The benefit of an ocular corticosteroid depends on identifying a steroid-responsive inflammatory process and controlling any relevant infectious component. Topical delivery does not make the treatment harmless or confine all effects to visible surface redness. Steroids suppress inflammation and can make an eye appear more comfortable while an infection persists, corneal tissue weakens or intraocular pressure rises. Before prescribing or renewing treatment, establish who made the diagnosis, what examination was performed, the intended duration and who will review the response.
The main diagnostic hazard is treating an undifferentiated red eye. Epithelial herpes simplex, fungal disease and other microbial keratitis may deteriorate with inappropriate corticosteroid exposure. A combined antibiotic preparation does not cover every organism and cannot compensate for an unrecognised ulcer. The relationship between infection and steroids is nevertheless disease-specific: selected stromal herpetic inflammation may receive a specialist steroid regimen with antiviral treatment. That carefully assessed indication must not be generalised to an epithelial dendrite or a painful red eye without examination.
A safe prescription is a complete plan rather than a bottle name. Identify the affected eye, formulation and concentration, initial frequency, intended treatment period and how any taper will be decided. Explain why follow-up continues after pain improves. Review requires examination of inflammatory activity, corneal integrity, visual function and intraocular pressure as appropriate. The product information gives supervision thresholds, but these are not guaranteed safe intervals without monitoring: severe inflammation, glaucoma, a previous steroid response or vulnerable ocular tissue may require earlier and more frequent review.

New or increasing pain, photophobia, discharge, a corneal opacity or persistent visual loss during treatment should trigger urgent reconsideration of infection and the original diagnosis.
Raised intraocular pressure can occur without a noticeable symptom, so the absence of headache or blurred vision cannot replace planned pressure measurements.
Corneal or scleral thinning and an unhealed epithelial defect increase concern for impaired healing or perforation and require specialist assessment of treatment suitability.
Unclear indication, indefinite repeats, conflicting taper instructions or no named reviewing service creates avoidable risk even when the original steroid choice was appropriate.

Prednisolone acetate 1% suspension, Pred Forte
For a confirmed steroid-responsive indication, the adult SmPC dose is one to two drops two to four times daily; the initial 24 to 48 hours may use two drops hourly. The ophthalmologist determines the indication-specific duration and taper.Shake before use. Contraindications include untreated purulent infection, epithelial herpes simplex, fungal and mycobacterial ocular disease. Use beyond ten days requires strict ophthalmic supervision and pressure checks; thinning tissue, glaucoma, cataract and masked infection are important risks.
Dexamethasone 0.1% suspension, Maxidex
For severe inflammation under ophthalmic supervision, the SmPC permits one to two drops every 30 to 60 minutes until a satisfactory response occurs, then reduction towards one drop every four hours. Subsequent duration and taper follow the specialist examination.Shake the suspension and use punctal occlusion. The SmPC contraindicates herpes simplex keratitis, specified viral ocular disease, fungal or untreated parasitic infection, mycobacterial infection and acute untreated bacterial infection. Use beyond one week requires ophthalmic supervision with regular pressure assessment.
06Complications, monitoring and follow-upAdverse effects, response and longer-term review.
- Give the patient a written schedule and ask them to explain it back, particularly when the initial frequency is high or a taper changes across days.
- Use the product-specific supervision requirements: Pred Forte beyond ten days and Maxidex beyond one week require ophthalmic oversight, with earlier monitoring when risk or severity warrants it.
- Check for pressure elevation and continuing inflammation even when the eye feels comfortable; neither can be reliably excluded by symptom reporting alone.
- Review bottle hygiene, opening expiry and administration; shake suspensions, avoid touching the tip and use punctal occlusion for about one minute to reduce drainage.
- Leave at least five minutes between different eye drops when used together, follow product instructions and clarify the order when an ointment is also prescribed.
- Ensure renewed pain, photophobia, a corneal opacity or worsening vision leads to urgent contact before the scheduled appointment.

Different products have different instructions
Steroid potency, formulation and licensed schedules differ. Substituting a preparation or copying another product's frequency without an appropriate prescription can alter treatment intensity and risk.
An antibiotic combination has limits
The antibacterial component does not reliably treat viral, fungal or amoebic infection. A combination bottle therefore does not remove the need to establish the cause of redness.
Systemic absorption still occurs
Ocular drugs can drain through the nasolacrimal system and be absorbed. Punctal occlusion reduces drainage, and prolonged intensive treatment warrants particular care in susceptible patients and children.
Tapering requires a clinical reason
Reducing treatment follows control of the underlying inflammation and the prescriber's plan. A fixed generic taper cannot account for different diagnoses, relapse risks or new infectious concerns.
Contact lenses require specific advice
Active ocular infection or significant inflammation usually prevents lens wear. Do not interpret a preservative-related removal interval as permission to resume lenses during an unresolved eye disorder.
Interactions can increase systemic exposure
Ask about ritonavir, cobicistat and other strong CYP3A inhibitors. These can increase systemic corticosteroid effects, including Cushing syndrome and adrenal suppression, despite ocular administration. Coordinate the interaction assessment, monitoring and any treatment change with the ophthalmic and prescribing teams, particularly after intensive or prolonged use.
