Carotid stenosis after TIA or stroke

Recognise symptomatic extracranial carotid disease, start safe early prevention, and identify patients who need urgent endarterectomy assessment.

Treat the brain event first

New or ongoing focal neurological deficit is an acute stroke emergency, even when carotid stenosis is already known.

Action: Activate the hyperacute stroke pathway immediately for brain imaging and reperfusion assessment; carotid prevention must not delay thrombectomy or thrombolysis decisions.

Symptomatic carotid stenosis means that an ipsilateral retinal or cerebral ischaemic event can reasonably be attributed to an extracranial internal carotid atherosclerotic lesion. Typical events include amaurosis fugax, hemispheric TIA and non-disabling ischaemic stroke. A coincidental bruit or stenosis on the opposite side does not establish causality. The clinician must connect symptom territory, timing and vascular anatomy while still investigating cardiac and intracranial alternatives.

Risk of recurrent stroke is front-loaded after a TIA or minor stroke, which makes urgent assessment and early prevention important. In neurologically stable people with 50–99% stenosis by NASCET criteria, CEA can remove an embolic plaque and reduce later ipsilateral stroke. Benefit rises with stenosis severity but is modified by time since symptoms, disability, infarct size, anatomy, comorbidity and the local procedural stroke/death risk. Near-occlusion and complete occlusion need specialist interpretation rather than simple application of a percentage threshold.

Carotid prevention is distinct from hyperacute stroke treatment. A patient with current deficit first needs acute stroke imaging and reperfusion assessment. Once stable, carotid imaging and the endarterectomy pathway run quickly alongside antiplatelet treatment, lipid lowering and risk-factor control. Major infarction or haemorrhage can make very early surgery hazardous because restored pressure may precipitate haemorrhagic transformation.

  • Continue long-term vascular prevention after the acute episode, including antithrombotic therapy, high-intensity lipid lowering, blood-pressure control and smoking support.

Atherosclerotic plaque

Lipid-rich and fibrotic plaque develops most often at the carotid bifurcation under the influence of smoking, hypertension, diabetes, dyslipidaemia, age and systemic atherosclerotic susceptibility.

Competing embolic sources

Atrial fibrillation, aortic plaque and intracranial arterial disease can coexist with carotid stenosis, so anatomical association must be tested against clinical territory and timing.

    Artery-to-artery embolism

    Plaque rupture, surface thrombus or friable material releases emboli into ipsilateral retinal or cerebral arteries, producing transient ischaemia or infarction even without critical flow limitation.

    Haemodynamic compromise

    Severe stenosis or near-occlusion can reduce distal perfusion, particularly when collateral supply is poor, making symptoms vulnerable to hypotension and impaired cerebrovascular reserve.

    Front-loaded recurrence

    An active symptomatic plaque carries its greatest recurrent embolic risk soon after the presenting event, while successful plaque removal offers less absolute benefit as treatment is delayed.

Retinal ischaemia

Transient monocular visual loss or retinal artery occlusion can localise to the ipsilateral carotid circulation and counts as symptomatic disease.

Hemispheric TIA

Contralateral face or limb weakness, sensory loss, dysphasia or neglect resolving completely suggests an anterior-circulation embolic event.

Non-disabling stroke

A recent infarct with stable useful function may still allow early CEA after brain imaging and multidisciplinary assessment.

Active stroke deficitRed flag

Persistent or evolving focal neurological loss requires hyperacute stroke assessment before any preventive carotid decision.

High-risk brain injuryRed flag

Large territorial infarction, haemorrhage, impaired consciousness or major disability raises perioperative haemorrhage risk and can justify deferral.

Incidental disease

A bruit or stenosis without a relevant ipsilateral event is asymptomatic disease and follows a different risk-benefit pathway.

Red flags requiring action

  • Ongoing weakness, aphasia, neglect, visual loss or reduced consciousness requires immediate acute stroke assessment.
  • A fluctuating or repeatedly recurring deficit may represent unstable cerebral perfusion or continuing embolisation and needs urgent specialist review.
  • A large infarct, haemorrhagic transformation, major disabling deficit or impaired consciousness changes the safety and timing of carotid intervention.

Cardioembolic stroke

Atrial fibrillation, ventricular thrombus or valvular disease can produce cortical or retinal embolism and changes long-term antithrombotic management if it better explains the event.

Intracranial arterial disease

Intracranial stenosis or occlusion can cause the same hemispheric territory, requiring intracranial vascular imaging and a different intervention strategy from extracranial CEA.

Stroke mimics

Migraine aura, focal seizure, hypoglycaemia and functional neurological symptoms may resemble TIA; positive spreading symptoms or metabolic findings require careful specialist interpretation.

Provides immediate antiplatelet treatment while diagnosis, carotid imaging and definitive secondary prevention are arranged.

Aspirin

For suspected TIA, give 300 mg orally once daily immediately unless contraindicated; after acute ischaemic stroke, give 300 mg within 24 hours once haemorrhage is excluded and continue to 14 days or earlier discharge review.

Check active bleeding, aspirin hypersensitivity, previous severe aspirin dyspepsia, anticoagulants and planned reperfusion or surgery. Use a safe oral, enteral or rectal route after swallowing assessment; add a proton-pump inhibitor for previous aspirin-associated dyspepsia after checking interactions. After intravenous thrombolysis, withhold antiplatelets for the first 24 hours; reassess and obtain follow-up brain imaging excluding haemorrhage before initiation.

Reduces early recurrent non-cardioembolic events in selected minor stroke or TIA and supports longer-term antiplatelet prevention.

Clopidogrel

For the selected product, start within 24 hours of minor ischaemic stroke (NIHSS ≤3) or moderate-to-high-risk TIA (ABCD2 ≥4): clopidogrel 300 mg orally once as a loading dose, then 75 mg once daily with aspirin 75–100 mg once daily for 21 days, followed by single antiplatelet therapy. This eligibility criterion must not be used to delay any suspected-TIA referral; peri-CEA use follows the specialist carotid protocol.

Do not apply this product DAPT regimen to non-minor stroke, planned thrombolysis, anticoagulation or CEA without specialist direction. Contraindications include active pathological bleeding and severe hepatic impairment. Review bleeding, blood count symptoms, interacting anticoagulants or NSAIDs, and avoid routine omeprazole or esomeprazole because CYP2C19 inhibition can reduce activation.

High-intensity lipid lowering reduces recurrent vascular events as part of comprehensive secondary prevention after TIA or stroke.

Atorvastatin

For established cardiovascular disease, offer atorvastatin 80 mg orally once daily unless drug interactions, adverse-effect risk or patient preference indicate a lower dose; check response after 2–3 months.

Check baseline liver function, interactions, pregnancy status and prior muscle toxicity; use a lower starting dose when risk or interactions warrant it. Measure transaminases and lipids at 2–3 months, transaminases at 12 months, and creatine kinase only if unexplained muscle symptoms develop.

Recurrent ischaemic stroke

Further embolisation can cause disabling or fatal infarction, particularly in the first days after a symptomatic event when assessment or intervention is delayed.

Retinal infarction

Repeated ophthalmic emboli may progress from transient monocular visual loss to permanent retinal artery occlusion and lasting visual disability.

Procedural harm

CEA can cause perioperative stroke, myocardial infarction, cranial nerve injury, neck haematoma or death; benefit depends on keeping local complication rates acceptably low.

  • Observe closely for recurrent TIA or stroke while awaiting intervention; any new deficit reactivates the emergency stroke pathway.
  • Record neurological status, functional independence and brain-imaging evolution because these determine whether early CEA remains safe.
  • Track the referral date, last-symptom date, imaging convention and planned procedure date to expose avoidable delay beyond 14 days.
  • Review bleeding, adherence and the intended stop or transition point for any short-course dual antiplatelet treatment.
  • Check blood pressure, smoking status, HbA1c when relevant and lipid response, aiming for LDL at most 2.0 mmol/L or non-HDL at most 2.6 mmol/L under NICE secondary prevention guidance.

Symptomatic means attributable

A recent ipsilateral event must fit the carotid territory. Finding plaque after dizziness, syncope or a contralateral event does not automatically make that plaque symptomatic.

The denominator matters

NASCET compares the narrowest residual lumen with the normal distal internal carotid artery. ECST uses the estimated original bulb, so percentages are not interchangeable.

Benefit is time sensitive

Most preventable recurrence occurs early. A theoretically suitable operation delayed for weeks loses benefit, which is why urgent imaging and parallel work-up matter.

Occlusion is different

A completely occluded internal carotid artery is not a very severe stenosis waiting for routine CEA. Confirm the distinction from near-occlusion and obtain specialist advice.

Mechanisms can coexist

Atrial fibrillation and carotid stenosis may coexist. Determine the most plausible stroke mechanism and preserve independent anticoagulation indications rather than treating one scan result in isolation.
