The explanation should support confidence without promising that every possible adverse effect can be predicted or prevented.
Communication is also part of consent and shared decision making. People may reasonably value benefits and harms differently, have concerns about dependence or long-term treatment, or face barriers that are not visible in a clinic. Ask rather than infer. A person who says they cannot take a medicine on an empty stomach may be describing a shift-work routine, nausea or a misunderstanding of the timing requirement. Resolving that issue may enable effective treatment; dismissing it as poor adherence may not. Check whether the person can read the label, open packaging, measure a liquid or use a device before assuming that verbal understanding guarantees successful administration.
A patient who cannot remain upright or has significant swallowing problems may be unsuitable for a particular oral bisphosphonate. Do not simply repeat the standard instructions if the person cannot follow them. The disclosure should trigger a treatment review and consideration of an appropriate alternative.

Numbers need a common frame
