prevention plan to the dose and duration.
- Monitor for relapse during tapering and arrange urgent reassessment if ocular symptoms occur; do not advise the patient simply to wait for the next routine appointment.
- Coordinate ophthalmology and rheumatology follow-up so visual recovery, disease activity, diagnostic confidence and treatment toxicity are assessed together.

Tocilizumab eligibility is specific
NICE TA518 supports tocilizumab for adults with relapsing or refractory GCA who have not previously received it, initially alongside tapering glucocorticoids. The recommendation limits treatment to at most one uninterrupted year and includes the applicable commercial arrangement; specialist selection is essential.
Visual treatment aims at prevention
A profoundly ischaemic optic nerve may not regain useful function despite prompt therapy. Starting treatment still matters because the other eye and other arterial territories may remain at risk, and apparent failure to restore sight does not show that treatment was unnecessary.
Markers are part of a clinical picture
Inflammatory results support diagnosis and follow-up but have limitations, including effects of treatment and other illnesses. A convincing new ocular ischaemic event deserves urgent reassessment even when a recent blood test appeared satisfactory.
Antithrombotic treatment needs its own indication
BSR does not recommend routine antiplatelet or anticoagulant treatment solely for GCA. A patient with another vascular indication may still need it, with the benefits and bleeding risks assessed through the relevant prevention pathway.
