CRP rises after cytokine stimulation, especially interleukin-6, and falls as synthesis decreases. Its kinetics make it useful for following many acute inflammatory states, but immunomodulatory medicines that block relevant pathways may blunt the response. ESR measures how far red cells settle under standard conditions and changes more slowly. It is influenced by plasma proteins and red-cell number, shape and size, so it often remains elevated after the acute process improves.
Interpretation begins with pre-test probability and timing. In a hot swollen joint, aspiration and cultures answer the dangerous question; CRP only supports severity and trend. In suspected giant-cell arteritis, the combination of age, new headache, scalp tenderness, jaw symptoms, visual disturbance and vascular examination determines immediate action. A normal marker reduces probability but does not justify delay when vision is threatened.
Discordance is informative only after artefact and context are considered. Raised ESR with normal CRP may occur with anaemia, high immunoglobulins, renal disease or resolved inflammation. Raised CRP with normal ESR can occur early in infection or tissue injury. In systemic lupus erythematosus, CRP may be less elevated during some immune flares than expected, while infection can produce a stronger rise; this is a clue, not a diagnostic rule.
Monitoring should be tied to a decision. A falling CRP after source control can support response but cannot confirm eradication. Persistently raised markers in treated arthritis may reflect ongoing synovitis, infection, obesity or another disease. Conversely, normal markers with pain may reflect damage, fibromyalgia or active disease that is poorly captured by biomarkers. Always compare with joint counts, organ findings and functional trajectory.

- CRP is a hepatic acute-phase protein that changes relatively quickly; ESR is an indirect sedimentation measure affected by fibrinogen, immunoglobulins, red-cell characteristics, age and pregnancy.

- Disease activity scores may incorporate markers, but patient pain and comorbidity can diverge from synovitis; combine scores with examination and function.
- A very high or unexpectedly persistent inflammatory response should reopen infection, malignancy and tissue injury rather than automatically prompting more immunosuppression.
02Indications, selection and cautionsWhen it is useful, when urgency changes and important limitations.
A rapid CRP increase with systemic illness supports an acute inflammatory or infectious process and should be interpreted alongside cultures, imaging and organ function.
Anaemia, paraproteins, renal disease, pregnancy and age can increase ESR independently, especially when CRP and examination do not show comparable inflammation.
Objective synovitis, inflammatory back pain or compatible vascular symptoms can occur with normal values, so clinical referral and definitive testing may still be required.
Markers that remain high despite apparently controlled rheumatic disease should prompt review for occult infection, malignancy, tissue injury and treatment adherence before escalation.
Severe pain with low objective inflammation may reflect structural damage or pain amplification, while limited symptoms can coexist with silent organ inflammation.

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

ESR is a systems measurement
It reflects plasma proteins and red-cell behaviour as well as inflammation, explaining why anaemia and paraproteins can dominate the result.
CRP kinetics matter
A result drawn early may precede a rise, while a falling value after treatment can lag behind or fail to reveal residual local infection.
Biologics can change the signal
Therapies targeting inflammatory cytokines may blunt acute-phase responses, so infection assessment must not depend on the expected CRP alone.
Normal is not exclusionary
Reference intervals describe populations; important inflammatory disease can remain within them when the clinical phenotype is strong.
Trends need a comparator
Serial values become meaningful only when timing, assay, treatment and the clinical state are sufficiently comparable.
