Cardiovascular risk reduction in PAD

Reduce myocardial infarction, stroke, limb events and mortality by treating peripheral arterial disease as established systemic atherosclerotic cardiovascular disease.

New vascular event

Acute chest pain, focal neurological deficit or sudden limb ischaemia requires the relevant emergency pathway rather than a routine prevention review.

Action: Assess immediately, activate the appropriate acute cardiac, stroke or vascular service, and reconcile antithrombotic therapy without delaying reperfusion decisions.

01Role and principlesWho benefits and the main preventive aims.

A diagnosis of lower-limb PAD identifies established atherosclerotic cardiovascular disease. The leg may remain stable while the patient experiences myocardial infarction, stroke or vascular death, so prevention is a core PAD treatment rather than an optional addition to symptom care. Discuss absolute benefit, bleeding and medicine burden in terms the patient can use, then document who will monitor each intervention.

A practical plan combines smoking cessation, lipid lowering, antithrombotic treatment, blood-pressure management, diabetes care, exercise, diet and attention to kidney disease. National NICE recommendations define lipid and selected antithrombotic options; local formularies determine product selection and shared-care arrangements. Coexisting atrial fibrillation, recent coronary intervention, stroke, renal impairment or bleeding can change the regimen and should trigger deliberate reconciliation rather than automatic stacking of medicines.

Established atherosclerotic disease

Symptomatic PAD, previous lower-limb revascularisation or objective arterial disease places the patient in a secondary-prevention population regardless of a calculated primary-risk score.

Polyvascular burden

Ask about angina, myocardial infarction, TIA or stroke, carotid disease and prior arterial procedures because multiple territories increase ischaemic risk and complicate antithrombotic choice.

Bleeding vulnerability

Previous gastrointestinal or intracranial bleeding, anaemia, frailty, falls, kidney or liver disease, alcohol excess and interacting medicines alter antithrombotic net benefit.

Treatment intolerance

Distinguish true adverse effects from non-adherence, access barriers or misconceptions, checking timing, interactions and rechallenge options before abandoning effective prevention.

Acute event symptomsRed flag

New chest pain, focal neurological deficit or sudden painful cold limb is an emergency and must not wait for routine risk-factor review.

Limb progressionRed flag

New rest pain, ulceration or gangrene warrants urgent vascular assessment even while cardiovascular preventive treatment continues.

Red flags requiring action

  • Sudden limb pain, coldness, sensory loss or weakness requires immediate vascular assessment.
  • New focal neurological deficit or acute coronary symptoms require emergency assessment through their dedicated pathways.
  • Active major bleeding, melaena, haematemesis or intracranial symptoms require urgent review of antithrombotic therapy.
  • Rapidly worsening rest pain, ulceration or gangrene needs urgent limb assessment alongside prevention.

Investigation order

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

    Why
    Quantify atherogenic cholesterol and establish safety information before intensive lipid lowering.
    Interpretation and limitations
    Use total, HDL, non-HDL and LDL cholesterol to establish a baseline and assess response. Mild stable transaminase elevation below three times the upper limit of normal does not automatically preclude statin treatment under NICE guidance; investigate clinically significant abnormalities.

    Repeat lipid profile and liver transaminases
    Why
    Confirm response, adherence and early hepatic tolerance after treatment changes.
    Interpretation and limitations
    NICE recommends lipids and transaminases at two to three months after starting or changing lipid treatment. Recheck transaminases at twelve months, then only if clinically indicated; failure to reach target should prompt adherence, dose and add-on review.

    Blood pressure including postural assessment when indicated
    Why
    Detect and safely treat hypertension without overlooking orthostatic symptoms or frailty.
    Interpretation and limitations
    Confirm persistent hypertension with the recommended clinic and out-of-office pathway where appropriate. Apply current NICE targets to age, comorbidity and standing pressure; PAD does not justify ignoring symptomatic hypotension.

    HbA1c or glucose and renal function
    Why
    Identify diabetes control and renal constraints relevant to vascular risk and prescribing.
    Interpretation and limitations
    Interpret against the individual diabetes plan and eGFR trend. Kidney impairment increases both vascular and bleeding risk and may alter medicine eligibility, monitoring and dose, but does not remove the need for prevention.

    Full blood count and bleeding review
    Why
    Establish haemoglobin and platelet context before or during antithrombotic treatment.
    Interpretation and limitations
    Unexplained anaemia, thrombocytopenia or falling haemoglobin requires evaluation rather than reflex continuation or permanent cessation. Relate results to symptoms, renal function, medicines and the urgency of the vascular indication.

  1. 1Check adherence, administration, diet, alcohol, interacting drugs, thyroid or other secondary causes and whether symptoms truly track statin exposure.
  2. 2AlternativeMaximise tolerated statin intensity, using a lower dose or alternative statin when adverse-effect risk, interaction or preference prevents atorvastatin 80 mg.
  3. 3For secondary prevention, aim for LDL cholesterol at or below 2.0 mmol/L or non-HDL cholesterol at or below 2.6 mmol/L under current NICE guidance.

  3. 3Use NICE technology-appraisal eligibility for any rivaroxaban-plus-aspirin vascular regimen and involve the relevant specialist when indications compete.
  4. 4Document the chosen regimen, review or stop point and advice for bleeding, procedures and missed doses.
04Smoking treatmentTurn advice into an assisted quit attemptThe patient currently smokes or has recently stopped and wants relapse-prevention support.
  1. 1Ask about dependence, previous attempts, triggers, preferences, mental health, pregnancy potential and current medicines.
  2. 2Offer behavioural support with a licensed stop-smoking medicine suited to the person and the current NICE tobacco guideline.
  3. 3Arrange early follow-up to manage adverse effects, lapses and adherence, treating a lapse as information for the next attempt rather than failure.
05Medicines and treatment safetyRegimens, contraindications and review points.
Lowers LDL cholesterol and reduces systemic atherosclerotic cardiovascular events in established PAD.

Atorvastatin

Offer 80 mg orally once daily for secondary prevention; use a lower dose when interactions, adverse-effect risk or patient preference require it.

Check baseline lipids and liver transaminases, pregnancy status where relevant, interacting medicines and unexplained muscle symptoms. Review two to three months after starting or changing treatment. The NICE page records that this recommendation involved off-label use in December 2023, so confirm current product authorisation when prescribing.

A NICE-recommended antiplatelet option for people with peripheral arterial disease.

Clopidogrel (Milpharm 75 mg tablets)

75 mg orally once daily when selected for long-term prevention of occlusive vascular events in PAD.

Do not use in active pathological bleeding or severe hepatic impairment. Review anaemia, bleeding history, other antithrombotics and procedure plans. Concomitant omeprazole or esomeprazole is discouraged because reduced CYP2C19 activation can weaken the antiplatelet effect.

Provides additional LDL reduction when the secondary-prevention target is not reached or statins cannot be used.

Ezetimibe (Ezetrol 10 mg tablets)

10 mg orally once daily, alone when statins are contraindicated or not tolerated, or added to maximally tolerated statin treatment.

Ezetrol is not recommended in moderate or severe hepatic impairment. Monotherapy should be used during pregnancy only if clearly necessary and must not be used during breastfeeding. Statin combination is contraindicated during pregnancy, breastfeeding, active liver disease or persistent unexplained transaminase elevation. Check liver tests with a statin, assess new muscle symptoms promptly, and review ciclosporin and warfarin interactions.

06Targets, monitoring and follow-upResponse, safety and longer-term review.
  • Repeat full lipid profile and liver transaminases two to three months after starting or changing lipid-lowering treatment.
  • Check liver transaminases again at twelve months after statin initiation, then only when clinically indicated.
  • Review blood pressure, smoking, diabetes control, kidney function, physical activity and adherence at planned vascular or primary-care reviews.
  • Ask directly about bruising, gastrointestinal bleeding, neurological symptoms, muscle symptoms and every non-prescription anti-inflammatory medicine.
  • Reconcile antiplatelets and anticoagulants after hospital discharge, coronary intervention, stroke, bleeding or a planned invasive procedure.
  • Monitor feet and walking symptoms, because systemic risk reduction does not replace urgent assessment of new limb threat.

Primary-risk calculators are unnecessary

Once PAD is established, the patient already has clinical cardiovascular disease; treatment should not be withheld because a primary-prevention risk score appears modest.

Target and intensity are linked

NICE begins secondary prevention with atorvastatin 80 mg and also uses an LDL or non-HDL target to trigger adherence, dose and add-on review.

More antithrombotic is not automatically better

Atrial fibrillation, recent stenting and polyvascular disease create competing indications, while duplicate therapy can sharply increase bleeding; name each indication and duration.

Rivaroxaban has restricted eligibility

NICE TA607 permits rivaroxaban plus aspirin within its marketing authorisation for selected high-risk coronary or symptomatic peripheral artery disease after bleeding-risk assessment; it is not routine for every PAD patient.

Limb and life outcomes coexist

Walking therapy and revascularisation address function and limb events, while lipid, antithrombotic and smoking treatment primarily reduce systemic vascular harm; both plans are needed.

    Using a primary-prevention calculator to decide whether a person with established PAD deserves secondary prevention.

    Prescribing multiple antiplatelet or anticoagulant medicines without a named indication, duration and bleeding review.
