Juvenile idiopathic arthritis in transition to adult care

Deliver planned, developmentally appropriate transition for young people with juvenile idiopathic arthritis, preserving disease control, uveitis surveillance, medicine safety, self-management and continuity across paediatric and adult services.

Time-critical presentation

Urgently assess a hot joint with fever, painful red or photophobic eye, acute neurological or cardiopulmonary symptoms, or systemic-JIA fever with falling blood counts and organ dysfunction. Transition arrangements must never become a barrier to same-day paediatric or adult emergency care.

JIA includes oligoarticular, polyarticular, systemic, psoriatic, enthesitis-related and other phenotypes. Some remit; many remain active or leave structural and psychosocial consequences. Adult teams need the original category, antibody and HLA status, uveitis history, growth and surgery details, and complete treatment chronology. Simply relabelling the disease as adult RA can lose important screening and biological context.

NICE transition guidance recommends early planning, a named worker, developmentally appropriate information and coordinated health and social support. Readiness includes understanding disease, medicines, appointments, monitoring, emergency contacts, consent, confidentiality and self-advocacy. Assess cognitive, communication and social needs and maintain reasonable adjustments. Family involvement changes gradually with the young person's wishes and capacity.

Clinical review separates activity, damage and development. Examine joints, entheses, spine, growth-related alignment and function; assess eyes, skin and systemic features. Review uveitis screening rather than asking only about red eyes because chronic JIA uveitis can be asymptomatic. Check laboratory monitoring, immunisation, infection, bone health, exercise, pain amplification and mental wellbeing.

Medication safety is vulnerable during transfer. Confirm prescriber, shared-care agreement, dose, weekly methotrexate day, biologic supply, blood-test owner and actions for infection or surgery. Discuss pregnancy, contraception and teratogenic drugs confidentially with all relevant young people. Plan flare access, university or travel supplies and communication with primary care before the paediatric team closes follow-up.

Adult inflammatory classification

Persistent childhood disease may resemble adult RA, spondyloarthritis or Still disease, but the original phenotype and treatment history remain clinically important.

Maintains control of persistent peripheral JIA phenotypes across transition when effective and tolerated.

Methotrexate with folic acid

Continue the verified specialist dose once weekly, commonly 7.5–25 mg in adult practice. Record one fixed dosing day and prescribe folic acid at least 5 mg once weekly on a different day.

Never prescribe or take it daily. Confirm risk-based FBC, renal and liver monitoring and interacting medicines; withhold it during a serious infection until recovery and clinical review. For maternal exposure, low-dose methotrexate must be stopped at least 1 month before planned conception and is not compatible with pregnancy or breastmilk exposure; accidental pregnancy needs immediate cessation, folic acid 5 mg/day and specialist fetal-risk review. BSR regards paternal exposure at no more than 25 mg/week as compatible; ensure confidential contraception and specialist pre-conception planning before transfer.

Biologic treatment for eligible persistent arthritis, enthesitis-related disease or uveitis under coordinated rheumatology and ophthalmology care.

Adalimumab

Continue the verified age-, weight- and indication-appropriate specialist regimen; adult maintenance is commonly 40 mg subcutaneously every other week.

Screen for tuberculosis and hepatitis and stop for a serious active infection with acute specialist review. Avoid live vaccines during treatment; review demyelinating disease, and do not use in NYHA III or IV heart failure. When exposure continues beyond 28 weeks of pregnancy, document the BSR plan to defer infant live vaccines until 6 months.

Persistent childhood inflammation can cause deformity, limb-length difference, growth disturbance, osteoporosis and lasting limitation into adult life.

Uveitis and visual loss

Undetected chronic anterior uveitis can produce cataract, glaucoma, synechiae and irreversible sight loss despite few ocular symptoms.

Systemic JIA can develop life-threatening cytokine activation with fever, cytopenia, coagulopathy, liver injury and neurological dysfunction.

It is a supported developmental process whose success is measured by continuity, capability and outcomes after the administrative transfer.

Quiet joints do not quiet eyes

Chronic anterior uveitis may be asymptomatic and follow a different course, so slit-lamp surveillance must remain visible.

Adult labels can lose history

The childhood phenotype, growth effects and prior uveitis or MAS remain relevant even when adult classification resembles RA or SpA.

Confidentiality supports adherence

Private time allows honest discussion of missed doses, alcohol, sexual health, pregnancy and mental health while families remain valued partners.

Administrative ownership is clinical safety

A clear prescriber, blood-result reviewer and first appointment prevents interruption of potent medicines and monitoring.
