Vitamin supplementation in infancy

Prescribe age-appropriate vitamin D and A, C and D supplements, recognise when formula already supplies enough, prevent duplicate dosing, distinguish routine prevention from treatment of deficiency, and adapt plans for prematurity, restricted diets and malabsorption.

Symptomatic calcium disturbance or severe deficiency

Seizure, tetany, stridor, apnoea, arrhythmia, marked hypotonia or acute encephalopathy may reflect hypocalcaemia or another serious metabolic disorder and is not managed by routine vitamin drops.

Action: Stabilise airway, breathing and circulation, check bedside glucose and obtain urgent paediatric assessment with ECG, adjusted or ionised calcium, phosphate, magnesium, alkaline phosphatase, renal function, parathyroid hormone and vitamin D as directed. Correct symptomatic hypocalcaemia in a monitored setting using the local paediatric protocol; investigate the cause before setting a longer-term vitamin regimen.

01Role and principlesWho benefits and the main preventive aims.

Vitamin D promotes intestinal calcium and phosphate absorption and skeletal mineralisation. UK sunlight is unreliable for cutaneous synthesis during winter, and infants are protected from direct sun, so dietary prevention is needed.

Breast milk supports infant nutrition but usually contains too little vitamin D to meet the preventive recommendation. Maternal antenatal supplementation improves maternal status but does not remove the infant's recommendation after birth.

Infant formula is fortified with vitamin D and other micronutrients. The 500 mL threshold prevents unnecessary duplicate vitamin D when formula intake is substantial while ensuring supplementation as milk volume falls during weaning.

Vitamin A supports vision, epithelial integrity and immunity, while vitamin C supports collagen formation and iron absorption. UK advice therefore uses combined A, C and D drops from 6 months to 5 years for children not receiving enough fortified formula.

A precise history is more useful than asking whether the child receives vitamins. Record brand, strength per drop or millilitre, measured volume, frequency, who administers it and every other supplement in the household.

Micronutrient needs differ after preterm birth because mineral stores and placental transfer are reduced and growth is rapid. Neonatal units may prescribe vitamins, iron or phosphate with gestation- and feeding-specific review points.

Restricted diets can be nutritionally complete when planned, but vitamin B12 has no reliable unfortified plant source. Deficiency may cause anaemia and neurological harm before the diet is recognised as inadequate.

Prevention includes access and adherence. A once-daily preparation, demonstration with the supplied device, written dose, safe storage and linkage to Healthy Start are more effective than a vague instruction to buy vitamins.

Biochemical testing is unnecessary before routine preventive supplementation in a well child. Test when symptoms, bone findings, chronic disease, malabsorption, medication exposure or a therapeutic high-dose course changes management.

Treatment of established deficiency requires assessment of calcium balance, dietary intake and underlying disease. Recheck according to the specialist plan and step down to prevention only after the treatment course is complete.

  • First-line prevention for a breastfed infant is a daily vitamin D supplement containing 8.5–10 micrograms from birth, even when the breastfeeding parent takes vitamin D.

  • Healthy Start vitamin drops provide A, C and D for eligible families and may also be locally available universally; help the family obtain them rather than giving advice alone.
  • Vitamin K given after birth prevents vitamin K deficiency bleeding and is a separate intervention; it does not replace daily vitamin D or A, C and D drops.
  • Healthy term infants do not routinely need medicinal iron drops. From around 6 months, provide iron-rich complementary foods; use a prescribed neonatal plan for preterm or low-birth-weight infants.
  • Vitamin B12, iodine, iron and calcium need explicit review in vegan or highly restricted diets. Dietitian input is preferable to improvised high-dose supplementation.
  • When deficiency or rickets is suspected, investigate and treat using a paediatric therapeutic regimen. Routine preventive doses are not adequate treatment, and high-dose products should not be used without supervision.

Milk and complementary diet

Quantify breast milk pattern, average formula volume, fortification, age at complementary feeding and regular sources of vitamin D, calcium, iron and vitamin B12.

Exact supplement exposure

Inspect or photograph every bottle, noting active ingredients, concentration, device, dose, frequency, adherence and possible duplication between drops and fortified products.

Risk context

Ask about gestation, birth weight, neonatal discharge plan, skin pigmentation, sun exposure, covering clothing, chronic gut, liver or kidney disease and enzyme-inducing medicines.

Growth and skeleton

Plot growth and examine tone, motor development, fontanelle, skull, ribs, wrists, legs, gait, dentition and tenderness when deficiency is possible.

Dietary restriction

Clarify veganism, food allergy exclusions, sensory restriction, food insecurity and whether the breastfeeding parent's diet or supplement use limits transferred nutrients.

Toxicity clues

Look for nausea, constipation, thirst, polyuria, dehydration, weakness or renal symptoms when concentrated or multiple vitamin D preparations may have been used.

Red flags requiring action

  • Seizures, apnoea, stridor, tetany, carpopedal spasm or arrhythmia requires emergency assessment for hypocalcaemia and other metabolic causes.
  • Poor growth, bone pain, delayed motor development, muscle weakness, widened wrists, bowed legs or a persistently wide fontanelle raises nutritional rickets.
  • Prolonged jaundice, pale stool, dark urine, chronic diarrhoea or steatorrhoea suggests cholestasis or malabsorption and makes routine oral dosing unreliable.
  • Prematurity, very low birth weight, prolonged parenteral nutrition or complex neonatal disease requires the neonatal discharge regimen rather than a generic healthy-term schedule.
  • A restricted vegan diet without a reliable vitamin B12 source risks irreversible neurological injury; assess the breastfeeding parent's intake as well as the infant's.
  • Repeated vomiting, constipation, polyuria, dehydration or lethargy in a child receiving several supplements raises vitamin D toxicity and hypercalcaemia.

Vitamin D preventive supplement

Breastfed infants from birth: 8.5–10 micrograms (340–400 units) orally once daily. Formula-fed infants: give when formula intake is below 500 mL daily. Age 1–4 years: 10 micrograms (400 units) daily.

Select an age-appropriate product, prescribe its exact volume, check strength and all concurrent vitamins, and keep concentrated drops out of reach. Treatment doses for deficiency are different and need paediatric supervision.

The threshold follows formula volume

A mixed-fed infant may need vitamin D even before 6 months when average fortified-formula intake remains below 500 mL daily.

Micrograms prevent errors

Different products use drops, millilitres and international units; writing both product volume and microgram dose reduces tenfold mistakes.

Prevention is not rickets treatment

A child with skeletal signs needs mineral assessment and a therapeutic pathway because calcium deficiency, malabsorption or renal disease may coexist.

B12 is time critical

Neurological injury from infant B12 deficiency may become irreversible, so a restrictive maternal or infant diet deserves direct enquiry and rapid specialist management.

Access is clinical

Healthy Start eligibility, pharmacy availability, language and dosing literacy determine whether an apparently simple prescription is actually delivered.
