Synopsis
Recognise sarcoidosis beyond the chest, secure proportionate tissue and microbiological evidence, identify cardiac, neurological, ocular, renal and calcium threats, and coordinate organ-specific treatment without mistaking granulomas for a diagnosis.
- Sarcoidosis is a diagnosis of compatible multisystem disease plus supportive histology when needed and exclusion of competing granulomatous causes; non-necrotising granulomas alone are not diagnostic.
- Map lung, lymph node, skin, eye, heart, nervous system, calcium, kidney, liver, gland, joint and constitutional features at baseline rather than treating the chest film as the whole disease.
- First safety priority is high-risk organ screening: ask about syncope, palpitations, visual loss and focal neurology, check ECG, calcium, renal and liver profiles and escalate abnormalities promptly.
Key red flags
Syncope, presyncope, sustained palpitations, new conduction block or ventricular arrhythmia requires urgent cardiac monitoring and sarcoid-aware electrophysiology assessment.
Unexplained AV block in a younger adult, ventricular ectopy, syncope, ventricular tachycardia or non-ischaemic cardiomyopathy should trigger urgent cardiac-sarcoid assessment.
Investigation priorities
Detect silent cardiac, metabolic, renal, hepatic and haematological involvement at diagnosis.
Management branches
Clinical or imaging findings raise a new multisystem sarcoidosis possibility.
- Screen immediately for cardiac, eye, neurological, calcium and renal danger while documenting thoracic, skin, gland and musculoskeletal features.
- Review infection, malignancy, occupational exposure, medicine and immune-deficiency mimics and select the least invasive site that can distinguish them.
Syncope, arrhythmia, visual loss, central neurological disease, severe calcium disturbance or renal decline occurs.
