Gallstones are common and may remain silent. A scan finding is clinically useful only when anatomy and symptoms fit: biliary colic reflects temporary obstruction of the cystic duct, whereas persistent impaction and inflammation produce acute cholecystitis. Classic pain is steady rather than truly colicky, usually epigastric or right sided, can radiate to the back or right shoulder, and may follow meals without requiring that association.
A complete assessment asks when pain began, whether it fully resolves between attacks, and whether vomiting, fever, jaundice, altered urine or stool colour accompanies it. Examine during and after pain because tenderness may disappear with decompression. Cardiac, peptic, hepatic and pancreatic disorders remain relevant; discovering stones does not prove they caused atypical daily bloating or lower abdominal pain.
NICE recommends liver blood tests and ultrasound for suspected gallstone disease. Symptomatic gallbladder stones are treated definitively with laparoscopic cholecystectomy, while asymptomatic stones in a normal gallbladder and biliary tree need no treatment unless symptoms develop. During a waiting interval, supply usable analgesia and precise safety-netting rather than an unrealistic promise that dietary fat avoidance will prevent every attack.

Pancreatic inflammation
Persistent epigastric pain radiating to the back with raised lipase suggests pancreatitis, including disease triggered by migrating gallstones.
Additional chapter-specific clues
Inferior myocardial infarction, perforated ulcer, hepatitis and pancreatitis can imitate upper abdominal biliary pain and require targeted exclusion.

Ibuprofen for uncomplicated biliary pain
For an adult who can take an NSAID, give 400 mg orally with or just after food up to three times in 24 hours, leaving at least six hours between doses; maximum 1,200 mg in 24 hours and stop when the acute pain has settled.Avoid or seek prescriber advice with peptic ulcer or gastrointestinal bleeding, kidney, liver or heart disease, anticoagulation, NSAID-sensitive asthma or pregnancy. Acute cholecystitis
Sustained cystic-duct obstruction raises pressure, impairs mural perfusion and causes gallbladder inflammation that may progress to infection, necrosis or perforation.
Common-duct obstruction
Migrated calculi may block bile drainage, producing jaundice and sometimes ascending cholangitis as bacteria multiply behind the pressurised obstructed duct.
Gallstone pancreatitis
Transient ampullary obstruction can activate pancreatic inflammation, ranging from a self-limited illness to organ failure and necrosis.

- Record whether each attack completely resolves, because persistent pain is evidence against uncomplicated colic.

Steady pain, transient mechanism
The word colic describes temporary obstruction, yet biliary pain is usually sustained during an attack rather than fluctuating every few minutes.
Ultrasound is not causation
The more atypical and continuous the symptom pattern, the less confidently coincidental stones explain it.
Normal liver tests have limits
Cystic-duct obstruction can cause intense pain without cholestasis because the common bile duct remains patent.
Definitive care prevents recurrence
Removing the gallbladder addresses the source of symptomatic stones; duct clearance alone does not prevent later gallbladder events.
