Synovial aspiration is both a diagnostic procedure and a stewardship intervention. The highest-value use is a new unexplained effusion where infection or crystals would change immediate care. Record onset, fever, bacteraemia risk, surgery, prosthesis, skin breach, immunosuppression, antimicrobial exposure, trauma and bleeding risk. Examine whether the effusion is accessible and whether cellulitis, anatomy or patient instability requires image guidance or operative sampling.
Normal synovial fluid is clear and viscous with few cells. Inflammation reduces viscosity and increases leucocytes, but categories overlap extensively. Septic arthritis can present with a lower count early, after antibiotics or in immunosuppression; crystal arthritis can produce a very high neutrophil count. Therefore interpretation combines fluid appearance, microscopy, Gram stain, culture, blood cultures, host factors and the direction of illness.
Crystal identification depends on suitable microscopy and observer expertise. Urate and CPP deposition may be incidental in some joints, yet intracellular crystals in a compatible flare support active crystal arthritis. Delayed examination can reduce crystal visibility, and a negative result does not fully exclude disease. Serum urate is a separate longitudinal test and can be normal during an acute gout episode.
Specialised requests should be purposeful. Mycobacterial or fungal culture, molecular tests, cytology and biochemical assays require laboratory discussion and adequate sample. Prosthetic-joint infection may need multiple tissue samples, prolonged culture and a coordinated surgical plan. Always document who will review preliminary and final results, because late culture growth can materially change treatment after discharge.

- Monosodium urate crystals are needle-shaped and strongly negatively birefringent; CPP crystals are commonly rhomboid and weakly positively birefringent when examined by trained staff.
- Crystals and infection may coexist, so every clinically suspicious aspirate still needs microbiological culture.
- Interpret a bloody aspirate using trauma, anticoagulation, bleeding disorder, fracture and procedural blood contamination rather than labelling it traumatic by appearance alone.
02Indications, selection and cautionsWhen it is useful, when urgency changes and important limitations.
Cloudiness, reduced viscosity and neutrophil predominance indicate inflammation but cannot distinguish bacterial infection from crystal or immune-mediated arthritis without culture and context.
Needle-shaped intracellular or extracellular crystals with strong negative birefringence support monosodium urate deposition when the clinical presentation is compatible.
Rhomboid or rod-shaped weakly positively birefringent crystals support calcium pyrophosphate deposition; absence does not eliminate CPP disease when handling or observer sensitivity is limited.
Uniformly bloody fluid may reflect haemarthrosis, fracture, anticoagulation, tumour or severe synovitis, whereas increasing blood through collection may indicate procedural contamination.
Thresholds, sample number and culture strategy differ around a prosthesis; coordinate aspiration and antibiotic timing with orthopaedics and microbiology.

Cell counts overlap
Counts are continuous biological data influenced by timing, host immunity and prior antibiotics; a memorable threshold should never overrule a dangerous clinical pattern.
Culture comes before convenience
When sample is scarce, a microbiologically useful specimen is more valuable than completing every routine field on an analysis panel.
Crystals require technique
Weak CPP birefringence is easy to miss, so observer experience and timely examination influence the apparent sensitivity of microscopy.
Blood changes the differential
True haemarthrosis raises trauma, coagulopathy and tumour as well as inflammatory causes, and may require imaging or haemostatic intervention.
The prosthetic joint is different
Diagnostic thresholds and operative sampling strategy depend on implant context; early specialist coordination protects both culture yield and surgical options.
