New visual loss with retinal separation
A tractional or exudative detachment can threaten central vision even when the patient has no classic flashes or obvious retinal tear.
Action: Arrange urgent same-day ophthalmic assessment for new visual loss, a field defect or suspected retinal detachment. Communicate diabetes, inflammatory symptoms, trauma or a suspected mass. Macula-threatening diabetic detachment requires prompt vitreoretinal-team assessment rather than routine screening follow-up.
