Prescribing begins with a credible mechanism and a treatment objective. A man seeking faster relief from difficult voiding may benefit from reduced smooth-muscle tone. A man with an enlarged gland and substantial progression risk may also value reducing future acute retention and surgery. These effects occur on different timescales. Neither medicine family drains a dangerously overfilled bladder, treats a urinary infection or substitutes for investigation of cancer or a urethral stricture.
The relevant trials and recommendations largely concern adult men with non-neurogenic LUTS associated with benign prostatic enlargement. They do not make alpha blockade a routine prescription for women with retention, cauda equina dysfunction or a recent reconstructed outlet. Review existing antihypertensives, falls, liver disease, sexual priorities and eye surgery before starting. Explain the follow-up plan at the same time as the dose so that delayed benefit, adverse effects and lack of response have an agreed interpretation.

Moderate or severe bothersome male LUTS after unsuitable or unsuccessful conservative measures supports considering alpha blockade. A very large gland is not required for a symptomatic response. Establish whether dizziness or postural blood-pressure problems would make the treatment unsafe.
Larger prostate volume and an appropriate PSA pattern support a greater likelihood of progression. Under CG97, estimated size above 30 g or PSA above 1.4 ng/ml plus high progression risk supports offering a 5-alpha-reductase inhibitor. Cancer assessment remains distinct from using PSA as a size proxy.
Falls, presyncope, poor intake, orthostatic symptoms or planned cataract surgery may change alpha-blocker selection or timing. Low libido, erectile difficulty, altered ejaculation and mood history should be discussed without assuming that a patient will volunteer them spontaneously.
No improvement despite adherence may reflect a stricture, underactive detrusor, predominant overactive bladder or nocturnal polyuria. Recurrent retention, infection or renal deterioration is an escalation signal rather than a reason for indefinite empirical dose escalation.

Tamsulosin hydrochloride Zentiva prolonged-release capsule
400 micrograms by mouth once daily after breakfast or the first daily meal; swallow whole without crushing. Use this daily dose, reviewing benefit at 4–6 weeks.Contraindicated with orthostatic hypotension history or severe hepatic insufficiency. No routine renal adjustment; caution if creatinine clearance is below 10 ml/min. Avoid initiation before scheduled cataract or glaucoma surgery and disclose previous use. Strong CYP3A4 inhibitors must not accompany this treatment in a poor CYP2D6 metaboliser; other strong or moderate CYP3A4 combinations require caution.
Finasteride 5 mg film-coated tablet
5 mg orally once daily with or without food; do not crush or divide. Assess benefit after at least six months and continue long term if the indication remains.No renal dose reduction is required; hepatic data are limited. Explain persistent sexual symptoms, psychiatric effects and prompt medical contact for mood changes. Pregnant people must avoid broken tablets. After six months, double PSA for comparison with untreated ranges and investigate sustained rises.
Dutasteride, Avodart 0.5 mg soft capsule
500 micrograms orally once daily, swallowed whole with or without food; allow up to six months for response. Use instead of finasteride, with long-term continuation based on benefit.Avoid in severe hepatic impairment, women, children and patients allergic to soya, peanut or other listed constituents. Renal adjustment is not anticipated. Consider CYP3A4 interactions, avoid leaking-capsule contact, establish six-month PSA baseline, and seek prompt assessment for mood or sexual changes.
06Feedback, follow-up and evidenceReview outcomes, seek feedback and identify what to improve.
- Review alpha-blocker symptoms and tolerability within 4–6 weeks; assess standing symptoms before issuing a longer supply to someone at risk of falls.
- For a 5-alpha-reductase inhibitor, review at 3–6 months and subsequently every 6–12 months when stable, including adherence and the person’s continuing treatment objective.
- For finasteride, a sustained PSA rise deserves evaluation; for dutasteride, compare against the six-month baseline and lowest subsequent value even when the result remains within an untreated reference interval.
- Ask about mood and sexual function during follow-up and make a plan for reported problems. Counselling should not be limited to a leaflet handed out at the first prescription.

Capsule versus tablet
Food instructions for a modified-release capsule cannot be copied automatically to every tamsulosin tablet. Check the supplied product and explain how to swallow it without damaging its release characteristics.
Eye surgery history
Intraoperative floppy iris syndrome can occur after previous tamsulosin exposure. A last-minute stop does not reliably remove the risk; the ophthalmic team needs the medication history to plan safely.
Delayed pharmacology
Dutasteride has a long terminal half-life, so exposure and some effects may persist after stopping. An adverse-effect review should consider timing without promising that everything will resolve immediately.
Dual pathway discussion
Combining drugs can be reasonable when symptomatic relief and progression prevention both matter. A fixed combination may simplify dosing but can obscure which component is causing an adverse effect or remains necessary.
