The retina offers a direct view of a person's small blood vessels, but interpreting that view requires a timescale. Narrowed arterioles, increased wall reflection and arteriovenous crossing changes often reflect longstanding hypertension and arteriosclerosis. Fresh haemorrhage, cotton-wool lesions and leakage indicate a different degree of tissue disturbance. The examination should describe the lesions seen rather than stopping at a remembered grading label.
Hypertensive retinopathy is clinically valuable because it can reveal systemic disease before the patient reports cardiovascular symptoms. The immediate question is whether there is acute injury requiring hospital assessment; the subsequent question is how to achieve durable pressure control without reducing perfusion too abruptly. A normal retinal view does not exclude serious hypertension-related injury elsewhere, and a poor view cannot safely be treated as evidence that the fundus is normal.

Longstanding pressure exposure
Persistent systemic hypertension produces arteriolar remodelling and promotes vascular stiffness. Age, diabetes and renal disease can add to the burden, making the retinal appearance a marker of cumulative rather than purely momentary injury.
Acute destabilisation
Severe pressure may follow untreated disease, medication interruption, renal or endocrine disorders, sympathomimetic exposure or pregnancy-related disease. Identifying the precipitant helps prevent recurrence after the immediate pressure has been controlled.

- 1Autoregulatory narrowing
Retinal arterioles initially constrict in response to increased perfusion pressure. Sustained exposure changes vessel walls and produces chronic narrowing and altered light reflexes, with crossing changes where arterioles and veins share tissue.
- 2Barrier failure and ischaemia
When vascular regulation is overwhelmed, endothelial injury permits leakage while microvascular occlusion reduces perfusion. Haemorrhage, retinal oedema and cotton-wool lesions can therefore appear together within the same fundus.
- 3Vulnerable perfusion after lowering
Longstanding hypertension changes the pressure range over which organs maintain blood flow. A sudden excessive reduction can move perfusion below that range, explaining why rapid numerical normalisation can itself cause injury.
04Clinical features and red flagsSymptoms, examination findings, patterns of presentation and time-critical warnings.
Generalised or focal arteriolar narrowing, an increased vessel-wall light reflex and apparent compression of a vein at an arteriovenous crossing support chronic vascular change. These signs are influenced by age and observer interpretation, so avoid inferring the exact current pressure from a photograph.
Flame-shaped haemorrhages follow the superficial nerve-fibre layer, while cotton-wool spots represent focal axoplasmic-flow disturbance associated with ischaemia. Hard exudates reflect leakage. The distribution and the presence of diabetes or venous occlusion help distinguish overlapping processes.
Disc swelling, retinal oedema or a macular pattern of exudation may accompany severe hypertension. Check acuity, pupils, colour function and fields. Disc swelling is a descriptive finding; papilloedema specifically implies raised intracranial pressure and other causes must still be considered.
Ask about headache, altered cognition, chest pain, dyspnoea, urine output, pregnancy, stimulant use and abrupt withdrawal or poor tolerance of antihypertensive medicines. An apparently calm patient can still have serious organ injury, while pain or anxiety may also elevate the measurement.

- Use an appropriate cuff and supported arm and repeat a very high reading without delaying emergency care. Consider both arms initially and assess postural symptoms when relevant. Do not require ABPM before referral when severe hypertension and retinal haemorrhage or disc swelling coexist.

- Obtain creatinine, electrolytes, urine albumin or protein assessment, urinalysis, ECG and appropriate blood tests. Anaemia, thrombocytopenia or haemolysis may suggest thrombotic microangiopathy. Chest symptoms, breathlessness or reduced urine output warrant targeted acute investigations.

- Interpretation and limitations
- New neurological findings may require urgent brain imaging. Young age, abrupt onset, resistant hypertension or specific symptoms can prompt renal, endocrine or medication-related investigation. Acute illness affects some biochemical tests, so stabilisation and specialist interpretation matter.
06Differential diagnosisRealistic alternatives and the features that help distinguish them.
Diabetic retinopathy
Diabetes produces microaneurysms, haemorrhage and exudation that can coexist with hypertensive disease. Evaluate distribution and diabetic history rather than forcing all observed lesions into one explanation.
Venous occlusion
A sectoral haemorrhagic retina or diffuse unilateral venous engorgement suggests branch or central retinal vein occlusion. Associated macular oedema and ischaemia require their own retinal management.
Other swollen-disc disorders
Raised intracranial pressure, optic-nerve inflammation and disc anomalies may resemble hypertensive disc swelling. Symptoms, pupil findings, bilateral comparison and targeted imaging help determine whether another diagnosis is present.

Amlodipine for an agreed oral hypertension plan
In an adult selected for oral treatment, the usual starting dose is 5 mg by mouth once daily, increasing according to response and tolerability to a maximum of 10 mg once daily. Continue as ongoing blood-pressure therapy with scheduled review; it is not an as-needed rescue dose for an isolated high reading.Its SmPC does not establish safety and efficacy in hypertensive crisis; acute use requires the specialist plan rather than self-treatment. Avoid severe hypotension, shock, significant left-ventricular outflow obstruction and haemodynamically unstable heart failure after infarction. Titrate cautiously in older people and hepatic impairment, and assess ankle oedema, dizziness and interacting medicines.
08ComplicationsImportant consequences, why they occur and why they matter clinically.
Persistent visual impairment
Macular ischaemia, exudative damage or optic neuropathy can leave impaired acuity or fields after blood pressure improves. Anatomical improvement should therefore be considered alongside measured visual function.
Multiorgan injury
Retinal lesions may accompany acute renal dysfunction, cardiac failure or hypertensive encephalopathy. The eye examination can provide the first clue to a systemic process requiring immediate medical assessment.
Recurrent severe hypertension
Unresolved causes or difficulty continuing medicines can produce another episode. Reviewing adverse effects, access to treatment and follow-up arrangements is part of preventing further retinal and systemic damage.
09Monitoring and follow-upTreatment response, safety checks and longer-term review.
- After an acute presentation, the medical team should specify the next pressure and renal review rather than relying on a routine annual appointment. Monitor symptoms of hypoperfusion as well as whether the numerical target has been reached.
- Follow retinal haemorrhage, exudation and disc swelling over time, using images where possible. Structural regression may lag behind blood-pressure improvement, while established optic-nerve or retinal damage may remain.
- For severe pressure without signs requiring same-day referral, NICE advises prompt organ-damage assessment and confirmation or clinical review within seven days when no damage is identified. This pathway does not apply to severe pressure with acute retinal haemorrhage or papilloedema.
- Discuss whether visual impairment affects driving, medication reading or falls risk. Persistent central or field loss merits ophthalmic follow-up and practical support even when systemic treatment is effective.

The meaning of disc swelling
Some guidance uses papilloedema within its definition of accelerated hypertension. At the bedside, describe what is visible and avoid assuming its mechanism. Raised intracranial pressure, optic neuritis, infiltrative disease and pseudopapilloedema may need a separate explanation, especially when findings or recovery are atypical.
Pregnancy and the postpartum period
Hypertension with headache or visual symptoms during pregnancy or after birth needs urgent assessment for pre-eclampsia and neurological complications. Adult essential-hypertension thresholds and drug choices cannot simply be transferred to that setting; obstetric assessment should run alongside appropriate eye and neurological evaluation.
Retinal injury without dramatic symptoms
Advanced vascular changes may be detected during an eye examination in a person who feels well. Lack of headache is not reassuring when the measured pressure and fundus meet urgent criteria. Conversely, chronic crossing changes alone should not trigger an unsupported diagnosis of an acute emergency.
Communication between services
A referral should state actual blood-pressure values, symptoms, each eye's acuity and the retinal lesions observed. Sending only a grade number can conceal whether there is fresh haemorrhage or disc swelling. Confirm which medical service will assess the patient and how ophthalmic findings will be followed.
