Glucocorticoids and steroid risk reduction

Use systemic and local glucocorticoids only for a defined inflammatory indication, prescribe a time-limited regimen with an exit strategy, and prevent adrenal, infectious, metabolic, skeletal and psychiatric harm.

Adrenal crisis or steroid-associated infection

Hypotension, collapse, vomiting, severe weakness, hypoglycaemia, hyponatraemia, fever or confusion in a steroid-exposed patient may represent adrenal crisis, severe infection or both; glucocorticoids can suppress typical inflammatory signs.

Action: Treat suspected adrenal crisis immediately with hydrocortisone 100 mg intravenously or intramuscularly, rapid isotonic saline and endocrine guidance, while obtaining cultures and treating sepsis without waiting for a cortisol result; never stop chronic glucocorticoid abruptly during acute illness.

01Purpose and principlesWhat the treatment does and how it fits into care.

Glucocorticoids alter transcription through the intracellular glucocorticoid receptor and produce rapid anti-inflammatory and immunosuppressive effects. They also reduce osteoblast function, increase bone resorption, raise glucose, retain sodium, redistribute fat, weaken muscle and skin, alter mood and suppress the hypothalamic-pituitary-adrenal axis. Benefit can appear within hours while cumulative harm grows with dose, duration, repeat courses and patient vulnerability.

The prescription must answer five questions: what inflammatory process is being treated, what organ or function is threatened, why glucocorticoid is needed now, what steroid-sparing treatment is starting, and when the dose will stop or taper. A vague instruction such as use as directed is unsafe. Use one named preparation and express any conversion carefully because anti-inflammatory potency and mineralocorticoid activity differ.

Doses are indication-specific. Prednisolone 5–10 mg once each morning may be used as a brief bridge in some inflammatory-arthritis plans, whereas immediately sight-threatening giant-cell arteritis or severe vasculitis requires a specialist high-dose pathway. Methylprednisolone 250–1000 mg intravenously daily for one to three days is reserved for selected organ-threatening disease; pulses are not routine flare therapy and demand infection, cardiovascular, glucose and psychiatric safeguards.

Adrenal suppression is not equivalent to Cushingoid appearance. NICE identifies prednisolone 5 mg daily or more for four weeks or longer as a dose that can produce tertiary adrenal insufficiency, but repeated evening dosing, potent preparations, drug interactions and combined inhaled, topical, injected or oral exposure modify risk. Patients need a steroid emergency card and clear sick-day rules; vomiting that prevents oral absorption is an emergency.

If adrenal crisis is suspected, treatment precedes confirmation. Give hydrocortisone 100 mg IV or IM immediately, restore circulating volume with isotonic saline, correct glucose and electrolytes, identify the precipitant and continue stress hydrocortisone, commonly 200 mg over 24 hours by infusion or 50 mg IV or IM every six hours, under acute and endocrine care. A pre-treatment cortisol can be stored if this does not delay therapy.

Tapering protects both disease control and adrenal recovery. Above physiological range, dose reduction is mainly limited by inflammatory relapse; near prednisolone 3–5 mg daily, endogenous cortisol recovery becomes central. Morning dosing supports circadian rhythm. Distinguish disease flare from glucocorticoid withdrawal syndrome and adrenal insufficiency using symptoms, examination, inflammatory disease markers and, when appropriate, an 8–9 am cortisol or Synacthen pathway directed by endocrinology.

Bone protection starts with exposure planning. Record previous fragility fracture, age, menopause, falls, smoking, alcohol, calcium intake and other secondary causes; calculate fracture risk and obtain DXA when it will refine management. NOGG recommends prompt bone-protective treatment in high-risk people starting glucocorticoids because fracture probability rises quickly. Adequate calcium, vitamin D, weight-bearing activity and falls reduction complement, but do not replace, anti-osteoporosis medicine when indicated.

Key points

  • Glucocorticoids are a bridge or rescue treatment, not a substitute for diagnosis and disease-modifying therapy; record the indication, dose, duration, taper and review date at initiation.
  • In rheumatoid arthritis, NICE permits short-term glucocorticoid bridging when starting a new conventional DMARD and short-term treatment for flares; continuing longer requires discussion of long-term complications and alternatives.
  • Use the lowest effective dose for the shortest period. A disease-specific regimen takes precedence over an improvised standard dose, particularly in giant-cell arteritis, vasculitis and organ-threatening connective-tissue disease.
  • Prednisolone 5 mg daily or more for four weeks or longer can suppress the hypothalamic-pituitary-adrenal axis; provide a steroid emergency card and sick-day guidance when risk criteria are met.
  • Never stop a prolonged systemic course abruptly. Tapering is faster above physiological range and slower near prednisolone 3–5 mg daily, with endocrine assessment if withdrawal symptoms, adrenal features or uncertain recovery develops.
  • Before and during treatment assess infection, vaccination, blood pressure, weight, glucose, mood, sleep, eyes and bone risk; prophylaxis is determined by dose, duration and comorbidity.
  • Glucocorticoid-induced fracture risk starts early and is partly independent of bone-mineral density; begin fracture assessment promptly when long-term exposure is expected, not after the first fracture.
  • Intra-articular injection requires secure anatomy, asepsis, consent, infection exclusion and a rehabilitation plan; it can still cause hyperglycaemia and transient adrenal suppression.
  • A normal temperature or CRP does not reliably exclude infection during higher-dose steroid treatment. Investigate the patient, not the inflammatory marker.
  • During vomiting, severe illness, surgery or trauma, patients at adrenal risk need stress-dose instructions and must carry enough information for emergency teams to act.
02Indications, selection and cautionsWho may benefit, who needs urgent treatment and important alternatives.
Expected anti-inflammatory response

Reduced swelling, stiffness, inflammatory pain and organ activity within the anticipated time supports effectiveness but does not prove the original diagnosis.

Cushingoid toxicity

Weight gain, facial rounding, proximal weakness, thin bruised skin, striae, hypertension and hyperglycaemia indicate cumulative exposure and need for steroid-sparing action.

Adrenal insufficiencyRed flag

Fatigue, anorexia, nausea, abdominal pain, postural dizziness, hypotension, hyponatraemia or hypoglycaemia during taper or illness suggests inadequate endogenous cortisol.

Masked infectionRed flag

New functional decline, delirium, focal pain, breathlessness, dysuria or shingles may be the main infection signal because fever and inflammatory markers can be attenuated.

Psychiatric toxicityRed flag

Insomnia and activation may precede mood lability, mania, psychosis, depression or suicidality, often after initiation or dose escalation.

Skeletal harm

Height loss, vertebral pain, low-trauma fracture, hip or groin pain, proximal muscle weakness and falls should prompt fracture or osteonecrosis assessment.

Red flags requiring action

  • Collapse, hypotension, vomiting, abdominal pain, hyponatraemia or hypoglycaemia during illness or after steroid interruption requires immediate adrenal-crisis treatment.
  • Fever, rigors, focal pain, breathlessness, confusion or zoster during systemic glucocorticoid treatment may indicate serious infection even when CRP or fever is blunted.
  • New severe agitation, insomnia, mania, psychosis, depression or suicidal thinking after a dose increase needs urgent mental-health and prescribing review.
  • Acute visual loss, severe eye pain or halos may indicate ocular hypertension, glaucoma or another ophthalmic emergency.
  • New back pain, height loss, hip pain after minor trauma or sudden limb pain can represent fragility fracture or osteonecrosis.
  • A hot swollen joint should be aspirated for infection when feasible before intra-articular steroid; a recent injection does not make subsequent sepsis benign.

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

    Send cell count, Gram stain, culture and crystal analysis; visible crystals do not exclude infection, and injection should wait when sepsis remains plausible.

Rapid systemic control while a disease-modifying treatment begins or during a clearly defined flare.

Prednisolone

For a selected inflammatory-arthritis bridge, a common specialist regimen is 5–10 mg orally once each morning for the shortest feasible period with an explicit taper; organ-threatening diseases use diagnosis-specific higher regimens and must not be inferred from this example.

Screen infection and monitor glucose, blood pressure, mood, eyes and bone. Courses causing adrenal suppression require taper, emergency card and sick-day cover; avoid abrupt withdrawal.

Emergency induction when rapid high glucocorticoid exposure is justified by threatened organ function.

Methylprednisolone intravenous pulse

For selected organ-threatening autoimmune disease, specialist protocols use 250–1000 mg intravenously once daily for one to three days, followed by a diagnosis-specific oral and steroid-sparing plan.

Exclude or treat infection, monitor rhythm, pressure, glucose, electrolytes, psychiatric effects and fluid status; the regimen is specialist-directed and not routine musculoskeletal flare treatment.

Life-saving cortisol replacement during suspected adrenal crisis or severe physiological stress in a patient unable to mount a response.

Hydrocortisone for adrenal crisis

Administer hydrocortisone 100 mg IV or IM without delay; continue 200 mg across each 24 hours by infusion, or 50 mg IV or IM every six hours, while acute illness persists.

Do not await cortisol results. Give isotonic fluid, treat hypoglycaemia and the precipitant, and seek endocrine guidance for subsequent taper to replacement dosing.

Local anti-inflammatory treatment for a confirmed injectable synovial or periarticular target after infection has been excluded.

Triamcinolone acetonide intra-articular injection

Use a product- and joint-specific dose; a common licensed range is 5–10 mg for small joints and up to 40 mg for a large joint, given once with repetition determined by response and specialist policy.

Use asepsis, avoid tendon substance and unstable joints, and counsel on post-injection flare, infection, skin or fat atrophy, transient hyperglycaemia and systemic adrenal exposure.

A common first-line antiresorptive option for glucocorticoid-associated fracture prevention when fracture risk and renal or gastrointestinal factors permit.

Alendronic acid

When oral bisphosphonate treatment is indicated, give 70 mg orally once weekly on an empty stomach with plain water, remaining upright and taking no food, drink or other medicine for at least 30 minutes.

Check renal function, calcium and vitamin D, swallowing or oesophageal disease and dental context; do not use in hypocalcaemia or if administration precautions cannot be followed.

06Complications, monitoring and follow-upAdverse effects, response and longer-term review.
  • At every review record current and cumulative dose, flare control, the intended next reduction and whether the steroid-sparing treatment is working; unexplained dose persistence requires a new plan.
  • Monitor weight, blood pressure and glucose early after initiation or dose escalation and at intervals determined by dose, duration and diabetes or cardiovascular risk.
  • Ask directly about infection, shingles, sleep, mood, psychosis, weakness, falls, bruising, visual symptoms and gastrointestinal bleeding because laboratory surveillance alone misses major toxicity.
  • Reassess fracture risk, adherence to bone protection, calcium and vitamin D status and new back or hip pain; repeat DXA according to risk and osteoporosis guidance rather than annually by default.
  • Verify that patients at adrenal risk hold a current steroid emergency card, understand sick-day dose escalation, possess adequate tablets and know that vomiting or collapse needs injectable treatment.
  • After an intra-articular injection, document clinical response and adverse effects before considering another; repeated failure should reopen diagnosis and rehabilitation rather than shorten the interval automatically.

Morning dosing respects physiology

Giving once-daily prednisolone in the morning generally reduces sleep disturbance and aligns exposure more closely with the normal cortisol rhythm.

Inflammation and adrenal symptoms overlap

Fatigue, pain and malaise during taper can represent disease relapse, withdrawal or cortisol deficiency, so a reflex dose increase can perpetuate harm.

Fractures occur before low BMD is obvious

Glucocorticoids impair bone quality and raise falls risk, meaning bone-mineral density alone underestimates early fracture probability.

Injection is systemic exposure

A local depot can transiently suppress the adrenal axis and raise glucose, especially after multiple sites or concurrent steroid routes.

Normal CRP can be false reassurance

Immunosuppression may attenuate inflammatory markers and fever; focal symptoms, physiology and cultures determine whether infection remains credible.

Stress dosing is replacement, not flare therapy

Extra hydrocortisone during severe illness prevents cortisol failure and should not be confused with escalation for rheumatic inflammation.
