Transient versus persistent visual loss

Use the time course and anatomical pattern of visual loss to recognise retinal, optic-nerve and cerebral emergencies, including episodes that have already resolved.

Sudden persistent loss, a new homonymous field deficit, or transient monocular blackout may represent retinal or cerebral ischaemia; visual symptoms with giant cell arteritis features threaten further irreversible loss.

Action: Activate emergency eye or stroke assessment for ongoing sudden loss, arrange immediate suspected-TIA referral after a resolved ischaemic episode, and start urgent clinician-directed glucocorticoid treatment with same-day specialist assessment when GCA is strongly suspected.

Visual loss can mean complete darkness, a patch missing from the field, reduced sharpness or distortion. Ask the patient to describe what they could and could not see, then reconstruct the onset and recovery in minutes or seconds where possible. Establish the last time vision was definitely normal, especially after waking with symptoms. Persistent and transient describe the time course, not severity or safety: a short-lived vascular episode can carry substantial risk, while a slowly progressive optical problem may follow a different pathway.

Localisation begins with laterality but requires careful questioning. Did the patient actually cover each eye during the event, or assume one eye was affected because one side of the scene disappeared? Retinal and optic-nerve disorders usually produce monocular findings, whereas a homonymous deficit reflects the same hemifield in both eyes and suggests a lesion behind the chiasm. Examine fields in each eye even when the reported acuity is good. A focal cortical visual deficit may be missed by an assessment limited to facial symmetry, arm strength and speech.

An abrupt painless monocular blackout that fully resolves can represent retinal ischaemia and should enter a suspected-TIA assessment pathway. A normal examination afterwards is compatible with a transient event. Ongoing sudden loss may represent retinal arterial occlusion, another retinal emergency or a cerebral event and requires immediate eye and, where indicated, stroke assessment. Do not attempt to prove the cause with prolonged bedside fundoscopy before contacting the relevant team. Distinguish an ongoing deficit from a resolved episode before applying a TIA medication plan.

Positive visual phenomena help characterise an episode but require context. Typical migraine aura may spread across the visual field over several minutes and include shimmering, zigzags or a developing scotoma, with or without subsequent headache. NICE describes fully reversible symptoms developing over at least five minutes and lasting five to sixty minutes. Abrupt maximal onset, persistent loss, a first atypical episode or visual symptoms confined to one eye should prompt reconsideration and further investigation or referral. A previous migraine diagnosis does not protect against a new vascular or retinal disorder.

Pain and associated symptoms broaden the differential. Painful red eye with haloes and vomiting suggests acute angle closure; photophobia with corneal signs suggests an anterior-segment process. Pain on movement with impaired colour perception can indicate optic-nerve inflammation. Flashes, floaters and an advancing shadow suggest vitreoretinal pathology. These patterns overlap, so combine symptoms with examination and age, vascular risks, trauma, recent surgery and medicines. Gradual central distortion can indicate macular disease and still need urgent referral, particularly when active neovascular AMD is suspected.

Giant cell arteritis must be considered in older adults with ischaemic visual symptoms. Ask about new headache, scalp tenderness, jaw or tongue claudication, constitutional change and shoulder or hip girdle stiffness. The absence of one classic symptom or a single reassuring blood result does not conclusively exclude it. Obtain blood counts and inflammatory markers promptly when this will not delay treatment, arrange the local fast-track pathway and seek same-day ophthalmology for visual involvement. Confirmatory vascular ultrasound or biopsy remains necessary as directed, but preventing further ischaemic damage takes priority over waiting for the test.

Management depends on the working mechanism. For a resolved episode judged to be suspected TIA, NICE recommends immediate aspirin 300 mg daily unless contraindicated and specialist assessment within twenty-four hours of onset. Ongoing suspected stroke instead requires its acute imaging and treatment pathway; do not simply apply the pre-assessment TIA regimen to persistent deficits. Suspected GCA requires immediate glucocorticoid treatment with specialist coordination. A threatened retina or painful ocular emergency needs eye-service treatment. When uncertainty spans specialties, make direct contact so the patient receives coordinated assessment rather than sequential routine referrals.

Retinal or cerebral vascular disease

Embolism, thrombosis or impaired perfusion can interrupt retinal or cerebral visual function. Carotid and cardiac disease are potential sources, and recovered vision does not establish that the underlying vascular risk has resolved.

Ocular and optic-nerve pathology

Retinal detachment, vitreous haemorrhage, macular disease, optic neuropathy and corneal or pressure-related disorders can reduce vision. Onset, pain, field pattern and associated ocular signs help prioritise these possibilities.

Inflammation and migraine mechanisms

Giant cell arteritis can compromise the blood supply to the eye and optic nerve. Migraine aura can cause reversible visual symptoms, but an atypical pattern requires assessment for alternative causes.

    Temporary versus sustained perfusion failure

    Brief interruption of blood supply may cause reversible dysfunction, whereas sustained ischaemia can produce infarction and lasting loss. Symptom duration alone cannot reliably determine the vessel involved or future risk.

    Prechiasmal and retrochiasmal localisation

    Disease affecting one retina or optic nerve usually alters that eye's function. Damage after the chiasm can affect the same visual hemifield in both eyes despite preserved central acuity.

    Retinal separation and optical obstruction

    Detachment disrupts the relationship between sensory retina and underlying supporting tissue. Blood or another opacity can instead block light transmission, producing loss without the same primary neural injury.

    GCA-related arterial inflammation can reduce ocular perfusion and cause irreversible optic-nerve or retinal injury. Treatment aims to control inflammation and protect remaining vision, including the fellow eye.

Recovered monocular blackout

A sudden darkening of one eye followed by complete recovery remains concerning for retinal ischaemia. Document the onset and duration, check for GCA features and arrange immediate suspected-TIA assessment rather than a routine sight test.

Persistent hemifield loss

Missing the same side of the visual scene through either eye suggests a homonymous deficit. A new sudden pattern requires emergency stroke assessment even when speech and limb strength seem unaffected.

Visual symptoms with jaw claudication

New chewing-related jaw pain with headache or constitutional symptoms in an older adult raises suspicion of GCA. Begin urgent clinical treatment and same-day specialist coordination without waiting for vascular imaging.

Flashes with an expanding shadow

A new shadow or curtain alongside photopsia or floaters suggests retinal tear or detachment. A central direct ophthalmoscopic view is insufficient to exclude a peripheral lesion.

Red flags requiring action

  • Abrupt monocular blackout remains urgent after recovery; suspected TIA requires immediate referral for specialist assessment within twenty-four hours of symptom onset, without using ABCD2 to defer review.
  • In a person over fifty, visual symptoms with new headache, jaw claudication, scalp tenderness or constitutional symptoms require urgent consideration of giant cell arteritis and treatment without waiting for confirmatory testing.

Investigation order

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

Typical visual migraine aura

Gradual evolution, complete reversibility and a familiar repeated pattern support aura. Truly monocular symptoms, abrupt onset or persistent deficits require further consideration rather than an automatic migraine label.

Retinal ischaemia or cerebral TIA

Sudden negative visual symptoms that resolve may reflect an ischaemic event. Determine actual laterality and associated neurological findings and arrange urgent vascular assessment even if vision is normal now.

Detachment, haemorrhage or macular disease

A curtain with flashes, obscuration with floaters or new central distortion suggests different retinal processes. Specialist examination is needed when the peripheral retina or macula cannot be assessed adequately.

Optic neuropathy or anterior eye disease

Colour impairment and afferent dysfunction suggest optic-nerve or severe retinal disease. Redness, corneal abnormalities and pressure-related symptoms indicate an ocular process but do not replace a full visual assessment.

Aspirin for a resolved suspected TIA

Start 300 mg orally once daily immediately when a clinician suspects TIA, unless contraindicated, and continue pending urgent specialist assessment and an agreed secondary prevention plan.

Check allergy, active bleeding and major bleeding risk; existing anticoagulation or complex antithrombotic treatment needs urgent clinical advice. Persistent suspected stroke follows its own imaging and treatment pathway, including exclusion of intracerebral haemorrhage before routine aspirin.

Immediate treatment for acute or intermittent visual loss attributed to strongly suspected arteritis.

Glucocorticoids for GCA with ischaemic visual symptoms

Under urgent specialist direction, BSR permits methylprednisolone 500 mg–1 g intravenously once daily for up to three consecutive days, followed by oral prednisolone. If intravenous therapy is not immediately possible, start oral treatment without delay; prednisolone 60–100 mg orally once daily may be used for up to three days, then continue the specialist-directed regimen and eventual taper.

Arrange same-calendar-day ophthalmology and specialist dose selection, with glucose, blood pressure, infection and mental-state monitoring. Consider cardiac and metabolic risks during intravenous pulses; manage bone and gastrointestinal risk during prolonged therapy. Do not stop glucocorticoids abruptly or wait for confirmatory imaging before initiating a strongly indicated treatment.

Permanent visual impairment

Retinal, optic-nerve or cerebral damage can leave lasting acuity or field loss. Delay in recognising a treatable cause may reduce the opportunity to preserve remaining function.

Further vascular events

An ischaemic transient visual episode may signal a need for urgent stroke prevention. Investigation can identify carotid or cardiac disease requiring a more specific secondary prevention strategy.

03

Fellow-eye involvement in arteritis

GCA can threaten the other eye after an initial visual event. Improvement in headache does not establish that ocular risk is controlled, so specialist treatment and monitoring remain essential.

  • Confirm that the urgent eye, stroke or GCA referral was received and that the patient understands the next assessment time and location.
  • Review the final diagnosis and specialist prevention plan so an initial short-term aspirin or glucocorticoid instruction is not left indefinitely without oversight.
  • Assess persistent visual disability, mobility, work and driving implications and coordinate rehabilitation or low-vision support when needed.
  • Provide explicit emergency advice for recurrent blackout, a new field defect, worsening vision or accompanying speech, balance or limb symptoms.

The clock belongs in the referral

State the last-known-well time, onset, duration and whether recovery was complete. This lets the receiving service select the appropriate acute or transient-event pathway.

Pain is not a severity scale

Retinal arterial occlusion and many detachments can be painless, while painful surface disease may have a different prognosis. Absence of pain should never be used as reassurance about sudden loss.

The fellow eye can reveal localisation

Testing each eye's field may show that the missing region is shared between them, redirecting assessment from a presumed isolated eye problem towards a retrochiasmal process.

One pathway may need another

A retinal vascular presentation can require both ophthalmic assessment and stroke prevention. Direct coordination helps avoid an apparent specialty boundary delaying the time-sensitive part of care.
