Name the clinical phenotype before treating. Acute CPP crystal arthritis begins over hours with swelling, heat and severe pain, most often in a knee or wrist but also ankle, elbow, shoulder or hip. Chronic CPP inflammatory arthritis produces persistent or fluctuating wrist, MCP and large-joint synovitis. Osteoarthritis with CPPD involves unusual sites or compartments, including radiocarpal, second and third MCP, glenohumeral and lateral knee disease. Incidental chondrocalcinosis without symptoms needs explanation, not anti-inflammatory treatment.
Examine for a true effusion and restriction of both passive and active movement. Map every inflamed joint because oligoarticular attacks occur. Look for bony enlargement, wrist collapse, second- and third-MCP involvement and haemochromatosis signs. In acute neck pain, assess meningism, temporal arteries, neurological deficits and infection. Crowned dens syndrome classically causes severe upper-neck pain and restricted rotation in an older adult, sometimes with fever and high CRP.
Synovial fluid is the diagnostic reference when uncertainty matters. CPP crystals are typically rhomboid or rod-shaped and weakly positively birefringent, appearing blue when parallel and yellow when perpendicular to the compensator axis. They are less conspicuous than urate crystals and may be missed. Request Gram stain, culture and cell count with microscopy because infection and crystals coexist. No white-cell threshold safely excludes bacterial disease.
Use metabolic testing selectively. In onset under sixty, extensive deposition, recurrent attacks or suggestive family or systemic history, measure corrected calcium, magnesium, phosphate, alkaline phosphatase, PTH, ferritin and transferrin saturation. Low alkaline phosphatase can suggest hypophosphatasia; high calcium with non-suppressed PTH suggests hyperparathyroidism; a high transferrin saturation directs HFE assessment. Treating these conditions improves systemic risk but may not reverse established cartilage deposits.
For a single accessible joint, aspirate to relieve pressure and establish culture and crystal data, then inject intra-articular corticosteroid once infection is reasonably excluded. This limits systemic exposure in an older patient with multiple comorbidities. Local cool packs and short rest help initially; begin gentle range and load progression as pain permits so immobilisation does not create weakness and stiffness.
When local injection is not feasible, use the lowest-risk short systemic option. Colchicine 500 micrograms twice daily for a short course is commonly used off-label; avoid the historic high-dose strategy and check renal, hepatic and interaction risk. A short NSAID course with gastroprotection or prednisolone 20–30 mg daily for three to five days are alternatives under local practice. Revisit sepsis if inflammation fails to settle rather than simply extending treatment.
Recurrent attacks may justify colchicine 500 micrograms once or twice daily prophylaxis under rheumatology review. Persistent inflammatory CPPD has a weak evidence base; EULAR permits NSAID with gastroprotection, low-dose colchicine, low-dose corticosteroid and selected hydroxychloroquine or methotrexate approaches. Define whether the goal is fewer acute attacks or control of true ongoing synovitis, and stop an ineffective toxic trial.
There is no licensed crystal-depleting therapy. Do not prescribe allopurinol for CPP crystals unless the patient separately has gout. Treat coexisting osteoarthritis with tailored strengthening, aerobic activity, weight support when relevant, walking aids, topical NSAID and surgical referral based on symptoms and function. Review metabolic disease, falls and medicine burden, because repeated acute-care prescribing can be more harmful than the deposits themselves.

Ageing and cartilage susceptibility
CPP deposition rises markedly with age as cartilage extracellular-matrix and chondrocyte pyrophosphate regulation change, especially in fibrocartilage and hyaline cartilage.
Joint injury and osteoarthritis
Previous trauma, meniscectomy, instability and osteoarthritic cartilage damage create a local environment that favours deposition and atypical structural degeneration.
Metabolic associations
Hereditary haemochromatosis, primary hyperparathyroidism, hypomagnesaemia and hypophosphatasia are established associations, particularly in younger or extensive disease.
Familial deposition
Rare inherited disorders of extracellular pyrophosphate regulation cause earlier, widespread and sometimes strongly familial CPPD phenotypes.

Septic arthritis
Bacterial infection is the priority alternative in any acute hot joint and can coexist with CPP crystals, so culture is not optional when sepsis is plausible.
Gout
Monosodium-urate disease favours podagra and shows strongly negative needle crystals; serum urate and imaging support but do not replace fluid when uncertainty matters.
Osteoarthritis flare
Mechanical pain and bony enlargement may coexist, but an abrupt effusion with marked warmth suggests superimposed crystal inflammation or infection.
Rheumatoid or polymyalgia phenotype
Chronic symmetric wrist and MCP synovitis can mimic rheumatoid disease, while shoulder and hip inflammation in older adults can resemble polymyalgia rheumatica.
Crowned dens mimics
Meningitis, cervical discitis, epidural abscess, giant cell arteritis, subarachnoid haemorrhage and mechanical neck disease require exclusion from the acute febrile neck presentation.

Intra-articular triamcinolone acetonide
After aspiration and infection exclusion, inject 5 to 10 mg into a small joint or up to 40 mg into a large joint using the 40 mg/mL suspension and strict asepsis; dose depends on joint size and inflammation.CPPD is not a named licensed indication, although the product is licensed for inflammatory joint disorders. Never inject through infection or into a possibly septic or unstable joint. Warn about glucose rise, post-injection flare and infection and do not use fixed repeat intervals. Pregnancy requires an individual benefit-risk review.
Colchicine for acute CPP arthritis
A common specialist off-label regimen is 500 micrograms orally twice daily for 3 to 5 days, reduced to once daily in selected older or renally impaired patients and stopped promptly when symptoms settle or toxicity occurs.This indication is off-label. Check eGFR, liver disease and macrolide, azole, ciclosporin, verapamil and statin interactions. Stop for diarrhoea or vomiting; accumulation can cause marrow failure, neuromyopathy and multiorgan toxicity. Pregnancy requires specialist benefit-risk review.
Naproxen for acute CPP arthritis
A common off-label course is 250 to 500 mg orally twice daily with food for 3 to 5 days, using the lowest effective dose and adding a proton-pump inhibitor when gastrointestinal risk warrants it.Avoid active ulcer, severe renal, hepatic or heart failure, NSAID hypersensitivity, concurrent NSAIDs and significant anticoagulant risk. Review ACE inhibitor and diuretic combinations. Avoid from 20 weeks of pregnancy unless essential and do not use in the final trimester.
Prednisolone for acute CPP arthritis
When local injection, colchicine and NSAID are unsuitable, a common off-label regimen is 20 to 30 mg orally once daily for 3 to 5 days, with a longer taper only for a clearly documented persistent inflammatory phenotype.Can mask infection and worsen diabetes, pressure, mood, fluid retention and gastrointestinal or bone risk. Repeated courses require adrenal review; pregnancy usually favours prednisolone if systemic steroid is necessary but should remain individually supervised.

Accelerated structural arthropathy
Deposition-associated cartilage loss can produce severe knee, wrist, shoulder, hip or MCP degeneration and eventual joint-replacement need.
Crowned dens morbidity
Unrecognised peri-odontoid deposition causes severe pain, immobility and repeated invasive infection or vasculitis work-ups; true neurological compression is uncommon but serious.
Anti-inflammatory medicine harm
Older patients often have CKD, bleeding and cardiovascular risk, making repeated NSAID, colchicine and steroid exposure a major source of morbidity.

Weak crystals are easy to miss
CPP birefringence is less vivid than urate, and low numbers or delayed examination can produce a false-negative microscopy report.
Calcification is not symptoms
Chondrocalcinosis may be incidental; current joint pain must still fit an inflammatory or structural CPPD phenotype and alternatives need assessment.
Atypical OA is a clue
Radiocarpal and second or third MCP degeneration or lateral-compartment knee disease should prompt CPPD and haemochromatosis thinking.
Crowned dens is a CT diagnosis
Plain cervical films frequently miss peri-odontoid deposits, whereas targeted CT demonstrates the crown-like calcification.
Metabolic treatment has limits
Correcting magnesium, parathyroid or iron disease benefits systemic health but cannot be promised to remove established cartilage crystals.
