01OverviewDefinition, clinical context and the essential points that orientate the chapter.
Chronic pancreatitis develops through persistent or recurrent pancreatic injury with fibrosis and loss of functional tissue. Pain may dominate the early history, but the patient’s burden can later include maldigestion, weight loss, diabetes, fatigue and impaired work or social life. Structural changes vary with disease stage; a normal early scan is not sufficient to dismiss a coherent clinical presentation. Equally, pain in a patient with pancreatic calcification is not necessarily caused by the pancreas on every occasion.
Treatment has several simultaneous aims: address the cause and modifiable exposures, restore adequate nutrition, investigate a treatable obstruction or local complication, and manage pain and glucose problems in a coordinated way. The extent of visible calcification does not measure the individual’s pain experience, and disappearance of pain does not prove that exocrine or endocrine function has recovered. Planned follow-up is therefore necessary even when the patient has fewer attacks and stops attending emergency care.

Environmental and metabolic injury
Alcohol exposure and smoking are relevant contributors, but risk is multifactorial. Metabolic abnormalities and recurrent pancreatic injury can also contribute, and a patient’s reported drinking should not replace an individual causal assessment.
Inherited, immune and structural causes
Genetic susceptibility, autoimmune inflammation and obstructed or abnormal duct anatomy can produce chronic disease. Age at presentation, family history and imaging guide further specialist assessment and determine whether relatives or associated organs need consideration.
03PathophysiologyThe causal sequence from the underlying abnormality to symptoms and harm.
- 1Fibrosis and functional loss
Repeated or sustained injury activates a fibro-inflammatory process with distortion of pancreatic tissue and ducts. Loss of acinar tissue reduces digestive capacity, while later endocrine impairment can produce pancreatitis-associated diabetes.
- 2Multiple pain mechanisms
Inflammation, ductal pressure, local complications and altered neural processing can contribute to chronic pain. These mechanisms help explain why pain severity does not follow the amount of calcification and why one intervention may not resolve every symptom.
- 3Maldigestion and metabolic interaction
Reduced digestive enzyme availability impairs nutrient breakdown, while pain or fear of diarrhoea may reduce intake. Coexisting diabetes adds management complexity because food absorption and consumption can vary, affecting glucose control and hypoglycaemia risk.
04Clinical features and red flagsSymptoms, examination findings, patterns of presentation and time-critical warnings.
Ask about repeated upper abdominal pain, previous acute attacks, eating-related symptoms and functional impact. Pain can have inflammatory, obstructive and neuropathic components, while peptic, biliary, intestinal and musculoskeletal disease remain possible. Document sleep, activity and existing analgesic use; a changing pattern needs a new explanation rather than automatic escalation of a long-standing prescription.
Oily, difficult-to-flush stools, bloating, excess gas and weight loss suggest exocrine insufficiency, but overt steatorrhoea is not required. Patients may conceal symptoms by avoiding food or fat and thereby worsen their nutritional deficit. Establish meal intake, dietary restrictions and muscle loss, and ask whether prescribed enzymes are actually taken with each relevant meal and snack.
Record alcohol intake and smoking, previous attacks, family history, age at onset, metabolic abnormalities and any obstructive or autoimmune features. Drinking alcohol does not establish that it caused the disease. Younger patients, an affected family and an unexplained course may need specialist genetic assessment; autoimmune disease requires a distinct diagnostic pathway rather than empirical steroids.
Progressive jaundice, a pancreatic mass, a new pain pattern or otherwise unexplained deterioration should prompt specialist investigation. Chronic pancreatitis can make imaging and tissue interpretation difficult, so an initially non-diagnostic sample does not automatically settle a concerning lesion. Pseudocyst, duct obstruction and vascular complications can also account for new symptoms and require their own treatment decisions.

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

Pancreatic structural imagingFirst step - Why
- Look for calcification, ductal abnormalities, atrophy, stones, complications and a possible mass.
- Interpretation and limitations
- CT is particularly useful for calcification. MRI/MRCP assesses ducts and parenchyma, but a normal MRI does not rule out mild chronic pancreatitis. When suspicion remains clinically important, specialist EUS or other directed assessment may be appropriate. Routine diagnostic ERCP exposes the patient to procedural risk without being the preferred first diagnostic test.

- Interpretation and limitations
- Faecal elastase on a formed stool can support exocrine insufficiency, but it must be interpreted with symptoms, nutrition and the underlying disease. A watery sample may produce a falsely low value. Review weight trend, dietary intake, muscle status and relevant biochemical deficiencies; normal BMI or a single normal nutrient result does not exclude malnutrition.

- NICE advises HbA1c monitoring at least every six months and bone density assessment every two years in adults with chronic pancreatitis. Assess symptomatic glucose disturbance sooner. Use the clinical and biochemical nutritional review at least annually, with shorter intervals for ongoing weight loss, deficiencies, changing enzyme requirements or treatment response.

- Explain the disease and investigate findings that would change treatment.
- Interpretation and limitations
- Choose metabolic, autoimmune, genetic or additional pancreatic investigations according to the history and imaging. A focal lesion may require EUS-guided tissue and multidisciplinary interpretation. Genetic testing and cancer surveillance should follow the relevant specialist pathway; NICE suggests considering annual pancreatic cancer monitoring for hereditary pancreatitis, rather than routine scanning of every patient with chronic disease.
06Differential diagnosisRealistic alternatives and the features that help distinguish them.
Recurrent acute pancreatitis
Discrete acute attacks may precede or coexist with chronic disease, but recurrent attacks alone do not demonstrate established fibrosis. Investigate their cause and assess persistent structural or functional abnormalities in the appropriate clinical setting.
Pancreatic cancer
A mass, progressive jaundice or an unexplained change in symptoms requires evaluation even when chronic pancreatitis is already known. Fibrosis may make both imaging and tissue interpretation more difficult and increase the need for multidisciplinary reassessment.
Other upper abdominal disease
Biliary disease, peptic pathology and non-pancreatic abdominal disorders can cause pain in someone with pancreatic calcification. Reconsider them when the symptom pattern or response is inconsistent with the presumed pancreatic mechanism.

01Worked caseWeight loss behind apparently quieter diseaseFirst stepA 59-year-old man with calcific chronic pancreatitis reports fewer painful attacks but has lost 6 kg in four months and stopped eating lunch because of oily stools.+
- 1CT shows established calcific disease without a new mass or biliary obstruction. He has a formed-stool elastase of 72 micrograms/g, reduced muscle bulk and inadequate intake. The team identifies pancreatic maldigestion and nutritional restriction as active problems despite improved pain, checks nutritional blood tests and glucose, and arranges specialist dietetic support.
- 2He starts Creon 25,000 capsules at two with each main meal and one with each snack, spread through eating, with the formulation and safety counselling described below. The dietitian restores adequate energy and protein without a routine low-fat restriction. At four weeks his stools are less oily, he again eats lunch and his weight has increased by 1.4 kg; residual symptoms prompt a timing and dose review.
- 3The team records a continuing enzyme prescription, nutritional reassessment and the six-month HbA1c and two-year bone-density schedule, adjusted for findings requiring earlier follow-up. His pain review remains separate: enzymes have treated maldigestion, and a future change in pain or new jaundice will trigger investigation rather than being attributed to known calcification.

- 1Explain the cause as far as it is known and support smoking cessation and avoidance of alcohol, with appropriate addiction care when needed. Review nutritional intake and exocrine insufficiency, restore food intake with enzyme support where indicated, and identify financial, psychological or practical barriers to sustained treatment.
- 2AlternativeAssess pain mechanism and impact, review current medicines for benefit and adverse effects, and include pancreatic and pain specialists when pain remains disabling. Look for a treatable obstructed duct, pseudocyst or alternative cause before increasing analgesia. Psychological and functional support can accompany physical treatment; pain is not dismissed because imaging changes are modest.
- 3First lineFor chronic pancreatitis causing painful main-duct obstruction, consider surgical treatment first line under NICE guidance. If surgery is unsuitable and dominant stones are responsible, consider extracorporeal shockwave lithotripsy. The choice depends on anatomy, fitness and patient priorities, with specialist endoscopic options discussed in that context rather than an automatic ERCP-first sequence.

- 1Reassess the actual change and exclude urgent infection, bleeding, obstruction or malignancy where suggested. Review food intake, enzyme access and administration, drug effects, alcohol and smoking without blame. Persistent diarrhoea despite correctly used adequate enzymes needs assessment for additional gastrointestinal causes rather than repeated unexamined dose increases.
- 2If diabetes develops, recognise pancreatitis-associated type 3c disease and involve a clinician experienced in its nutrition and treatment issues. NICE advises reassessing potential benefit from insulin every six months; symptomatic hyperglycaemia requires earlier assessment. Match glucose treatment to variable intake and malabsorption, and teach recognition and management of hypoglycaemia.
- 3Use observed outcomes to revise the plan: ability to eat, weight and strength, pain-related activity, nutrient results and glucose pattern. Arrange a defined review date and responsibility for outstanding investigations. A normal weight or improvement in one symptom should not cancel surveillance of other pancreatic functions.

Creon 25,000 capsules (porcine pancreatin)
For adult exocrine insufficiency, start 50,000 lipase units with a main meal (two capsules) and 25,000 with a snack (one capsule), distributed through eating. Increase gradually after checking response and administration. Swallow whole; if opened, take intact granules immediately with acidic soft food or fluid of pH below 5.5. Never chew or crush. Review additional causes when requirements exceed 100,000 units per meal or symptoms persist at 10,000 units/kg/day.Do not use with porcine-pancreatin or excipient hypersensitivity; stop and obtain urgent care for an allergic reaction. Maintain hydration and ensure no granules remain in the mouth. Assess new or unusual abdominal symptoms, especially at high doses: fibrosing colonopathy has been reported in cystic fibrosis, particularly above 10,000 lipase units/kg/day. Exercise caution with renal impairment, gout or hyperuricaemia; no routine renal or hepatic dose schedule is specified in this SmPC. Extreme overdose with raised urate needs enzyme withdrawal and rehydration. Pregnancy requires caution; breastfeeding is compatible, with doses sufficient for nutrition. The July 2026 product information advises immunosuppressed patients receiving high daily doses to seek medical attention for viral-hepatitis symptoms because possible porcine-source HEV transmission cannot be eliminated. Record product name and batch for traceability. No interaction studies have been performed. Store below 25°C, tightly closed, and use within six months after opening.
08ComplicationsImportant consequences, why they occur and why they matter clinically.
Exocrine insufficiency and malnutrition
Impaired enzyme delivery can lead to fat maldigestion, weight loss and nutritional deficiencies. Restrictive eating and muscle depletion can worsen the consequences even before overt steatorrhoea becomes apparent.
Endocrine and skeletal consequences
Pancreatitis-associated diabetes and reduced bone density contribute to long-term morbidity. Their detection requires planned monitoring because early changes may not produce symptoms that bring the patient back to clinic.
Local and neoplastic problems
Duct stones, pseudocysts, biliary or duodenal obstruction and vascular complications can alter the clinical course. Pancreatic malignancy is a further concern, particularly when new features do not fit the established pattern of disease.
09Monitoring and follow-upTreatment response, safety checks and longer-term review.
- Review intake, stool symptoms, weight trajectory, muscle function and treatment use during nutritional recovery. Measure relevant deficiencies according to clinical context and repeat abnormal results to demonstrate response; symptom improvement alone does not establish that nutrient stores are adequate.
- For stable adult chronic disease, ensure at least annual assessment of exocrine function and malnutrition, HbA1c at least every six months and bone density every two years. Increase review frequency when symptoms, biochemical abnormalities or treatment changes require it.
- Assess pain treatment by function, sleep and tolerability as well as a pain score. If opioids are already prescribed, check sedation, constipation, dependence and whether there is a clear continuation or reduction plan, using the current product and pain-service guidance for changes.
- Provide a route for rapid reassessment of jaundice, unexpected further weight loss or a new pain pattern. For hereditary pancreatitis, discuss the specialist annual cancer-monitoring recommendation; explain that surveillance and investigation of a new symptom are different decisions.

Clinical suspicion survives a normal early scan
Advanced calcific disease is easier to recognise than early fibrosis. A normal MRI reduces some concerns but cannot exclude mild chronic pancreatitis in a convincing presentation. Further investigation should answer a clinical question rather than turn a non-specific symptom into a diagnosis through repeated tests.
Nutrition requires more than a BMI
Body weight can appear acceptable while muscle is depleted, micronutrients are low or dietary restriction is severe. Ask what the patient actually eats over a day and how symptoms influence those choices. Correct enzyme timing and adequate food can improve the usefulness of an otherwise sound nutrition prescription.
Pain and enzyme function can diverge
The amount of visible pancreatic damage does not predict pain reliably, and pain may lessen while endocrine or digestive function continues to fail. Follow these domains independently. Enzyme therapy addresses maldigestion; evidence does not support prescribing it simply to relieve pancreatic pain in a patient without insufficiency.
An obstructed duct changes the discussion
Persistent pain associated with main-duct obstruction is a specific treatment problem. NICE’s consideration of first-line surgery prevents an assumption that every patient must first undergo repeated endoscopic procedures. Discuss the probable benefits and burdens of the feasible options with the patient and specialist team.
