Sarcoidosis as a multisystem inflammatory disorder

High-risk extrapulmonary sarcoidosis

Syncope, high-grade heart block, ventricular arrhythmia, acute heart failure, visual loss, severe uveitis, seizure, myelopathy, pituitary crisis, severe hypercalcaemia or rapidly declining kidney function can cause sudden death or irreversible disability.

Action: Admit or arrange same-day organ-specific assessment, monitor rhythm and physiology, treat immediate cardiac, neurological, ocular or metabolic instability, obtain infection and malignancy samples where safe, and start specialist immunosuppression when the probability and threat justify action without waiting for a low-yield serum marker.

Synopsis

Recognise sarcoidosis beyond the chest, secure proportionate tissue and microbiological evidence, identify cardiac, neurological, ocular, renal and calcium threats, and coordinate organ-specific treatment without mistaking granulomas for a diagnosis.

  • Sarcoidosis is a diagnosis of compatible multisystem disease plus supportive histology when needed and exclusion of competing granulomatous causes; non-necrotising granulomas alone are not diagnostic.
  • Map lung, lymph node, skin, eye, heart, nervous system, calcium, kidney, liver, gland, joint and constitutional features at baseline rather than treating the chest film as the whole disease.
  • First safety priority is high-risk organ screening: ask about syncope, palpitations, visual loss and focal neurology, check ECG, calcium, renal and liver profiles and escalate abnormalities promptly.

Key red flags

Syncope, presyncope, sustained palpitations, new conduction block or ventricular arrhythmia requires urgent cardiac monitoring and sarcoid-aware electrophysiology assessment.

Cardiac involvement

Unexplained AV block in a younger adult, ventricular ectopy, syncope, ventricular tachycardia or non-ischaemic cardiomyopathy should trigger urgent cardiac-sarcoid assessment.

Key medicines

PrednisoloneFor clinically important systemic disease, start 20–40 mg orally each morning for four to six weeks, then taper against objective organ response toward 5–10 mg daily and withdrawal over roughly six to twelve months.
Methylprednisolone intravenous pulseFor selected sight-, cardiac-, neurological- or renal-threatening disease, give 500–1000 mg intravenously once daily for three days under organ-specialist supervision before an oral and steroid-sparing plan.
